Permanent hypoparathyroidism developing post-thyroidectomy.

A case of permanent hypocalcaemia following a subtotal thyroidectomy for a simple multinodular goiter in a 35 year -- old married teacher is presented. This further confirms the need for utmost precaution during thyroid surgery to prevent the damage or inadvertent removal of the parathyroid glands and its consequential complications. It also throws more light on the problem faced in the management of hypocalcaemia vis a vis patient compliance and availability of proper medications in this environment.

Typhoid ileal perforation in Nigerian children: an analysis of 106 operative cases.

Typhoid ileal perforation (TP) is a major problem in developing countries and carries a high mortality. The purpose of this retrospective study from Nigeria was to review the outcome in children less than 15 years of age who underwent surgery for TP from 1984 to 1999. Demographic data, clinical features, results of investigations, findings at surgery, postoperative course, and complications were recorded. There were 55 boys and 51 girls. The median age at presentation was 10 years (range 3-14). The surgically confirmed perforation rate was 11.0%. The clinical features in children older than 5 years were similar to already documented patterns in the literature. In children less than 5 years old the predominant symptoms were fever and vomiting only. Therefore, in the very young a high index of suspicion is required to avoid delay in diagnosis. Ninety eight patients (92.5%) had simple double-layer closure of the perforation. The mean hospital stay among survivors was 23.6 +/- 18.8 days. The commonest postoperative complications were wound infection and enterocutaneous fistula. The overall mortality was 23.8%, increasing to 50% in children aged less than 5 years, although the difference was not statistically significant (P > 0.05). To improve survival in TP, attention should be focused on perioperative resuscitation and early intervention. The provision of potable water, adequate sanitation, and active immunisation are means to eradicate the disease.

Giant omental cyst simulating ascites in a Nigerian child: case report and critique of clinical parameters and investigative modalities.

We report our experience of managing an 18-month-old boy in whom a giant omental cyst of 4.6 kg, which constituted 42% of his pre-operative weight, masqueraded as massive ascites. Pre-operative diagnosis and early surgical intervention were facilitated by inter-disciplinary collaboration, ultrasonography and radiological contrast studies. The differential diagnoses and treatment options of omental and mesenteric cysts are discussed. The importance of ultrasonography as an initial imaging tool for arriving at the correct diagnosis in a child with ascites of obscure aetiology is emphasized.

Gangrenous bowel in Nigerians.

OBJECTIVE: This study is to critically examine gangrenous bowel as seen in a developing country to identify factors associated with mortality and to reappraise the problems in our environment. We also looked at mortality associated factors not considered by other studies from the subregion. DESIGN: We retrospectively reviewed the records of all patients who had gangrenous bowel over a 10 year period (1986 to 1995) at the University of Ilorin Teaching Hospital, Ilorin, Nigeria. SETTING: University of Ilorin Teaching Hospital, Ilorin, Nigeria. SUBJECTS: All patients with gangrenous bowel seen at the University of Ilorin Teaching Hospital, Ilorin Nigeria from January 1986 to December 1995 were included is this study. RESULTS: Ninety five patients were seen with a male to female ratio of 2:1. A high prevalence at age zero to five (17.9%) was noted due to childhood intussusception. The average duration of symptoms before surgery was 5.6 days. External hernia were responsible for 42.1%, intussusception for 22.1% and adhesions for 12.9% of cases. Other rarer and unusual causes of gangrene are also presented. Ileal gangrene was seen in 80.9% of cases. The overall mortality was 35.8% with no significant age and sex differences. Mortality varied directly with the length of gangrenous bowel irrespective of the cause and with the delay in operative intervention up to eight days. Peritonitis and involvement of the colon were also associated with high mortality. CONCLUSION: As a result of late presentation allowing for high diagnostic accuracy, elaborate investigations to determine bowel viability have little role in our setting at present. Improvement in socio-economic status, health education, elective repair of hernia and early operative intervention are recommended to bring down the mortality from gangrenous bowel.

[Determination of the purity of Eimeria species of chickens].

The morphologic criteria length and breadth were used for the examination of the purity from 23 strains of different Eimeria species in the fowl (Eimeria tenella, E. acervulina, E. maxima, E. necatrix) by three methods: Tabulate graphic method, determination of the size of concentration, analysis with counter. The tabulate graphic method by use of peak value as only criterion of valuation has suitable proved for the screening test. A qualification of the degree of purity is reached in the form of classes of homogeneity with the criteria peak value, first and second classes of the neighbour by the determination of the size of concentration. The quantitative certainly of the assertion increases by the analysis with counter from six statistical parameters (frequency of peak value, frequency of the sum from peak value, first and second classes of the neighbour, number of taking classes in the specific range of species, coefficient of variation, coefficient of correlation, divergence of specific index of the form in the species).

[Detection of Eimeria tenella antibodies by the serum dye test].

The serum dye test by Sabin-Feldman used in the toxoplasmosis serology was adapted to the Eimeria tenella system. The antibodies detected by this specific test reflect a protective immune response. All animals immunized with virulent and attenuated Eimeria tenella oocyst antigens demonstrated serologically immune reactions. Antigen reactivities over 50% correlate with the grade of protection of animals in challenge test.
