[Diagnostics, treatment, and prevention of iatrogenic iodine-induced thyroid gland diseases].

The authors of the study investigated the effects of pharmacological doses of iodine as an ingredient of amiodaron, an anti-arrhythmic agent, and iodine radiopaque contrast agents (RCA), on the thyroid gland (TG) function. The subjects were 133 patients aged 60.5 +/- 8.5 years, who had been taking amiodaron for 12 to 164 months, and 164 patients aged 59.4 +/- 9.1 years, who were examined before coronarography (CAG) with RCA, and 1, 3, 6, and 12 months after the procedure. Serum levels of TSH, FT4, FT3, TG antibodies, TPO antibodies, and urine iodine level were measured; TG ultrasonography was performed. Forty-nine (51.9%) out of the 133 patients on amiodaron therapy developed thyroid disfunction. Subclinical hypothyroidism (24 cases; 18%) and manifest thyrotoxicosis (21 case; 15.8%) prevailed among functional TG disorders. Thirty-one per cent of the patients with thyrotoxicosis had preceding TG diseases; 70% of the thyrotoxicosis patients had a cardiac arrhythmia relapse. The concentration of thyroid antibodies did not change in patients with normal TG. The patients who underwent CAG displayed elevated renal iodine excretion 1, 3, and 6 months after RCA administration. Thyroid disfunction (thyrotoxicosis in 7.6% of the cases; hypothyroidism in 15.8% of cases) was revealed in 23.4% of the patients with preceding TG diseases one month after CAG. Cardial pathology exacerbation was observed in patients with thyrotoxicosis.

[Impairment of cardiovascular function in subclinical hypothyroidism].

Subclinical hypothyroidism (SH) is the most frequent thyroid disease, but administration of replacement L-thyroxin in it is not obvious. SH may present both with general nonspecific symptoms and disturbance of lipid metabolism, myocardial dysfunction. Many physicians think it valid to restore euthyroid condition, as it may improve general condition of the patients, diastolic and systolic myocardial function, prognosis in ischemic heart disease. However, there are no definite indications to starting L-thyroxin in SH.

[Clinico-laboratory algorithms for assessing the functional activity of the thyroid gland].

Three diagnostic algorithms making use of automated chemiluminescent analyzers are proposed for evaluation of the thyroid function, depending on the clinical situation. Laboratory criteria of hypothyrosis and thyrotoxicosis therapy monitoring are discussed. Main variants of disagreement between clinical data and values of thyrotropic hormone and free T4 are presented.

[Sildenafil citrate (viagra) treatment in patients with diabetes mellitus].

AIM: To evaluate different dosages of vasoactive drug viagra in patients with erectile dysfunction suffering from diabetes mellitus (DM) type I and II. MATERIALS AND METHODS: Viagra in doses 25, 50 and 100 mg was given to 30 DM patients with different forms of erectile dysfunction. RESULTS: The response was obtained in 70.3% of cases. The highest effect was achieved with the dose 100 mg (the response rate 68.5% of patients). The dose 50 mg was effective in 31.5%, 25 mg in 0% of patients. Most nonresponders had low testosterone levels in the blood.

[The connection between respiratory disorders during sleep and peripheral autonomic insufficiency in diabetics].

To reveal the connection between sleep breathing disturbances (SBD) and autonomic insufficiency (AI) 23 patients with diabetes mellitus (DM) were examined (7 cases of insulin-dependent DM and 16 cases of noninsulin-dependent DM). Clinical examination, night polysomnography and autonomic nervous system assessment were performed. No connection between the existence and severity of AI and SBD was found in patients with DM of both types. Patients with more pronounced obesity had significantly higher number of SBD. The conclusion was made that SBD were not connected with AI in patients with DM, but obesity was an important factor in their genesis.

[Sipple syndrome].

[Congenital hypoplasia of the pancreas with lipomatosis and bone marrow dysfunction (Shwachman syndrome)].

The description is given of Shwachman syndrome in a male of 17, with a long anamnesis of malabsorption syndrome, granulocytopenia, repeated bronchopneumonia, somatogenic subnanism and hypogonadism. The death resulted from acute cardiovascular insufficiency. Post-mortem examination revealed exocrine pancreas hypoplasia with lipomatosis, Wirsung duct atresia, fibrocholangiocytosis and fat hepatosis, visceral anemia, delay in the maturation of granulocytes in the bone marrow, somatogenic polyglandular endocrinopathy, metabolic myocardium necrosis, calcinosis of the small arteries wall, lymphoid tissue atrophy, focal pneumonia of the upper lobe of the left lung.
