Early events in atherogenesis.

Most of the factors that are thought to be involved in atherogenesis can act by producing increased endothelial permeability. They may cause damage in their own separate ways or through their effects on blood-platelets. Several constituents of platelets could produce vascular damage that may result in intimal oedema and subsequent atherosclerosis.

Injury and repair of tracheobronchial cartilage following accidental exposure to ehtyleneimine.

A fatal case following ehtyleneimine inhalation is described. Profound destructive effects on tracheobronchial cartilage were found at necropsy after an interval of apparent recovery from early phases of poisoning. The pathogenesis of these cartilaginous changes is discussed. It is proposed that they may have been due to proteases liberated from liberated from mucosal granulation tissue.

Partial nodular transformation of the liver presenting with ascites.

Partial nodular transformation of the liver is a rare condition. Only six cases have been reported previously, all of which presented with bleeding from oesophageal varices. A further example of the disorder is recorded differing in the mode of onset with ascites. The total duration of the illness was eight weeks. Necropsy showed widespread evidence of liver cell damage with nodule formation around the hilum. These nodules had caused portal venous obstruction and ascites.

Fatal fat embolism following replacement arthroplasty for transcervical fractures of femur.

Seven cases are reported of death in old people associated with transcervical fractures of the femur and occurring shortly after insertion of a Thompson prosthesis. They are compared with six fatalities following similar injury but not surgically treated and with three which followed internal fixation. Positive controls for massive fat embolism were selected from fatal cases of multiple and severe skeletal fractures due to road accidents. It seems likely that patients undergoing this operation are peculiarly susceptible to lethal fat embolism. The clinical features, diagnosis, pathological aspects, and prevention of fat embolism are discussed.
