Cutaneous ulceration resembling pyoderma gangrenosum in the primary antiphospholipid syndrome: a report of two additional cases and review of the literature.

The antiphospholipid antibody syndrome has been associated with various cutaneous manifestations, the most common of which may be leg ulceration. Cutaneous ulcers with clinical features suggestive of pyoderma gangrenosum have been reported in this setting only rarely. We report here two additional cases of the association between antiphospholipid antibody syndrome and pyoderma gangrenosum-like skin lesions. These patients suffered from the primary antiphospholipid syndrome. We suggest that clinicians assay for antiphospholipid antibodies when a clinical diagnosis of pyoderma gangrenosum is entertained.

The abraded punch graft for pitted facial scars.

The abraded punch graft procedure is one in which the surrounding scar area and the graft itself are superficially dermabraded before correction of the defect. This alternative procedure has distinct advantages over the standard punch graft and should be considered when contemplating the surgical correction of "ice pick" facial scarring. Following the normal reparative process, the abraded punch graft heals with a pleasing aesthetic appearance.

The punch scalp graft.

The punch scalp graft hair transplant procedure is an established, acceptable, and successful method of hair replacement for various forms of alopecia, in both men and women. The surgical skills require discipline, knowledge, and a modicum of surgical training, which allows most physicians, particularly surgeons, to be qualified to perform the procedure. Selection of the patient, objective evaluation of the alopecias and the donor area, and postoperative care and instructions are all essential ingredients to satisfying the hair transplant patient. The punch graft scalp technique hair transplant has become essentially a common operative procedure and is available to almost any qualified and desirous patient. The cost factors have remained reasonable and in keeping with other nonsurgical methods of artificial hair replacement. For the past 15 years, there have been numerous symposia and seminars on hair transplantation conducted by qualified experts in this particular field, which have made such courses available to essentially all physicians. Sufficient journal articles and textbooks have been published to provide any physician with adequate reference material, and ongoing additional publications with updates continue to be made available to all physicians. Many university medical teaching centers have developed free hair transplant clinics or low-cost hair transplant clinics at which residents in training perform the punch scalp graft hair transplant procedure to allow indigent patients likewise to obtain hair transplant procedures. Hence, the day has come when any patient, male or female, of any race, with suitable objective indications, can obtain a hair transplant if he or she is properly motivated and desires to do so.

Control of bleeding during dermabrasion.

Dermabrasion may be complicated by excessive bleeding during the procedure. Adrenosem salicylate has been used to control capillary oozing in various other surgical procedures. In a double-blind stidy of ten patients undergoing dermabrasion, Adrenosem salicylate was found to reduce capillary oozing during the procedure to a worthwhile degree. No untowards effects were encountered.

Football acne--an acneiform eruption.

Football acne is a peculiar form of localized acneiform eruption of the chin that occurs in acne-prone individuals, as a result of the use of a protective chin strap under conditions of physical and emotional stress. Perhaps it is a locus minoris resistentiae phenomenon. The condition responds favorably to the removal of the chin strap and to the usual topical and systemic acne therapies.

Dermabrasion for showers of seborrheic keratoses.

Seborrheic keratoses, skin tumors of undetermined etiology, are easily traumatized with consequent local irritation and inflammation, and are cosmetically undesirable. They can occasionally undergo malignant degeneration. A rapid increase in size and number of these lesions is thought by many to be a sign of internal malignancy. Dermabrasion using a diamond fraise is a simple and fast method for removing multiple seborrheic keratoses with minimal trauma, rapid healing, and good cosmetic results.

Clinical evaluation of the effect of two types of dental floss on plaque and gingival health.

The aim of this clinical study was to evaluate two different flosses (an expanded PTFE-type floss and a nylon waxed floss) on the reduction of dental plaque, gingival health (MGI and BI), and patient preference when used in an oral hygiene program. Sixty adult subjects with gingival inflammation were assigned randomly to two groups. The control group were those subjects that used the nylon waxed floss and the test group were those subjects that used the PTFE-type floss. At 0, 2, 5, and 6 weeks, the patients were scored for plaque (PI), gingivitis (MGI), and interproximal bleeding sites (BI). The data were then analyzed statistically within each group and between both groups using Student t tests. It was found that there was no statistical difference between the two types of dental floss regarding PI, MGI, and BI. The only discrepancy was at the end of the second week, where the PTFE-type floss showed a significantly greater reduction in BI as compared to the nylon floss (p less than 0.01). Here the percent reduction of the PTFE-type floss was 56.1%, while the nylon waxed floss showed a percent reduction of only 39.1%. At the fifth week of examination the type of floss used was reversed for one week. This was done to determine patient preference. It was found that 74.5% of the study population preferred the PTFE-type floss while only 24.5% preferred the nylon waxed floss.
