Concomitant treatment of severe uterine adenomyosis in a premenopausal woman with an aromatase inhibitor and a gonadotropin-releasing hormone agonist.

OBJECTIVE: To assess the effect of aromatase inhibitors with GnRH agonist for a severe symptomatic adenomyosis that is refractory to GnRH agonist and danazol with GnRH agonist. DESIGN: Case report. SETTING: Clinical practice in university hospital. PATIENT(S): A 34-year-old woman with a complaint of severe dysmenorrheal, symptomatic anemia, and a desire to retain fertility. INTERVENTION(S): Aromatase inhibitor anastrozole given orally (1.0 mg or 2.0 mg daily) for 16 weeks and GnRH agonist given monthly (injected SC, 1.8 mg) for 4 months. MAIN OUTCOME MEASURE(S): Measurements of uterine volume and levels of serum E(2), estrone, A, dehydroepiandrosterone sulfate, LH, FSH, and CA125. RESULT(S): Uterine volume was reduced. The reduction rate of uterine volume estimated by magnetic resonance imaging and ultrasonography was 60% after 8 weeks of treatment. CONCLUSION(S): Aromatase inhibitor with GnRH agonist therapy was useful for the management of a severely adenomyotic woman whose desire was for conservative treatment.

Effect of micro-environment maintenance on embryo culture after in-vitro fertilization: comparison of top-load mini incubator and conventional front-load incubator.

PURPOSE: To investigate the effect of microenvironment maintenance on embryo culture and clinical results using two types of incubators. METHODS: Temperature and oxygen concentration in a mini-incubator and a conventional incubator were compared following a 5-s door opening/closing procedure. Embryos of 30 in-vitro fertilization embryo transfer (IVF-ET) cases were randomly allocated to either one of the incubator, cultured, and the early-stage good embryo formation rate and the good blastocyst formation rate were compared, as indicators for micro-environment maintenance ability. RESULTS: Temperature recovery after a 5-s door opening/closing procedure was approximately 5 min for the mini-incubator and 30 min for the conventional incubator. The oxygen concentration return was significantly improved in the mini-incubator (3.0 +/- 0 min) compared with the conventional incubator (7.8 +/- 0.9 min). Both the early-stage good embryo formation rate and the good blastocyst formation rate were significantly higher in the mini-incubator (39.5% and 15.1%) than the conventional incubator (28.4% and 7.8%). CONCLUSION: The microenvironment maintenance ability of incubators appears to significantly influence the formation of good embryos.

Resting serum dehydroepiandrosterone sulfate level increases after 8-week resistance training among young females.

This study examined changes among young females of resting serum dehydroepiandrosterone sulfate (DHEAS) concentration after an 8-week period of resistance training. Nineteen healthy untrained young females [training group: age 18.9 (0.3) years, n=10, control group: age 19.3 (1.0) years, n=9; mean (SD)] were recruited in this study. The training group participated in an 8-week resistance training program (2 days per week on nonconsecutive days). The control group did not involve in any resistance training or regular exercise during the study period. Muscular strength, anthropometry, and resting hormonal levels were measured before and after training in both groups. Serum concentrations of DHEAS, dehydroepiandrosterone (DHEA), testosterone and cortisol were measured by radioimmunoassay. Body mass (2.4%) and lean body mass (2.4%) were significantly increased in the training group ( P<0.05), but not in the control group. The training also significantly increased one-repetition maximum (1-RM) values ( P<0.05). In the training group, resting concentration of serum DHEAS significantly increased after training ( P<0.05). Percent change of DHEAS in the training group was greater than that of the control group ( P<0.05). In the training group, the change of DHEAS level was positively correlated with the change of lean body mass during the training ( r=0.61; P<0.05). Serum DHEA, testosterone and cortisol concentrations did not change in either group during the training. The dramatic increase of resting serum DHEAS concentration after training indicates that DHEAS might be an anabolic hormone marker of adaptation to resistance training among young females. Acupuncture and responses of immunologic and endocrine markers during competition.

INTRODUCTION: Acupuncture is used to modulate the physical well-being of athletes in Asian countries. However, there is little information on the immediate effects of acupuncture treatment on physiological or psychological responses to exercise. PURPOSE: The purpose of this study was to examine the effect of acupuncture treatment on the physical well-being of elite female soccer players during a competition period. METHODS: Subjects were divided into two groups: those who received acupuncture treatment (18.1 +/- 2.3 yr [+/-SD], N = 9) and a control group (17.7 +/- 2.8 yr, N = 12). In the treatment group, acupuncture stimulus was applied at LI 4 (Goukoku), ST 36 (Ashi-sanri) for 20 min, and ST 6 (Kyosya), LU 6 (Ko-sai) points for 15 min 4 h after the game every night during the competition period. The measured parameters included salivary secretory immunoglobulin A (SIgA) level, cortisol level in saliva, subjective rating of physical well-being, and profile of mood states (POMS). RESULTS: The following were the main results: 1). Exercise-induced decrease of salivary SIgA and increase of salivary cortisol were inhibited by acupuncture. 2). Acupuncture improved subjective rating of muscle tension and fatigue. 3). The POMS score was modulated by acupuncture. CONCLUSION: These results support the effectiveness of acupuncture for physical and mental well-being of athletes.

Protein S gene mutation in a young woman with type III protein S deficiency and venous thrombosis during pregnancy.

BACKGROUND: We attempted to identify a gene defect in a young woman with type III protein S deficiency and venous thrombosis during pregnancy. METHODS: Measurements of total and free PS antigen levels in plasma were carried out using an enzyme-linked immunosorbent assay. Plasma PS cofactor activity was determined by a clotting assay using activated factor V as the substrate. Genomic DNA prepared from peripheral blood was amplified by polymerase chain reaction (PCR) with PROS1-specific oligonucleotide primers. PCR products were sequenced on both strands using specific oligonucleotide primers. RESULTS: Plasma PS cofactor activity was undetectable in every measurement at 36 weeks of gestation, as well as at 2 weeks and 4 months after delivery. Plasma total PS antigen levels were 70% and 67% at 2 weeks and 4 months after delivery, respectively. Free PS antigen level was 24% at 4 months after delivery. Of all exons analyzed, codon 295 of GGC in exon 10 was substituted for AGC. This missense mutation predicted an amino acid change of glycine to serine. CONCLUSIONS: Measurements of total and free PS antigen levels along with PS activity indicated that this was a case of type III PS deficiency. DNA analysis identified a heterozygous missense mutation of codon 295 in the PS gene, substituting glycine for serine.
