Optical coherence tomographic evaluation of a surgically treated traumatic macular hole secondary to Nd:YAG laser injury.

PURPOSE: To report the importance of optical coherence tomographic (OCT) evaluation of a macular hole caused by neodymium:yttrium-aluminum-garnet (Nd:YAG) laser injury. DESIGN: Observational case report. METHODS: A 33-year-old man presented with an accidental Nd:YAG laser-induced macular hole in his right eye. After observing the course of the macular hole by OCT, the macular hole was treated by standard pars plana vitrectomy. The appearance of the macular hole was also followed up postoperatively. RESULTS: Seven days after the injury, OCT showed a full-thickness macular hole without a fluid-cuff or swelling of the surrounding retina. Twenty-one days after the injury, however, OCT disclosed that the retina around the macular hole was swollen. After vitrectomy, OCT showed that the macular hole was closed, and the visual acuity improved from 20/30 to 20/20. CONCLUSIONS: Surgical treatment can improve the visual function of eyes with a Nd:YAG laser-induced macular hole. Optical coherence tomography is valuable in providing precise information about the condition of the macula.

Optical coherence tomographic findings of macular holes and retinal detachment after vitrectomy in highly myopic eyes.

PURPOSE: Macular holes cause retinal detachments in highly myopic eyes. Because degenerative macular changes often coexist, biomicroscopic evaluation of macular hole status after retinal reattachment is sometimes difficult. We studied macular holes with retinal detachment after vitrectomy using optical coherence tomography and evaluated the anatomic status of the hole and factors associated with anatomic success. DESIGN: Retrospective, nonrandomized, comparative study. PATIENTS: Sixteen eyes that underwent vitrectomy for retinal detachment associated with a macular hole were included. Internal limiting membrane peeling with indocyanine green was performed in 14 eyes; the epiretinal membrane was peeled with a diamond-dusted membrane scraper alone in two eyes. All retinas reattached postoperatively. The follow-up period at the optical coherence tomography examination was at least 6 months. METHODS: Optical coherence tomography was performed vertically and horizontally, and the presence of a persistent macular hole was determined. Other information was obtained from patient records. RESULTS: The macular holes closed in seven of 16 eyes (44%). Age, sex, axial length, preoperative best-corrected visual acuity, duration of symptoms, preoperative refractive error, and the preoperative area of the retinal detachment were not significantly correlated with hole closure. Improved postoperative best-corrected visual acuity (P <.05) was significantly associated with macular hole closure, and more frequent visual improvement (P =.06) was of borderline significance. CONCLUSIONS: The success rate was lower than those obtained in eyes without myopia or in myopic macular holes without retinal detachments. Macular hole closure may predict improved visual outcome for patients with retinal detachment and macular holes. Optical coherence tomography detects persistent macular holes in highly myopic eyes with retinal detachment.

Sutureless contact lens ring system during vitrectomy.

PURPOSE: To report a new sutureless ring system to fix corneal contact lens in place during vitrectomy. DESIGN: New surgical instrument. METHODS: The sutureless ring system consists of a stainless steel ring that has two upright tabs 180 degrees apart with a hook on the top and two elastic silicone bands 8 mm in diameter to fix the ring to the eyelid speculum. RESULTS: The contact lens fixed in place with the sutureless ring was stable and allowed intraoperative visualization of the fundus. CONCLUSION: This suture-free contact lens system facilitates visualization of the fundus during vitrectomy allowing shorter surgical time. It also enables us to avoid additional trauma associated with the lens ring fixation procedure.

A microperimeter that provides fixation pattern and retinal sensitivity measurement.

PURPOSE: To investigate a microperimeter (MP-1) that can independently measure fixation status and microperimetry of retinal threshold sensitivity by comparing the results obtained by the MP-1 with those obtained by scotometry using scanning laser ophthalmoscopy (SLO). METHODS: Fourteen patients (15 eyes) with a macular disorder were examined by the MP-1 fixation test, MP-1 microperimetry, and SLO scotometry. The scotoma size seen in MP-1 microperimetry was compared with that observed in SLO scotometry. The location of the preferred retinal locus (PRL) and the fixation stability, which were obtained from the fixation test in MP-1, were also compared with those obtained in SLO scotometry. RESULTS: The scotoma size, which was not seen with 0 dB in MP-1 microperimetry, was larger than that in SLO scotometry in 8 of the 15 eyes. Retinal threshold sensitivity decreases were found using MP-1 microperimetry within a sensitive area of SLO scotometry in all eyes. The PRL in the MP-1 fixation test and in SLO scotometry agreed in all eyes. Fixation stability in the MP-1 fixation test significantly correlated with that in SLO scotometry (P = 0.0192). CONCLUSIONS: The results of MP-1 microperimetry do not completely agree with those of SLO scotometry owing to the difference in stimulus intensity. The MP-1 fixation test is useful for measuring PRL and fixation stability in a short time. MP-1 might be helpful to evaluate the foveal function in patients with macular disorders without severely damaged macular function.

Induction of connective tissue growth factor in retinal pigment epithelium cells by oxidative stress.

PURPOSE: To investigate whether connective tissue growth factor (CTGF) is upregulated in the retinal pigment epithelium (RPE) by oxidative stress in vitro and in vivo. METHODS: ARPE19 cells were subjected to two forms of oxidative stress: hypoxia/reoxygenation and paraquat exposure. CTGF expression was studied by real-time reverse transcriptase-polymerase chain reaction (RT-PCR) and Western blot analysis. The ocular expression of CTGF was analyzed by RT-PCR and immunohistochemistry in an in vivo murine model of oxidative stress exposed to light after intravenous injection of rose bengal. RESULTS: CTGF mRNA expression in ARPE19 cells was upregulated by oxidative stress and peaked 1 h after reoxygenation; protein expression peaked 2 h after reoxygenation. Using paraquat, both CTGF mRNA and protein were upregulated by five doses of paraquat. In vivo, CTGF was upregulated when evaluated by RT-PCR and immunohistochemistry using murine retinas stimulated by oxidative stress. CONCLUSIONS: Upregulation of CTGF expression was confirmed in RPE cells by oxidative stress in vitro and in vivo. CTGF might be one of the factors that induces the changes observed in aging retinas.

Radial optic neurotomy for the management of hemicentral retinal vein occlusion: long-term follow-up study.

PURPOSE: To assess the efficacy of radial optic neurotomy (RON) for hemicentral retinal vein occlusion (hemi-CRVO) in patients with follow-up of >12 months. METHODS: Five consecutive patients with hemi-CRVO were enrolled in the study and underwent vitrectomy with RON. The effects of this treatment were assessed by visual acuity, optical coherence tomography, indocyanine green angiography, and kinetic visual field testing. RESULTS: The median and mean visual acuities improved from 20/250 to 20/50 and from 20/286 to 20/51, respectively. Four patients (80%) had visual acuity improvement of > or =3 lines; visual acuity in 1 patient (20%) remained the same. Visual acuity in all four eyes with preoperative visual acuity of <20/200 improved to >20/200 12 months after surgery. Foveal thickness was gradually reduced during the follow-up period. Chorioretinal anastomosis was detected by indocyanine green angiography in 3 eyes (60%). A temporal visual field defect was found in two eyes. No neovascularization on the iris, angle, optic disk, or retina was detected postoperatively. CONCLUSION: Visual acuity may improve more with RON compared with the natural history of the disease, and no neovascular formation may occur after RON, suggesting that this procedure may be an effective treatment for hemi-CRVO.
