Unusual presentation of cervical spinal intramedullary arachnoid cyst in childhood: case report and review of the literature.

The authors report a 7-year-old girl who presented with progressive quadriparesis which had started a month before admission. Magnetic resonance imaging of the spine revealed an intramedullary cystic lesion extending from C(2) to C(4). After performing a C(2-5) laminectomy, the cyst was drained and anatomically fenestrated with the subarachnoid space by a 1-cm vertical median myelotomy without using a stent. The histopathological findings revealed arachnoid cyst. In the first month after operation, the neurological deficits disappeared gradually, except for a slight left upper paresis. Cervical spinal intramedullary arachnoid cyst is extremely rare and should be considered in the differential diagnosis of intramedullary cystic lesions in childhood. Recovery is possible after subtotal excision and fenestration of the cyst to allow communication with the subarachnoid space even if neurological deficits are present for a long time.

Effect of ELF magnetic fields on lipid peroxidation, sperm count, p53, and trace elements.

BACKGROUND: Some epidemiological and laboratory studies suggest a possible connection between extremely low-frequency (ELF) magnetic fields and certain illnesses, such as cancer, immune suppression, as well as reproductive toxic effects and abnormalities. Therefore, the aim of this study was to investigate the effects of ELF magnetic fields (1.35 mT) on sperm count, malondialdehyde concentration, the histology of such organs as the testes, brain, liver, and kidney tissues, p53 immunoreactivity of bone marrow, and the serum concentrations of Cu2+, Zn2+, Mn2+, and Fe3+ in rats. MATERIAL/METHODS: Sixteen Sprague-Dawley male rats were divided into two groups. The rats in the experimental group were exposed to an ELF magnetic field 2 hr/day for 2 months (7 days a week). The rats in the control group were not exposed to the ELF magnetic field. The exposure was performed in a Faraday cage (130 x 65 x 80 cm) with grounded shielding against the electric component. The Mann-Whitney U-test was used for the statistical analysis of the data. RESULTS: Magnetic field measurements showed that, under the experimental conditions, the magnetic field-exposure system produced a stable flux density of 1.35+/-0.018 mT and a stable frequency of 50 Hz, with negligible harmonics and no transients. However, no statistically significant alteration was observed in the parameters measured in this study except in Mn2+ concentrations (p<0.001). CONCLUSIONS: The present study found no evidence of an adverse effect of ELF magnetic fields on the measured parameters except for significantly increased Mn2+ concentrations (p<0.001).

Expression of p53 oncoprotein and bcl-2 in renal cell carcinoma.

OBJECTIVE: The aim of the present study was to investigate the significance of p53 and bcl-2 as prognostic factors among others in renal cell carcinoma patients. METHODS: We evaluated the stages, histological grades, tumor diameters, cellular patterns and the presence of mutant p53 protein and bcl-2 overexpression in 57 cases of renal cell carcinoma (RCC). Kaplan-Meier and log-rank tests estimated the survival function of each parameter. The study was carried out in the Department of Pathology and the Department of Urology, Faculty of Medicine, Dicle University Hospital, Diyarbakir, Turkey, in 2003. RESULTS: The p53 mutation was 35% and bcl-2 overexpression incidence was 89.4% in the RCC cases included in the study. The 5-year disease specific survival rates of mutant p53 positive was 46.6% and p53 negative cases were 83.3%, (p=0.0063). There was no pathological parameter associated in bcl-2, and it has no prognostic significance. CONCLUSION: The tumor stage, grade, diameter and p53 mutations affect the survival of RCC cases. The bcl-2 staining did not play any role to estimate patients at high risk of the disease progression.

Hodgkin's disease and association with Epstein-Barr virus in children in Southeast Turkey.

OBJECTIVES: There are relatively few reports on histologic and immunophenotypic features of Hodgkin's disease (HD) in children in Turkey. The aim of the present study is to characterize the clinicopathologic, immunophenotypic features and Ebstein-Barr virus (EBV) status of HD in children in our region. METHODS: Fifty-two cases coded as HD in the Department of Pathology, Medical Faculty, Dicle University, Turkey, from 1990 to 2002 were retrieved. We analyzed clinicopathologic data, immunophenotype, and EBV status of all patients. RESULTS: In most cases, the cervical lymph nodes were involved. The 52 cases (35 boys, 17 girls; male to female ratio was 2.1) were categorized as mixed cellularity (MC) in 23 (44.2%), lymphocyte predominance (LP) in 16 (30.8%), nodular sclerosis (NS) in 10 (19.2%), and lymphocyte depletion (LD) in 3 (5.8%). Of 33 cases, including MC and NS, Hodgkin's Reed-Sternberg cells and variants were positive for CD15 in 27 cases (81.8%) and positive for CD30 in 30 cases (90.9%). All the cases of LP demonstrated the characteristic phenotype of this variant (CD45RB+, CD20+, CD15-, CD30-). Epithelial membrane antigen stained the lymphocytic/histiocytic cells in 3 cases (18.7%) of 16. Epstein-Barr virus-LMP1 were positive in 31 cases (61.5%) of a total of 52 cases and the most were MC (91.3%). CONCLUSION: In conclusion, mixed cellularity HD is the most common histologic subtype. This result differed from that reported in developed countries. We found a striking association with expression of EBV-latent membrane protein 1 in malignant cells in childhood HD.

Lymphoepithelial cyst of the mediastinum.

We report a rare case of lymphoepithelial cyst of the mediastinum. A 38-year-old woman was found to have a right paratracheal mediastinal mass on chest radiograph. Computed tomographic scanning showed a cystic mass on the right side of the trachea. The encapsulated mass was situated in the upper mediastinum and was adherent to the trachea. The lesion was resected via a right thoracotomy. Histopathologic examination showed that the cystic mass was lined with 1 layer of ciliate columnar epithelium. There were no malignant foci. These findings were consistent with a diagnosis of multicystic lymphoepithelial cyst. The postoperative course was uneventful, and the patient was discharged from the hospital on the 7th postoperative day.

Effect of nimodipine on histological alterations in basilar artery following the bilateral common carotid artery ligation (preliminary study).

BACKGROUND: The blood supply to tissues is reduced as a result of arterial occlusions. Angiogenesis, collateral circulation and reverse flow mechanisms go into operation to restore a continued adequate supply of blood. Ca++ channels undertake the major part of this function. As a result of the increasing tension on the arterial walls, vascular autonomy is affected, and ischemia and even necrosis are observed. METHOD: Adult 100 male hybrid rabbits were used in this study. The bilateral carotid arteries were ligated at the carotid bifurcation. The rabbits were divided into 2 main groups: treatment and control, and then both groups were further divided into 5 subgroups consisting of 10 rabbits each. The rabbits were sacrificed between the first day and the end of 8 weeks for histopathological examination of the basilar artery in two groups. RESULTS: In control groups, after 24 hours of the occlusion partial swelling and minor endothelial damage were observed in histopathological sections of the basilar artery. Luminal flattening started to decrease, and expanding of the diameter continued. The increase in the diameters of the basilar artery was higher in animals treated by nimodipine, and that difference was statistically significant (P=0,000). CONCLUSION: This study revealed that the intimal and medial alterations arising from the increased blood flow rate in the basilar artery might be lessened and even partially prevented by the use of nimodipine.

Recurrent malignant schwannoma of the small bowel.

Primary malignant schwannoma of the small bowel is an extremely rare disease. Only 24 malignant schwannomas of the small bowel have been reported in the medical English literature. We report a primary malignant schwannoma of the distal ileum in a 53-year-old woman, who was admitted to the hospital because of abdominal pain. CT scan revealed a semisolid mass in the small intestine of the serosa. The tumour measuring 14 x 13 x 8 cm and 100 cm of the distal ileum were resected. We observed a recurrence after seventeen months and the recurrent tumour measured 6 x 2.5 x 1 cm was excised.

The relation of Helicobacter pylori with intestinal metaplasia, gastric atrophy and BCL-2.

BACKGROUND/AIMS: It is assumed that the sequence of events in gastric cancer is as follows: chronic gastritis, atrophy, intestinal metaplasia (IM), dysplasia, and carcinoma. It is also known that Helicobacter pylori (Hp) can be involved in the chain of these chronic phenomena. Therefore, we studied the relation of Hp with chronic inflammation, atrophy, activity level, IM and bcl-2. METHODS: The number of patients included in this study was 52; 35 (67.3%) were female and 17 (32.7%) were male. The average ages of women and men were determined as 44.5 and 41.5 years, respectively. Hp was determined in 53.8% of all cases and chronic inflammation was found in all cases. No dysplasia or gastric cancer was found in our cases. A statistically significant positive correlation was found between the Hp intensity and the degrees of chronic inflammation, atrophy and activity. RESULTS: Bcl-2 was found positive in 7.1% of the patients with Hp, and in 4.1% of the Hp-negative patients. However, bcl-2 was positive in 2 (8.7%) of 23 Hp-positive patients with chronic atrophic gastritis and in 1 (11.1%) of 9 Hp-negative patients with chronic atrophic gastritis. In other words, bcl-2 was found more in Hp-negative chronic atrophic gastritis than in Hp-positive chronic atrophic gastritis. Also, atrophy and IM were present together in 2 bcl-2-positive patients in whom Hp was positive. Bcl-2 was found positive more in IM than atrophy, proportionally. According to these data, although bcl-2 was found positive more often in Hp (+) cases proportionally, it was not statistically significant. Nevertheless, no significant difference was found between bcl-2 positivity and atrophy according to the statistical data. The data regarding the relation between IM and bcl-2 were statistically significant, and a positive correlation was found between them. CONCLUSIONS: We concluded that Hp infections result in chronic gastritis, have a role in the development of atrophy and IM, and that Hp infection has a significant relation with neutrophile activation. The more Hp intensity increases, the greater the degrees of chronic gastritis, activity and atrophy. We also found that Hp, which is known to increase apoptosis, increases the expression of bcl-2, an anti-apoptotic gene, not directly but rather by causing atrophy development. We observed more bcl-2 positivity in IM than in atrophy. Further studies with an extensive series of cases, including patients with dysplasia and gastric cancer, are needed to clarify statistical rates and to support the suggestion that bcl-2 expression increases with the progression to gastric cancer.

Preventive effects of intracisternal alphatochopherol on cerebral vasospasm in experimental subarachnoid haemorrhage.

Vasospasm is an important cause of morbidity and/or mortality with a subarachnoid haemorrhage (SAH). The roles of lipid peroxidation in a vasospasm caused by a SAH remain to be investigated. The effect of an intracisternal administration of alphatochopherol on a cerebral vasospasm was investigated in an experimental model. The authors assessed whether the administration of alphatochopherol reduced the vasospasm. By means of an intracisternal blood injection model, a SAH was induced in 30 rats, which were randomly divided into three groups, as follows: group I (G1), without a SAH and drug, group II (G2), a SAH alone, group III (G3), a SAH and alphatochopherol. Following the withdrawal of cerebrospinal fluid (CSF), a fresh unheparinized arterial blood was injected into the cisterna magna to induce a SAH. In G3, 20 U (0.4ml) alphatochopherol was intracisternally injected forty-five hours after induction of the SAH. All rats were sacrificed 72 hours after the induction. The basilar artery, with surrounding tissue, was removed from the cranium. The cross-sectional diameter of the lumen and vessel wall of the rat basilar artery was assessed from a planimetric analysis, and changes compared with G1 and G2. The reduction in the luminal cross-sectional diameter of the vessels exposed to subarachnoid blood was found to be 29.01 % (p=0.001). The group treated with alphatochopherol had a 9% reduction (p=0.004). The role of lipid peroxidation on a vasospasm caused by SAH is well known to be critical. Data from the present study indicated that antioxidant therapy, with topical alphatochopherol, may be promising on a vasospasm caused by a SAH.

Whole body exposure of rats to microwaves emitted from a cell phone does not affect the testes.

The objective of this study was to investigate the effects of radiofrequency radiation emitted from cellular phones on the lipid composition, malondialdehyde concentration, p53 immune reactivity, sperm count, morphology, histological structure of testes, and on rectal temperature of rats exposed to microwave radiation emitted from cellular phones. Sixteen Spraque-Dawley rats were separated into two groups of eight, sham exposed (control) and experimental. The rats were confined in plexiglas cages specially designed for this study, and cellular phones were placed 0.5 cm under the cages. For the experimental group, cellular phones were activated 20 min per day (7 days a week) for 1 month. For the control group, the cellular phones were placed beneath the cages for 20 min a day, but the phones were turned off. Rectal temperatures were measured weekly. For 250 mW radiated power, the whole body average SAR (rms) is 0.52 W/kg and 1 g averaged peak SAR (rms) is 3.13 W/kg. The Mann-Whitney U-test was used for statistical comparisons of groups. No statistically significant alteration in any of the endpoints was noted. This study found no evidence suggesting an adverse effect of cell phone exposure on measures of testicular function or structure.

Cyclopia.

True cyclopia is a rare anomaly in which the organogenetic development of the 2 separate eyes is suppressed. We report a fetus with an association of cyclopia with other anomalies. The possible mechanism of the histogenesis is discussed, together with a review of the relevant literature.
