[A historical review of the study of electrical activity of heart and origination of electrocardiography].

This paper has an overall review of the historical process of electrocardiographic development and the origination of modern electrocardiography in retrospect and also has an analysis of the scientific background, the important events and the historical effect of outstanding figures throughout the development. The experience and laws contained in the historical development are briefly summerized in this paper. At the same time the authors point out that the establishment of modern electrocardiography and the related technical development are crystallization of the wisdom and the result of the hard work of many scientists including the figures who should have been awarded Nobel Prizes.

[Myelodysplastic syndrome with Sweet's syndrome--one case report and literature review].

OBJECTIVE: In order to raise understanding of myelodysplastic syndrome(MDS) with Sweet's syndrome(SS). METHODS: The characteristics of MDS with SS were discussed with literature review. RESULTS AND CONCLUSIONS: The characteristics of MDS with SS were as follows: 1. SS associated with MDS was a systemic disease, involving lung, liver, spleen, myocardium, mesentery and subcutaneous fat tissue, besides cutaneous lesions. The article reported the thyroid gland involved by SS with MDS for the first time. 2. SS with MDS might occur with neutropenia. 3. SS with MDS was a marker of advancing MDS. MDS with SS was transformed to acute myelogenous leukemia easily.

Changes in serum cardiac troponin I levels after percutaneous transluminal coronary angioplasty.

OBJECTIVE: To evaluate the possible effect of percutaneous transluminal coronary (PTCA) on myocardium. METHODS: Serum cTnl and CK-MB were measured in 60 patients with coronary artery disease (CAD) who underwent PTCA before and at the 6th, 12th, 24th, 48th and 72nd hour after the interventional procedure respectively. RESULTS: The serum cTnl levels began to increase at 6 hours (9.65 +/- 6.27 micrograms/L) in 18 patients, reached the peak levels during 12-24 hours (20.43 +/- 11.28 micrograms/L, 18.52 +/- 9.52 micrograms/L), and returned to normal range till 48-72 hours (7.35 +/- 7.62 micrograms/L, 5.51 +/- 3.13 micrograms/L) after PTCA. The serum cTnl and CK-MB levels were kept normal range pre- and post-PTCA in 30 cases. The levels of cTnl in 12 cases were over baseline either before or after the procedure, while for CK-MB, only in 3 cases were over normal range after PTCA. Compared with normal cTnl group, elevated cTnl levels were related to total inflation times and dilated times (P < 0.05). CONCLUSIONS: PTCA may cause some minor damage in myocardium, serum cTnl level was more sensitive and specific for monitoring myocardial injury.

[A study on the cariogenic bacteria of plaque on abutment teeth of casting and wrought wire clasp removable partial dentures].

OBJECTIVE: The effect of clasp on the health of abutment teeth. METHODS: To analyse and compare the cariogenic bacteria plaque getting from the buccal cervical third of casting and wrought wire clasp teeth before and one week after wearing removable partial dentures. RESULTS: The average percentages of Streptococcus Mutans and Actinomyces Viscosus in total culturable bacteria rised significently in statistical analysis, but there was no significent difference of Lactobacilli. The average percentage of Actinomyces Viscosus increased less after wearing casting clasp RPD than that of wrought wire clasp. CONCLUSION: Clasps of RPD increased the risk of root caries of abutment teeth. However the influence of casting clasps was smaller than that of wrought wire clasps.

[Intervention treatment with percutaneous trans-arterial implantation of port-catheter system for advanced cancer patients].

OBJECTIVE: To examine the feasibility, clinical efficacy and complications of percutaneous trans-arterial implantation of port-catheter system (PCS) for intervention treatment of advanced malignant tumors. METHODS: Fifty-three cases with advanced malignant tumors were treated by infusion of chemotherapeutic drugs and/or chemo-embolization through PCS. Patients received one course of treatment every 4-6 weeks. RESULTS: In this series of treated patients, the survival period was 5-18 months with a mean survival time of 13.5 months. The overall response rate (CR + PR) was 62.3%. Detachment of the catheter tip occurred in 2 cases. Disruption of wound after removal of stitches occurred in one case and skin necrosis at the port occurred in another. CONCLUSION: Long-term intervention treatment with PCS is simple and safe. It is a treatment of choice for patients with advanced malignancy.

[The value of spiral CT in CT arterial portography and CT hepatic arteriography].

OBJECTIVE: To evaluate the usefulness of spiral CT in CT arterial portography (CTAP) and CT hepatic arteriography (CTHA). METHODS: The CTAP and CTHA manifestations in 50 patients with small hepatocellular carcinoma (< 3 cm in diameter) were analyzed and compared with those in bi-phase enhanced CT. RESULTS: The detection rate of CTAP and CTHA was 92.2% and 90.1%, respectively. They could reveal tumors of 0.2 cm-0.5 cm in diameter. Both CTAP and CTHA gave false-positive findings, such as perfusion defects in 13.0% of CTAP and non-pathologic enhancement in 19.1% of CTHA. CONCLUSION: With the use of spiral CT technique, the quantity of contrast material administrated in CTAP and CTHA can be considerably reduced, and the quality of CT images significantly improved. The detection rate of CTAP and CTHA is higher than that of biphase enhanced CT. Simultaneous use of both procedures may help decrease the false-positive rate, and increase the sensitivity of diagnosis for small hepatocellular carcinoma.

[Study on epidemic of drug-resistant tuberculosis in Shandong province, China].

OBJECTIVE: To establish the surveillance system for drug resistance of tuberculosis according to WHO and IUATLD guidelines, and to evaluate the initial and acquired drug resistance in Shandong province. METHODS: 30 of 141 counties/cities were randomly selected as surveillance points by cluster sampling from April to October 1997, in each of which 36 cases with smear positive pulmonary tuberculosis were continuously observed. Identification and susceptibility tests were done in all culture positive samples. RESULTS: The total frequency of drug resistance was 23.4%, initial drug resistance 17.6%, initial multi-drug resistance 2.9%, acquired drug resistance 50.0%, acquired multi-drug resistance 19.6%. CONCLUSIONS: The results suggest that the initial drug resistance still ranks high in Shandong province, National TB Control Program should be further strengthened.

[Cardiac syndrome X and insulin resistance].

OBJECTIVE: To determine whether there is insulin resistance in patients with syndrome X. METHODS: By using oral glucose tolerance test (OGTT), plasma glucose and insulin levels, and insulin sensitivity index (ISI) were measured in 11 patients with syndrome X and in 10 sex and age matched normal subjects as controls. RESULTS: The syndrome X group had a higher fasting insulin level and ISI than the control group (P < 0.05). After oral glucose, plasma insulin level and ISI in patients with syndrome X were significantly higher than the control group (P < 0.01). CONCLUSION: In patients with syndrome X there are hyperinsulinemia and insulin resistance.

[The clinical significance of changes in plasma levels of alpha-granule membrane protein and D-dimer in patients with unstable angina].

OBJECTIVE: To explore the relationship between platelet activation and enhanced coagulability and the occurrence unstable angina. METHODS: Plasma levels of a-granule membrane protein (GMP-140) and fibrin D-dimer were measured in 22 patients with unstable angina during the acute phase and after relief. The same procedure were compared with those in 20 control subjects and 18 patients with stable angina. RESULTS: Plasma levels of GMP-140 and D-dimer were significantly higher in patients with unstable angina both during the acute phase (P < 0.01) and after the relief (P < 0.05) as compared with those in control subjects and patients with stable angina, but there was no significant difference between the latter two groups (P > 0.05). There was also no significant difference between the patients with unstable angina during the acute phase and after the relief (P > 0.05). CONCLUSION: It is shown that activation of platelet and coagulation system combined with endothelial injury may play an important role in the development of unstable angina.

[Central nervous system involvement in systemic lupus erythematosus].

OBJECTIVE: To investigate central nervous system (CNS) involvements in systemic lupus erythematosus (SLE) so as to enhance our knowledge in diagnosing and treating CNS involvement of SLE. METHODS: The clinical data of 171 in patients with CNS involvement of SLE in our hospital were retrospectively reviewed. RESULTS: The mean SLE disease duration at onset of CNS involvement was (2.21 +/- 1.87) years and in 163 (95.3%) it was associated with active disease. Cerebral spinal fluid abnormality was seen in 91.4% (138/151) of the patients with CNS involvement of SLE. Among them protein elevation was found in 113, pressure elevation in 69, white cell elevation in 51 and glucose reduction in 6. For the evaluation of CNS involvement of SLE, the sensitivity of cranial CT and MRI was 77.4% and 81.4% respectively (P > 0.50). The positive rate of antiribosomal P in patients with diffuse CNS involvement of SLE was significantly higher than that in patients without CNS involvement (P < 0.01). On the contrary, the positive rate of ACL in focal CNS involvement of SLE was significantly higher than that in diffuse type or in patients without CNS involvement (P < 0.01). The total mortality rate in 171 patients with CNS involvement of SLE was 18.7%. The Mortality rate in the period of 1993 to 1998 (4.0%, 3/75) was significantly lower than that of 1980 to 1992 (30.2%, 29/96), P < 0.01. 24 SLE patients with CNS involvement received intrathecal dexamethasone and methotrexate, 22 cases (91.7%) improved considerably. CONCLUSION: CNS involvement occurs in the early stage of SLE, most of the cases are associated with active disease. Cerebral spinal fluid analysis is the most essential test and cranial imaging serves as a supplementary approach, of which CT is preferred. ACL is associated with focal CNS involvement of SLE while antiribosomal P with diffuse CNS involvement of SLE, suggesting there might be different mechanisms in CNS involvement of SLE. Intrathecal therapy is an useful alternative for patients with CNS involvement of SLE refractory to conventional therapy.

[Evaluation of normal submandibular gland function by sialo-scintigraphy].

OBJECTIVE: To study the normal glandular function of the submandibular gland as a background for evaluation of changes in submandibular gland diseases. METHODS: The backgrounds of both submandibular area and temporal area were analyzed comparatively with Emission Computerized Tomography in eleven pairs of normal submandibular gland. RESULTS: About 2.18 times of the temporal background equals to submandibular area background; the Concentrate Index of normal submandibular gland was 1.42 +/- 0.89; the Secretory Index was (109.4 +/- 39.8)%, the Secretory Index Ratio and the Concentrate Index Ratio were (77.2 +/- 17.2)% and 71.0 +/- 15.2% respectively; and the Function Index was (89.9 +/- 7.4)%. CONCLUSION: The use of submandibular area background and functional index to represent submandibular gland function is more accurate and more sensitive. Normal functional index can provide the basis for researching submandibular gland disease.

[Clinical features of 201 cases with Duane's retraction syndrome].

OBJECTIVE: To summarize the clinical features of 201 cases with Duane's retraction syndrome (DRS) and discuss its differential diagnosis. METHODS: The 201 cases in 1979 similar 1996 were retrospectively summarized. The clinical features including chief complaints, sex distribution, age at first visit, laterality, type of presentation, ocular deviation in the primary position, refractive errors, amblyopia, globe retraction, change of the palpebral fissure, upshoot and downshoot in adduction, binocular single vision, and its associated ocular and non-ocular anomalies were analyzed. RESULTS: There were 99 males and 102 females with a female-to-male ratio of 1:1.65.88% of DRS cases had left eye involvement with two-to-one predilection for the left eye. The most common form of the syndrome was type 1 (184 patients, 91.54%). Exotropia was the most common deviation in the primary gaze (72 patients, 35.82%). Among 118 patients, most had abduction deficits, globe retraction in adduction, and face-turn for maintaining single binocular vision. Crocodile tears (26 patients, 12.93%) were the most frequently encountered ocular abnormality. CONCLUSIONS: Diagnosis of DRS in a typical case is not difficult, however, children with bilateral abduction deficits which may mimic DRS must be differentiated from the following four motility disorders, namely, abducens nerve palsy, Moebius' syndrome, congenital oculomotor apraxia, and congenital or infantile esotropia.

[Studies on relationship between occupation and pregnancy outcome].

OBJECTIVE: To study the effects of maternal occupation on reproductive outcomes, including pregnancy outcome and neonatal status, and complications of pregnancy and delivery. METHODS: Data were extracted from 12 224 obstetric records of eight hospitals in Beijing, and all the subjects recruited in the study were interviewed before discharge during from May 1992 to April 1993. Pregnancy and reproductive outcomes were analyzed according to their occupational classifications, adjusted for the potential confounding factors, such as age, smoking and drinking during pregnancy and history of spontaneous abortion. RESULTS: Incidence rates of pregnancy induced hypertention, low birth weight, intrauterine growth retardation and postpartum hemorrhage were higher in the workers than that of the average levels. Incidence rates of threatened abortion, gestational anemia, stillbirth and uterine atony in professional persons, of prolonged pregnancy and neonatal asphyxia in farmers, of low birth weight in housewives, of gestational anemia and of uterine atony in administrative persons all were higher than the average levels. CONCLUSION: Occupation undertaken by pregnant women was associated with the adverse pregnancy and reproductive outcomes to certain extent.

[Clinical study of capsaicin in the treatment of allergic rhinitis].

OBJECTIVE: To study the clinical effect of capsaicin of nasal administration in 100 cases of allergic rhinitis. METHOD: Capsaicin was applied by nostril, once 5 days and 5 times as a therapeutic course. Acidophil were measured and nasal airflow resistance was analysed before and after capsaicin therapy. RESULT: It showed that nasal symptoms of patients were obviously relieved and 94% of the cases in the measure of acidophil changed to negative from positive. Moreover, capsaicin induced decrease in nasal airflow resistance from (0.6272 +/- 0.3534) kPa.s/L before therapy to (0.4057 +/- 0.2419) kPa.s/L after therapy (P < 0.001). 90 cases of 100 patients were fellowed up one year, the total effective rate was 89%. CONCLUSION: Our study suggests that capsaicin possessed remarkable effects in the treatment of allergic rhinitis; analyse of nasal airflow resistance may be used as one of the objective guideline in the determinant of therapeutic effects.

[Effects of microwave acute irradiation on biomechanic properties of rabbit tissues].

The rabbit body was irradiated acutely by 2.45 GHz continuous microwave. After irradiation, the values of blood viscosity, blood viscoelasticity, platelet aggregation, erythrocyte fragility, erythrocyte deformation, erythrocyte aggregation, the compressive mechanic property and flexible property of bones were tested. The data of irradiated group and unirradiated group were compared. The results showed: after microwave irradiation, the rabbit blood viscosity and viscoelasticity decreased; the rabbit platelet aggregation rate decreased and the disaggregation rate increased; the erythrocyte fragility increased; the biomechanic properties of bones did not change considerably.

[Treatment of chronic prostatitis with computerized herbal drug penetrator].

A computerized herbal drug penetrator for chronic prostatitis was designed and developed on the basis of a combination of the authors' clinical practice with modern Traditional Chinese Medicinal theories. The device has integrated many hi-techs of current engineering science. It works by applying special herbal medicine, electro-magnetic effects, thermal moxibustion, perfusion and pulsated massage with water-capsule. The result of using this technique in the treatment of 100 cases has demonstrated its satisfying effectiveness.
