[A recombinant autoantigen U 1 RNP 70,000: expression in E. coli. and its serodiagnostic application].

OBJECTIVE: To express the recombinant autoantigen U 1 RNP 70,000 and to evaluate its serodiagnostic value. METHOD: We used molecular cloning techniques and immunoblotting technique. RESULTS: The recombinant proteins had authentic U 1 RNP 70,000 antigenicity, with the molecular weight corresponding to their theoretical value. 112 of 115(97.4%) anti-70,000 positive sera(IBT) could be detected by r 70,000. CONCLUSIONS: r 70,000 has good specificity and can be used in further detailed epitopes mapping. r 70,000 can be put into use for serodiagnostic detection.

[A pharmacokinetic study of lidocaine in patients undergoing liposuction with tumescent technique].

The purpose of this study was to evaluate the pharmacokinetics of lidocaine in the patients undergoing liposuction after subcutaneous infiltration of 0.06% lidocaine. It was found that the absorption of lidocaine was slow. The mean peak concentration of lidocaine that was 1.16 ug/mg with a range of 0.67 to 2.84 ug/mg occurred 4-13 hours after lidocaine administration. The peak concentration (Y) was correlated with the dose (X) of lidocaine, which was drawn as Y = -0.494 + 0.071X (r = 0.934, P < 0.001). We concluded that the tumescent technique is an effective and safe method for the patients undergoing liposuction.

The relationship between specific RET proto-oncogene mutations and disease phenotype in multiple endocrine neoplasia type 2. International RET mutation consortium analysis.

OBJECTIVE: Multiple endocrine neoplasia type 2 (MEN 2) is an autosomal dominant disorder. The 3 recognized subtypes include MEN 2A, characterized by medullary thyroid carcinoma (MTC), pheochromocytoma (pheo), and hyperparathyroidism (HPT); MEN 2B, by MTC, pheo, and characteristic stigmata; and familial MTC (FMTC), by the presence of MTC only. The purpose of this study was to establish the relationship between specific mutations and the presence of certain disease features in MEN 2 which could help in clinical decision making. DESIGN: Correlative survey study of 477 MEN 2 families. SETTING: Eighteen tertiary referral centers worldwide. PATIENTS: A total of 477 independent MEN 2 families. MAIN OUTCOME MEASURES: Association between the position and type of germline mutation in the RET proto-oncogene and the presence or absence of MTC, pheo, HPT, and/or other features in a family. RESULTS: There is a statistically significant association between the presence of any mutation at a specific position (codon 634) and the presence of pheo and HPT. The presence of a specific mutation, CGC at codon 634, has yet to be associated with FMTC. Conversely, mutations at codons 768 and 804 are thus far seen only with FMTC, while codon 918 mutation is MEN 2B--specific. Rare families with both MEN 2 and Hirschsprung disease were found to have MEN 2-specific codon mutations. Patients with Hirschsprung disease presenting with such mutations should be monitored for the possible development of MEN 2 tumors. CONCLUSIONS: This consortium analysis suggests that genotype-phenotype correlations do exist and, if made reliably absolute, could prove useful in the future in clinical management with respect to screening, surveillance, and prophylaxis, as well as provide insight into the genetic effects of particular mutations.

Effects of enflurane and isoflurane anesthesia on the pharmacokinetics of pancuronium in adult patients.

OBJECTIVE: To evaluate the effects of 1 MAC of enflurane or isoflurane anesthesia on the pharmacokinetics of pancuronium. METHODS: Eighteen adult patients undergoing the elective plastic surgery were randomly divided equally into 3 groups, namely, Group 1 (control group), Group 2 (enflurane group) and Group 3 (isoflurane group). Anesthesia was maintained with thiopental and 67% N2O-O2 in Group 1, 1 MAC enflurane in Group 2 and 1 MAC isoflurane in Group 3. After administration of a bolus of pancuronium 100 micrograms/kg, an improved fluorimetric assay was used to determine the serum concentrations of pancuronium and the pharmacokinetic variables of pancuronium were calculated with a 3P87 program. RESULTS: The disposition of pancuronium may be well described mathematically by a two-compartment open model. The results showed that in comparison with the control group, patients in Group 2 and Group 3 had a longer T1/2 beta and MRT. The patients in Group 2 had the slower K10 and lower CL than those in the control group. There were no significant differences among the three groups in T1/2 alpha, V1, V2, Vdss, V beta, K21, K12 and AUC. CONCLUSION: During enflurane and isoflurane anesthesia, CL of pancuronium decreased, and T1/2 beta and MRT of pancuronium were prolonged in our patients, so duration of the effective plasma concentrations of pancuronium was much longer. As a result, the effects of enflurane and isoflurane of the pharmacokinetics of pancuronium may be part of reason why two drugs extended the duration of neuromuscular depression produced by pancuronium.

[A study on DNA content and cell cycle phase analysis in endometrial carcinoma].

OBJECTIVE: To study the prognostic value of DNA content, cell cycle phase in endometrial carcinoma. METHODS: DNA content and cell cycle phase analysis were performed on 100 paraffin-embedded archival specimens from 103 cases of endometrial carcinoma using flow cytometry. RESULTS: Aneuploidy was found in 71.0% of specimens. The aneuploidy rate in specimens from grade III, deep myometrial invasion and stage III, IV was significantly higher than that in specimens from grade I, nonmyometrial invasion and stage I (P < 0.05 respectively). 5-year survival rate for patients with aneuploidy was 65.9% and for patients with diploidy was 87.6% (P < 0.01). High S-phase fraction (SPF) was associated with high grade, deep myometrial invasion and advanced stage. The mean SPF was 13.4% in the living group and 17% in the dead group. The 5-year survival rate for patients with SPF > 17% was 50.4% and for patients with SPF < 17% was 79.1% (P < 0.01). SPF and stage were the most important prognostic factors in endometrial carcinoma by Cox's proportional hazards model. CONCLUSIONS: DNA ploidy and SPF were found to be of important prognostic indicators in endometrial carcinoma.

[Early postoperative hypoxemia in infants, children and adults undergoing elective plastic surgery].

Seven hundred ASA-class-1 patients undergoing elective plastic surgery were selected to study the effect of age on early postoperative hypoxemia with a pulse eximeter for continuous monitoring of arterial oxygen saturation (SpO2) in the postanesthesia recovery room. The patients were divided into four groups: group 1, 72 infants aged less than one year; group 2, 120 children aged from one to three years; group 3, 364 children aged over three years; and group 4, 144 adults aged from 18 to 58 years. The results showed that the younger the patients, the lower the SpO3 and the higher the incidence of hypoxemia in the early postoperative period. The incidences of early postoperative hypoxemia in the postanesthesia recovery room were 44.4% in group 1, 31.7% in group 2, 17.3% in group 3 and 8.3% in group 4. Early postoperative hypoxemia occurred most commonly within 40 min. in infants and within 15 min. in children aged over one year and adults.

[The pharmacokinetics of pancuronium bromide in infants, children and adults].

Twenty-four patients scheduled for elective plastic surgery were selected to study the pharmacokinetics of pancuronium bromide under enflurane anesthesia. The patients were divided into three groups by their ages; Group 1 consisted of 5 infants (0.75-2.95 years); Group 2 contained 13 children (4-14 years); Group 3 included 6 adults (16-27 years). An improved fluorimetric assay was used to measure the plasma concentrations of pancuronium bromide after administration of pancuronium bromide at a dose of 100 micrograms/kg. The results showed that the disposition of pancuronium bromide may be best described mathematically by a two-compartment open model in all patients. The younger the patients, the larger the distribution volumes and the higher the Cl. There were significant differences among the three groups with regard to V1, V2, Vdss, Cl, AUC. The T1/2 beta and MRT were longer in Group 1 than in Group 2 and Group 3.

Children's development effecting blood oxygen desaturation following apnea.

One hundred and fifty-six children aged from 4 months to 12 years undergoing elective plastic surgery were included in this study. All children were ASA physical status I, under normal development, ranging from 64 to 140 cm in body height and from 6.0 to 41.0 kg in body weight. Following 2-minute preoxygenation, the time when SpO2 dropped to 95% and 90% in apneic period had a close correlation with age, weight and height of children by means of linear and non-linear regression analysis. The smaller the children's weight, the higher the incidence of severe arterial desaturation after reinstitution of manual ventilation with 100% oxygen at SpO2 of 90%. It is suggested that younger children are more susceptible to hypoxemia than older ones during apnea and provided no problem existing other than oxygen delivery, an SpO2 of 95% might be the safe limitation of apnea in pediatric anesthesia induction.

[The role of sympathetic mechanism in the effect of electroacupuncture on immunoregulation].

1. In normal control mice (only injected SRBC) after EA the lymphocyte transformation test (LTT, P < 0.05) and interlukin-2 (IL-2) activity (P < 0.05) were significantly increased, but the serum lysozyme (IZM, P < 0.01) was apparantly decreased. 2. After injection of 6-OHDA in mice, the spleen weight, spleen index, thymus weight, thymusindex, spleen lymphocyte population and serum IZM, igG contents were markedly decreased, whereas LTT, IL-2 and plaque forming cell (PFC) were umchanged, In contrast, the thymus lymphocyte population was markedly increased. 3. After EA of mice axotomized sympathetic nerve with 6-OHDA the above mentioned immune parameters deceased by 6-OHDA could be returned to normal or nearing normal levels. However, the unchanged LTT, IL-2 and the increased thymus lymphocyte population could be enhanced further, especially IL-2. These results suggest that peripheral sympathetic nerve may be participation in the EA-mediating immunological responses.

[A study on expression of tumor suppressor gene p53 in endometrial carcinoma].

OBJECTIVE: To observe the relationship between suppressor gene p53 expression and endometrial carcinoma. METHODS: Expression of p53 was studied by immunohistochemistry in paraffin sections from 23 normal endometrium, 44 endometrial hyperplasia and 103 endometrial carcinoma. RESULTS: Immunohistochemical staining of p53 protein was negative in normal and hyperplastic endometrium but positive in 47.6% of endometrial carcinoma. In endometrial carcinoma, p53 over expression was significantly associated with histologic type, tumor grade, presence of vascular invasion and DNA aneuploidy. The survival rate of patients with p53 expression is significantly lower than that of patients without p53 expression (P < 0.001), the 5-year survival rate is 59.9% and 83.4% respectively. Patients with expression of mutant forms of the tumor suppressor gene p53 products had a 6.34-fold increased risk of death as compared with patients without p53 expression by Cox proportional hazards model. CONCLUSIONS: Aberrant expression of p53 gene is indicative of aggressive biological behavior and poor survival in endometrial carcinoma.

Germline RET mutations in MEN 2A and FMTC and their detection by simple DNA diagnostic tests.

Multiple endocrine neoplasia type 2A (MEN 2A) and familial medullary thyroid carcinoma (FMTC) are two closely related cancer syndromes inherited in an autosomal dominant manner. Mutations in the RET proto-oncogene were found in MEN 2A and FMTC families. In this study we report seven different germline mutations in the RET proto-oncogene in five of five MEN 2A and five of six FMTC families. Each of the mutations involves a cysteine residue in the extracellular cysteine-rich domain of the RET receptor tyrosine kinase. We developed simple polymerase chain reaction based diagnostic tests for all seven mutations in these families.

Rapid identification of overlapping YACs in the MEN2 region of human chromosome 10 by hybridization with Alu element-mediated PCR products.

An overlapping set of 21 yeast artificial chromosomes (YACs) spanning the RET proto-oncogene [Takahashi et al., Oncogene 3 (1988) 571-578] and D10S102 markers on human chromosome 10 was isolated in a series of hybridization-based chromosomal walks in a YAC library. Genetic linkage analyses implicate this chromosomal region as the location of the gene (MEN2A) responsible for multiple endocrine neoplasia type 2A. Four YACs carrying a RET sequence-tagged site (STS) and two YACs carrying a D10S102 STS were used to initiate chromosome walks. These were based on hybridization of Alu element-mediated polymerase chain reaction (Alu-PCR) products from YACs to dot blots of Alu-PCR products from complex pools of YAC clones. The hybridization anchor content of YACs identified in the walks was confirmed by probing blots of Alu-PCR products from individual YACs and by comparing Alu-PCR fingerprints of each YAC. Ten hybridization-based Alu-PCR anchors and three STS anchors were ordered within eleven intervals created by the 21 overlapping YACs. The order of anchors requiring the fewest gaps in the YACs is consistent with the walking results and establishes the STS anchor order as D10S102-D10S94-RET. The overlapping set of YACs represents about 1.55 Mb of the human genome according to restriction mapping of four representative YACs in the contig. These results demonstrate the power of Alu-PCR hybridization for chromosomal walking and provide a rich source of overlapping YACs which can be used to identify candidate MEN2A genes.

kelch encodes a component of intercellular bridges in Drosophila egg chambers.

Oocyte maturation in Drosophila is supported by a cluster of 15 germline-derived nurse cells whose cytoplasm is transported into the oocyte through intercellular bridges called ring canals. kelch was isolated as a female sterile mutation affecting cytoplasm transport. We have cloned the kelch gene and found that it encodes an unusual transcript containing two open reading frames (ORF1 and ORF2) separated by a single UGA stop codon. At least two protein products are made from the kelch mRNA: a short protein from ORF1 and a longer protein from both ORF1 and ORF2 as a result of partial suppression of the UGA codon. The kelch ORF1 product is conserved, and antibodies directed against it are localized specifically to ring canals. Our results suggest that kelch produces a component of ring canals that regulates the flow of cytoplasm between cells.

[Experimental and clinical research of dachengqi decoction in treating post-traumatic respiratory distress syndrome].

UNLABELLED: 12 cases of post-traumatic RDS treated with Dachengqi Decoction were reported. RESULTS: 10 patients survived, which showed a bright prospect for the treatment of post-traumatic RDS. In the experimental study, model of RDS was set up in rabbits. The pressure of PaO2 was determined and the morphologic changes of their lung tissues was observed after the rabbits were perfused with Dachengqi Decoction. The results showed that their PaO2 were markedly increased and lung conditions greatly improved, which provided an experimental basis for RDS with Dachengqi Decoction. Among 12 cases; nine were moderate cases and three serious ones. During the course of treatment with Dachengqi Decoction, two patients died, covering only 16.6% of the total number of patients. This fact confirms the efficacy of Dachengqi Decoction in treating the post-traumatic RDS.

[Synthesis of "danshensu" derivatives].

This paper describes the synthesis of alpha-nicotinyloxy-beta-(substituted phenyl) propionic acid and alpha-(o-acetoxy)benzoyloxy-beta-phenyl propionic acid. They were obtained in six steps starting from benzaldehyde or substituted benzaldehyde. The preliminary pharmacological tests show that all compounds were effective for inhibition of rabbit platelet aggregation.

[On the surgical treatment of pectus excavatum].

80 cases of funnel chest were treated surgically, 9 by Gross procedure, 41 by turn-over technique and 30 by reverse "V" shape osteotomy of the sternum and ribs with internal fixation. There were 56 boys and 24 girls, with ages ranging from 2 to 14 years. The sternal depression varied in depth from 3 to 4 cm, and in volume from 20 to 120 ml. Roentgenographically, there were different extents of deviation of the heart to the left. Some patients had abnormal ECG. To study the results of operations, 48 cases were followed-up for 0.5 to 19 years. We found that the results of reverse "V" shape osteotomy of the sternum and ribs with internal fixation were superior to others. Its main advantage lies in the internal fixation which holds the sternum stable thus favoring wound healing and permanent correction. It is simple with less surgical trauma and postoperative complications. Some disappointing results in the other two techniques, such as uneven chest wall, flat chest and forward protruding of the sternum could also be avoided by this procedure. The internal fixation device was removed one year after operation. Follow-up for 3 to 4 years found the chest wall in satisfactory contour.

Tyrosine hydroxylase maps to the short arm of chromosome 11 proximal to the insulin and HRAS1 loci.

Tyrosine hydroxylase (TH) is the rate-limiting enzyme for catecholamine biosynthesis and a candidate gene for manic-depressive illness. The TH locus was typed for a BglII RFLP using a cDNA clone Ty7 in four large kindreds. Pairwise analyses and multipoint analyses were carried out to map the TH locus more precisely in the region of the linked markers: D11S12, INS, and HRAS1 on 11p. Results confirm the close linkage between TH with these previously mapped markers and support a most likely ordering which places TH on the side of INS where the centromere lies.
