Comparative estrogenic activity of wine extracts and organochlorine pesticide residues in food.

The human diet contains industrial-derived, endocrine-active chemicals and higher levels of naturally occurring compounds that modulate multiple endocrine pathways. Hazard and risk assessment of these mixtures is complicated by noadditive interactions between different endocrine-mediated responses. This study focused on estrogenic chemicals in the diet and compared the relative potencies or estrogen equivalents (EQs) of the daily consumption of xenoestrogenic organochlorine pesticides in food (2.44 micrograms/day) with the EQs in a single 200-ml glass of red cabernet wine. The reconstituted organochlorine mixture contained 1,1,1-trichloro-2-(p-chlorophenyl)-2-(o-chlorophenyl)ethane, 1,1,1-trichloro-2,2-bis(p-chlorophenyl)ethane, 1,1-dichloro-2,2-bis(p-chlorophenyl)ethylene, endosulfan-1, endosulfan-2, p,p'-methoxychlor, and toxaphene; the relative proportion of each chemical in the mixture resembled the composition reported in a recent U.S. Food and Drug Administration market basket survey. The following battery of in vitro 17 beta-estradiol (E2)-responsive bioassays were utilized in this study: competitive binding to mouse uterine estrogen receptor (ER); proliferation in T47D human breast cancer cells; luciferase (Luc) induction in human HepG2 cells transiently cotransfected with C3-Luc and the human ER, rat ER-alpha, or rat ER-beta; induction of chloramphenicol acetyltransferase (CAT) activity in MCF-7 human breast cancer cells transfected with E2-responsive cathepsin D-CAT or creatine kinase B-CAT plasmids. For these seven in vitro assays, the calculated EQs in extracts from 200 ml of red cabernet wine varied from 0.15 to 3.68 micrograms/day. In contrast, EQs for consumption of organochlorine pesticides (2.44 micrograms/day) varied from nondetectable to 1.24 ng/day. Based on results of the in vitro bioassays, organochlorine pesticides in food contribute minimally to dietary EQ intake.

The carriage of Escherichia coli resistant to antibiotics in healthy populations in Shanghai.

Healthy populations represent the largest reservoir of bacteria resistant to antibiotics. We investigated the resistance of Escherichia coli to 12 antibiotics in fecal samples from untreated healthy populations in Shanghai, China by using Kirby-Bauer (K-B) method. The results showed that: (i) All subjects carried resistant strains of Escherichia coli. (ii) The carriage rates of Escherichia coli resistant to various antibiotics were different, less than 10% to amikacin and 30% to 100% to others. (iii) In the elder children group aged 10-11 years, the percentages of strains resistant to gentamicin, streptomycin, chloramphenicol, tetracycline, trimethoprim, and sulfamethoxazole were significantly lower than those in the younger group aged 5-6 years. In the adult group, the percentages of strains resistant to ampicillin, piperacillin, amikacin, streptomycin, chloramphenicol, tetracycline, trimethoprim, and sulfamethoxazole were significantly lower than those in the elder children group. (iv) The number of strains resistant to five or more antibiotics accounted for 31.8% in the younger children group, 23.7% in the elder children group, and 12.1% in the adult group. These findings suggest that all healthy people in Shanghai carry resistant strains of Escherichia coli in the intestine. The younger the populations, the higher the level of resistance of fecal Escherichia coli to antibiotics. Improvement of health behaviors and environmental sanitation and rational use of antibiotics could remarkedly decrease the resistant level of bacteria.

Study on the transmission threshold value of bancroftian filariasis.

OBJECTIVE: To elucidate the transmission dynamic and epidemic trend of bancroftian filariasis occurred under the condition with no control measure taken 5 years after elimination of filariasis. METHODS: A 10-year longitudinal observation (from 1984 to 1994) was made in Huayuan Village in Shengli Township of Tancheng County, which used to be a high bancroftian filariasis-endemic area in southern part of Shandong Province. RESULTS: The microfilarial rate decreased from 0.56% before the study to 0.12% after the study and 8 out of the 9 previous microfilaria-positive cases became negative gradually. During the study period, 6 new microfilaremia cases were detected, 5 of which became negative naturally within 3 to 4 years. Eighty-eight point eight nine per cent of the detected patients with microfilaremia converted into IgG4-negative after 10 years. The natural infective rate of vectors decreased year by year and became zero by the tenth year of the study, the annual transmission potency decreased also from 3.47 to zero by the tenth year. CONCLUSIONS: It showed that under the local natural environment the biting rate representing the vector density which was obtained by capture method was from 24.1 to 52.5 person/night among the residents who did not use mosquito nets, and 13.5 to 21 person/night among the residents who used mosquito nets. The microfilarial rate of 0.56% in population with the average microfilarial density of 6.6 to 20.7 capita/60 microliters ear blood of residual microfilaria-positive patients might be considered as the terminal threshold of transmission.

Lifestyle evaluation system to support health education.

A computer-based system for evaluation of one's lifestyle was designed so that public health nurses could use it for health education to improve a patient's lifestyle and to prevent lifestyle-related diseases. The Lifestyle Evaluation System (LES) is a computer program that works on a personal computer. The LES is consist of four parts; inputting personal data, answering the questionnaire, showing the result and getting health check up data. The questionnaire includes 40 questions regarding diet, smoking, drinking, physical activity, rest, social activity and health care activities, based on Breslow's seven lifestyles. The result offers participants' lifestyle evaluation in forms of values, graphs, tables and messages. Evaluation values are deviation value, rank, BMI and standard weight. The LES also offers participant's periodic health check data, average data derived from all residents' data, and past results of LES so that public health nurses are able to use these for health education. The health check data is obtained from another database stored on floppy disks. The participants input data by themselves and get result immediately. The interactive style is effective in raising interest in health education.

[Experimental study on electrophysiological evaluation of functional recovery in motor nerve regeneration].

To investigate the objective method for electrophysiological examination in evaluating the functional recovery in motor nerve regeneration, 30 rabbits were divided into 5 groups randomly. The common peroneal nerve on left side of every rabbit was sectioned and repaired by epineural suture, while that of the right side was left intact as control. In 2nd, 4th, 8th, 12th and 16th week after operation, the muscle power and the changes of the electrophysiological parameters of the nerve and the muscle were determined dynamically. The linear correlation analysis was used to assess their relationship. The results showed that the electrophysiological parameters and muscle contractibility revealed signs of recovery in parallel. There was a significant linear relationship among the amplitude of the muscle action potential, velocity of nerve-muscle conductivity and muscle contractibility. The conclusion was that the electrophysiological examination of motor nerve and muscle could be used to assess the regeneration of the motor nerve, and it would also reflect the recovery of muscle contactibility in the early stage.

[Antenatal diagnosis of albinism fetuses by fetalscopy].

OBJECTIVE: To popularize the clinical utility of fetalscopy in antenatal diagnosis of albinism fetuses. METHODS: Fetalscopy examination was carried out in 85 high-risk pregnancy women, during 17 to 26 gestational weeks. Guided by the ultrasonography, the fetalscopy was inserted into the aminotic sac. Albinism fetuses was diagnosed by the color of the fetal hair. RESULTS: Fetalscopy were succeed in 79 of 85 high-risk women (93%). 63 fetuses were diagnosed as normal, 16 fetuses was diagnosed of albinism. There were 2 false-positive cases at 17 weeks of gestation due to mistaking wooly hair for hair. CONCLUSION: Fetalscopy is a quick, accurate, safe method in antenatal diagnosis of heredity diseases, such as albinism.

[Laboratory diagnosis of Chlamydia trachomatis and Ureaplasma urealyticum of chronic prostatitis].

Prostatic fluid taken from 1038 cases of chronic prostatitis were detected by fluorescent monoclonal antibody technique and isolating cultural method for Chlamydia Trachomatis (CT) and Ureaplasma Urealyticum (UU). The control group consisted of 80 cases. CT and UU were not found in control group. The positive rates of CT and UU infection were 28.4% and 33.0% respectively in the tested group whose cases were abacterial prostatitis. The results suggested that CT and UU are pathogens causing chronic abacterial prostatitis.

[Relationship between extracellular matrix and progressive growth of malignant tumor].

OBJECTIVE: To explore the role of extracellular matrix proteins in tumor cell invasion and metastasis. METHODS: Mouse pulmonary adenocarcinoma cell line LA795 and human nasopharyngeal carcinoma cell line CNE-2Z were implanted subcutaneously and under the renal capsule in T739 and nude mice, respectively. At different time intervals, expression of fibronectin(FN), laminin(LN) and type IV collagen(IV C) in the tumor extracellular matrix and in tumor adjacent tissues was examined by indirect immunoperoxidase and immunofluorescence techniques. In addition, various in vitro methods (random migration of tumor cells in agarose droplet, growth of tumor cell colonies in soft agar, dot blotting, etc.) were used to analyse the effect of exogenous LN, FN or IV C. RESULTS: As tumor grew, the expression of FN, LN or IV C increased and presented different distribution patterns. Exogenous FN, LN and IV C enhanced the mobility of tumor cells and the growth of the tumor cell colonies. CONCLUSION: The distribution difference and the changes in synthesis and degradation of the extracellular matrix proteins are valuable in predicting the biologic behaviour. Tumor implanted under the renal capsule of mice offers a better experimental model to study the relationship between extracellular matrix and tumor cell invasion.

[Radiation-induced apoptosis of two nasopharyngeal carcinoma cell lines].

OBJECTIVE: To study apoptosis induced by radiation in two nasopharyngeal carcinoma (NPC) cell lines, CNE and CNE-2. METHODS: Hoechst 33342 staining, immunocytochemical staining, RT-PCR, DNA dot blotting and Southern blotting were used to identify apoptosis. RESULTS: A single dose of X-irradiation resulted in apoptosis, the apoptotic index (AI) was time- and dose-dependent. Different apoptotic responses existed in the two cell lines. Immunocytochemical staining showed that bcl-2 protein was strongly positive in CNE but negetive in CNE-2. However, RT-PCR revealed p53 mRNA in CNE-2 but not in CNE. p53 and bcl-2 genes were both present in the two cell lines as shown by DNA blotting, but the 2.8 kb fragment of the p53 gene was much lower than the 5.6 Kb fragment in CNE which was clearly shown in Southern hybridization, suggestive of partial deletion of p53 gene in CNE. CONCLUSION: Apoptotic response to radiation is different in 2 NPC cell lines. CNE is more radio-resistant than CNE-2. Overexpression of bcl-2 protein and partial deletion of p53 gene may explain their difference in radio-sensitivity.

[The clinical significance of multidrug resistance associated protein(MRP) gene expression in acute leukemia].

OBJECTIVE: To investigate the relationship between the expression of the multidrug resistance-associated protein(MRP) gene and clinical drug resistance in acute leukemia(AL). METHODS: Semi-quantitative reverse transcriptase polymerase chain reaction was used to examine the expression of MRP gene in 65 bone marrow aspirates of AL patients and 15 normal volunteer's peripheral blood mononuclear cells. The expression level of MRP were expressed as ratio of MRP/beta 2M(beta 2-microglobulin) and the ratio of MRP/beta 2M > or = 0.3 was defined as MRP positive. RESULTS: The level of MRP mRNA and the positive percentage of MRP in relapsed-refractory group were significantly higher than that in normal control group. In newly diagnosed group, the first complete remission rate of MRP negative patients(84%) was significantly higher than that of MRP positive(25%) patients. MRP expression level was different in leukemia sub-type. MRP and mdr-1 genes were examined simultaneously in 65 AL patients, the mean expression level and positive percentage of MRP and mdr-1 in clinical resistance group were significantly higher than those in non-resistance group. No significant correlation was found between the MRP and mdr-1 expression(rs = 0.1683). CONCLUSION: High expression of MRP leads to drug resistance and it is an unfavorable factor to prognosis.

Changes of peripheral tissue thyroid hormone metabolism in rats fed with selenium- and vitamin E-deficient artificial semisynthetic diet.

OBJECTIVE: To examine the relationship between selenium (Se)- and vitamin E(VE)-deficiency and thyroid hormone (TH) metabolic disturbance, especially type I iodothyronine 5'-deiodinase (ID-I) activity. METHODS: We observed the metabolic changes of TH-3, 3',5-triidothyronine (T3) and thyroxin (T4) and their free radicals-glutathione peroxidase (GSH-Px) and lipid peroxides (LPO) in the peripheral tissues (liver, kidney and blood) of Wistar rats maintained on Se- and VE-deficient artificial semi-synthetic diet for 8 weeks. RESULTS: In the Se- and VE-deficient rats (compared with Se- and VE-supplement rats): 1). Hepatic and renal ID-I activities decreased by 60% and 50% respectively, serum levels of T3 reduced 36%, and T4 increased by 32%. Hepatic ID-I activity and serum T3 concentration in Se- and VE-supplement rats were significantly higher than those in the Se-supplement and VE-deficient rats. 2). GSH-Px activities in the whole blood and liver decreased by 61% and 82% respectively. LPO concentrations in serum and liver increased by 53% and 40% respectively. When Se content remained the same, changes of VE content did not significantly affect GSH-Px activity, but the combined supplement of Se and VE decreased significantly the LPO concentration. CONCLUSIONS: The data suggests that GSH-Px activity is influenced mainly by Se level, but ID-I activity is influenced by VE concentration in addition to Se level. VE seemed to play a protective role on ID-I and its mechanism might be related to the fact that VE protects the stability of microsomal membrane in which ID-I exists, avoiding from free radical damage as well as the coordinated action and mutual sparing effect of Se and VE.

[Effects of non-focused ultrasound on the cochlea in guinea pigs].

Guinea pigs with normal preyer's reflex were researched for the morphological and the enzymatic change of cochlea under the influence of 2.5 MHz, 8 MHz non-focused ultrasound irradiation for 3 hours or 6 hours. The results showed that no histopathological changes could be found. But enzyme histochemistry studies showed that 2.5 MHz or 8 MHz non-focused ultrasound irradiation for 6 hours has led to enervation of SDH activity and enhancement of LDH activity of hair cells in the 0.87 +/- 0.20 or 0.80 +/- 0.20 mm areas of the second circle or the first circle respectively. The results suggest that the non-focused ultrasound irradiation for some dose may cause special injury of cochlear hair cells.

Myospasm of internal anal sphincter in children.

OBJECTIVE: To introduce a common but not well-recognized acute abdominal disease in children. METHODS: Thirty-eight children with spasm of the internal anal sphincter were reviewed. RESULTS: The main symptom was the sudden onset of continuous pain in the lower abdomen with paroxysmal exacerbation. The typical sign was the dilation of the sigmoid colon with tenderness aggravated by pressure. Plain X-ray films showed dilation of the sigmoid colon and rectum. The symptoms could be relieved by digital rectal examination or a glycerin suppository. CONCLUSIONS: It is the temporary achalasia of the internal anal sphincter that causes acute abdominal pain.

The influence of impaired microvasculature on regional blood flow of the spinal cord after microsurgery.

OBJECTIVE: To further investigate the impact of impaired microvasculature of spinal cord and its physiological compensation on postoperative morbidity after intramedullary microsurgery. METHODS: In 120 cats, the segmental anterior longitudinal spinal arteries (ALSA), the posterior longitudinal spinal arteries (PLSA), and the unilateral radiculomedullary arteries (RMA) were selectively coagulated in different patterns. Hydrogen electrode technique was used to detect the changes of regional blood flow of the spinal cord at different segments. Benzidine dihydrochloride (BDHC) staining was used to observe the microvascular pattern of the spinal cord, tetrazolium chloride (TTC) staining was applied to morphometric analysis of the ischemic area, and hematoxylin-eosin staining was applied to histologic examination. RESULTS: When the segmental ALSA was injured, the blood flow of the gray matter and white matter decreased greatly. Since the central arteries were the terminal blood supply arteries, no compensation occurred after the injury. The blood flow of the segment below the impaired segment hardly changed, indicating that the direction of the blood flow in ALSA altered to adapt the changes of the microvasculature. Injury to the unilateral PLSA and RMA at both cervical and lumbar area only caused a minor decrease in the regional blood flow, unless the perimedullary arterial system was considerably injured. In the thoracic medullary segment, the sparse microvasculature was the main cause for the segment vulnerable to ischemia and infarction. This was caused by not only the rarity of RMA as an anatomic factor, but also the small number and low activity of the neurons at this area. After microsurgery, the impairment and decompensation of the microvasculature were closely related to the ischemic volume of and pathological changes in the spinal cord. CONCLUSION: The impairment and decompensation of microvasculature after microsurgery are the rudimentary causes of spinal cord ischemia. During operation, one should protect the terminal arteries to decrease the severity of injury to the perimedullary system, and do the best to avoid disturbance of microvasculature to accelerate the recovery of postoperative ischemia and neurological dysfunction of the spinal cord.

[Relationship between lipometabolic disorder in stroke with TCM syndrome differentiation and typing].

OBJECTIVE: To analyse the relationship between lipometabolic disorder in stroke with TCM Syndrome Differentiation and Typing. METHODS: The serum lipid, lipoprotein and apo-lipoprotein level of TCM treated group (142 cases) and control group (100 cases) were determined. The patients were subdivided into three Type-groups according to TCM Syndrome Differentiation: Phlegm-Dampness Stagnation (PDS), Yin Deficiency with Yang Hyperactivity (YDYH) and Qi Deficiency with Blood Stasis (QDBS) group. RESULTS: The results showed that the level of TC, TG, HDL, LDL, apoA1, apoB and apoA1/apoB were significantly different in PDS and QDBS as compared with control group (P < 0.05, P < 0.01, respectively), while lipometabolic disorder of PDS was most serious in TCM Syndrome Differentiation of stroke. The level of TG, apoA1/apoB were significantly different in YDYH in comparing with control group (P < 0.05). While the level of TC, HDL, LDL and apoB were insignificantly different than those of control group (P > 0.05). CONCLUSIONS: This study showed that lipometabolic disorder of stroke were correlated with Syndrome Differentiation of TCM. Stroke with PDS and QSBS should give removing Phlegm-Dampness, dredging Meridian and removing blood stasis treatment besides routine treatment.

[Effect of castor oil-diet on the initiation of labor of pregnant rat].

OBJECTIVE: This study was designed to explor the effect of castor oil-diet, on the intiation of labor of preganant rat and determine what the active component of castor oil-diet was. METHODS: The time of the intiation of labor and course of delivery were observed by gavaged rats castor oil-diet at 18, 19 and 20 days of gestation. The compositions of fat acid of castor oil-diet and castor oil were analyzed by HPLC. RESULTS: The castor oil-diet could induce the initiation of labor and shorte the course of the delivery in pregnant rats. Ricinoleic acid was the active component of castor oil-diet in this study. CONCLUSIONS: Pregnant rats by gavaged castor oil-diet could serve as the animal model for induction of labor. Ricinoleic acid is likely the chief component to the induction of labor.
