[The dissociated hypersthesia in Vincent's symptom--an indication of the etiology].

The so-called Vincent's symptom is a functional disturbance in the region of the distribution of n. alveolaris mand. Etiologically, it may be due to pathologic disorders of different origin. In fractures of the mandible with rupture of the nerve in the mandibular canal, complete anesthesia is observed. In inflammatory processes pain is first felt differently, later anesthesia is noted. In the case of a space-occupying lesion, tactile differentiation is disturbed first and pain sensation only after a relatively long time. In the case of infiltrating tumors, a neuritis is noted first (late Vincent's symptom). Thus, partially disturbed sensitivity may be a helpful symptom for differential diagnostic considerations, as is demonstrated in a number of cases.

Osseointegration of titanium fixtures in onlay grafting procedures with autogenous bone and hydroxylapatite. An experimental histometric study.

The aim of the present study was to determine the degree of osseointegration of fixtures in different onlay grafting materials. Hydroxylapatite blocks of 2 different pore sizes and free monocortical bone grafts from the iliac crest were inserted in Göttingen minipigs and fixed with 2 titanium fixtures. The extent of the bone/implant interface area on the fixture surface was determined from histological specimens by morphometric measurements and related to the total thread surface of the fixtures. Fixtures inserted into bone grafts showed the most complete degree of osseointegration, whereas the smallest bone/implant interface area was found with fixtures inserted into those HA blocks with the smaller pore size. There was a significant decrease in the degree of osseointegration between the host bone site and the augmentation material only with the fixtures inserted into the HA blocks of smaller pore size.

Strong association of interleukin-6 -174 G>C promoter polymorphism with increased risk of oral cancer.

In view of the recently found contribution of factors associated with thrombosis and inflammation to carcinogenesis, we investigated the possible association of interleukin-6 (IL-6) with an increased risk of oral cancer. In DNA samples of 162 patients with oral squamous cell carcinoma and 156 healthy controls of comparable ethnicity, age and sex, we studied the -174 G>C polymorphism in the IL-6 gene, which affects its transcription. C allele frequencies were significantly increased in patients compared to controls, 42.6% versus 23.1% (p<0.001). The CC homozygotes had a 7-fold greater risk of developing oral cancer (odds ratio 7.39, 95% CI 2.61-20.92), while the GC heterozygotes had a 4-fold greater risk (odds ratio 3.74, 95% CI 2.29-6.11). A significant increase in C alleles was observed in patients regardless of their smoking or alcohol consumption habits, early or advanced stage of cancer, and presence or absence of a family history for cancer or thrombophilia (p<0.001; Fisher's exact test). These findings suggest that the -174 G>C polymorphism, by affecting IL-6 gene expression, is strongly associated with oral oncogenesis.

Correlation of telomerase activity, clinical prognosis and therapy in oral carcinogenesis.

Telomerase activity is associated with most malignant tumors. To evaluate the role of telomerase reactivation as a diagnostic and prognostic marker in oral carcinogenesis activity was investigated in mortal and immortal cell lines in eight oral leukoplakias (OL) and 46 oral squamous cell carcinomas (OSCC). Activity was also investigated in 13 histopathologically unaffected mucosas from distant sites or tumor-free margins of the same patients using a modified, highly sensitive, non-radioactive telomeric repeat amplification protocol (TRAP). This was correlated with histopathological stages and the clinical course of the disease. 50% of OL and 46% of OSCC showed activity. One patient with positive, high dysplastic OL developed an OSCC 11 month later. One of three specimens of adjacent tissue presented activity and a recurrence occurred after 6 months. Out of 10 tissues of distal normal mucosa, 2 demonstrated activity which could also be proved in the corresponding tumor. Detection of telomerase reactivation may be a novel method for early detection of immortalised cell clones and malignant cells in histopathologically normal oral squamous epithelium.

[Osseointegrated implants for the retention of restorative jaw prostheses and facial prostheses for functional and esthetic rehabilitation following tumor surgery].

To restore the masticatory function after tumor surgery, 110 transmucosal intraoral implants have been inserted in 21 patients, and 26 percutaneous implants in a total of 9 patients for the stabilization of facial prostheses. While 12 of the intraoral implants had to be removed for lack of bony integration, the losses of the percutaneous implants for the stabilization of facial prostheses were exclusively due to tumor recurrences in the direct vicinity of the implants. Our previous results in the use of osseointegrated implants for the retention of restorative maxillofacial prostheses show a limited incidence of complications and give reason to expect long-term success in functional and esthetic rehabilitation. This method provides a decisive improvement in the quality of the life of tumor patients compared to the procedures used up to now.
