[An echographic, serologic and radiologic register of human hydatidosis. Contributions to a control program].

Between June and November 1984, 904 asymptomatic people from endemic areas of hydatidosis in Rio Negro, Argentina, were studied by means of ultrasonography (U.S.) and double diffusion for arc five (dd5). The population included 272 inhabitants from Pilcaniyeu, 55 patients from the Zonal Bariloche Hospital admitted for diseases other than hydatidosis and 577 recruits from different departments of the Rio Negro Province. A chest X-ray was performed in every recruits. 47 (5.20%) cases of hepatic hydatidosis were detected by U.S.; 11 (1.22%) were detected by dd5 (p 0.01); 2 (0.34%) were detected by chest X-ray. Due to the low sensitivity of dd5 a presumptive diagnosis of hydatidosis should be made in every patient proceeding from an endemic area with a liver cyst diagnosed by ultrasound, even if dd5 is negative. U.S. must be incorporated as an elective method associated with chest X-ray and dd5 in epidemiological yielding and monitoring of control programs for hydatidosis.

[Diabetes and hypothyroidism].

A 7 1/2 year old girl, diagnosed as having insulin dependent diabetes "mellitus" and while receiving treatment, a clinical and analytical hypothyroidism was detected. She was found to have a level of antimicrosomal antibodies of 1/102,400. Discovery of microsomal antibodies in insulin dependent diabetic children is frequently mentioned in medical literature, whereas in 25 diabetics at this Centre these findings proved to be negative. Clinical association of both these illnesses is not common.

Treatment of chronic portal--systemic encephalopathy with vegetable and animal protein diets. A controlled crossover study.

A controlled crossover clinical comparison of 40-g/day and 80-g/day vegetable protein diets vs a 40-g/day meat protein diet plus neomycin-milk of magnesia (as control therapy) was performed on 10 cirrhotic patients with mild chronic portal-systemic encephalopathy. The 40-g vegetable protein diet had a high fiber volume and contained low methionine and low aromatic amino acids. The 80-g vegetable protein diet was rich in branched-chain amino acids and fiber, with a similar content of sulfur-containing amino acids as compared to the 40-g meat protein diet. Serial semiquantitative assessments were done, including mental state, asterixis, number connection tests, electroencephalograms and blood ammonia levels. No patient developed deep coma while ingesting either vegetable protein diet or neomycin-milk of magnesia plus 40-g meat protein diet. A significant improvement in the number connection test times was observed during the 40-g vegetable protein diet (P less than 0.05) and during the 80-g vegetable protein diet (P less than 0.05) as compared to their previous 40-g meat protein--neomycin periods. In addition, during the period of 80-g vegetable protein diet, the patients showed a significant improvement in their electroencephalograms (P less than 0.05). The frequency of bowel movements significantly increased (P less than 0.05) during the 80-g vegetable protein diet period. During the 40-g vegetable protein diet, two cirrhotic--diabetic patients experienced hypoglycemia. Three patients complained of the voluminous 80-g vegetable protein diet. Patients with mild portal--systemic encephalopathy may be adequately controlled with vegetable protein diets as a single therapy.

Calcium and renin release: inhibition of low sodium-induced renin secretion by high calcium concentration in rat kidney perfusion.

The effects of changes in calcium on renin secretion have been studied in the isolated perfused rat kidney. Perfusion with free calcium buffer significantly decreases renin secretion as compared with control experiments (Ca++: 2.5 mM/l). Other calcium concentrations (1.25 mM/l) and 5 mM/l) do not affect basal renin secretion. When the renin release is previously increased by low sodium concentration (Na+: 110 mM/l) however, perfusion with high calcium buffer (Ca++: 5 mM/l) significantly inhibits this stimulation.

Inhibition of the renin-angiotensin system by pinealectomy in female rats.

The influence of the pineal gland on the renin-angiotensin system has been studied in female rats. Pinealectomy produces a decreased plasma renin activity (PRA) concomitant with an increased corticosterone level, and with no changes in plasma aldosterone. Administration of indol melatonin reverses the effects of pinealectomy. On the other hand ovariectomy induces an increased PRA while decreasing corticosterone and aldosterone levels. However when pinealectomy and ovariectomy are both performed, neither PRA nor corticosterone changes. These results suggest that the pineal gland may exert an influence on the renin-angiotensin system via adrenal and/or ovarian pathways.

Calmodulin in brain of schizophrenics.

A heat-stable Ca2+-dependent regular protein with the characteristics of calmodulin was extracted and purified from striatum and cerebral cortex of autopsied human brains. The human calmodulin preparation cross-reacts with Ca2+-dependent phosphodiesterase isolated from rat, bovine, and human cerebral cortex. There was no significant difference between calmodulin content measured in membranes obtained from striatum or cortex of nine control subjects without a psychiatric history and seven schizophrenic individuals.

[Hypothyroidism caused by isolated TSH deficiency].

Hypothyroidism secondary to isolated thyrotropin (TSH) deficiency was confirmed by low TSH levels during TRF test. Other anterior pituitary hormones were normal. Replacement therapy with thyroxine improved the child's growth but not the intellectual ability. The diagnostic difficulties and the importance of early treatment are emphasised.
