[Clinical and sonographic study of the infant hip. A prospective study].

In 1985 a clinical and sonographic study in 693 children was done. The instability and the limitation of the abduction of the hip joint proved to be the most important clinical signs. The weight of the newborn and the time of birth had no influence on the hip configuration. The importance of the sonographic and clinical examination was demonstrated in the follow up. In the ultrasonic examination qualitative signs rather than quantitative measurements were preferred in routine diagnosis.

[Echography of the hip in children. Diagnostic importance].

This paper reports a technique of study of infant hip with ultrasound. Author report observations with 160 sonograms in 40 children. Luxation and subluxation of the hip can be visualised. Hip sonography technique is easy and without exposure to radiation.

[Ultrasonic and anatomical correlation of the normal hip in the newborn infant].

Ultrasound studies were made on the cadavers of seven normal new-born foetuses. The sonographic images obtained were compared with anatomical sections made in corresponding planes. An accurate analysis of the anatomy of the cartilaginous acetabular roof and labrum in these hips confirmed the accuracy of ultrasonography. Ultrasonography has made it possible to resolve certain doubts about the radiographic examination of infants between birth and the third month.

[Fatigue fractures in the area of the hip joint in joggers].

An increased incidence of fatigue fractures of the lower extremities has been observed as a result of the jogging craze. The authors describe a fatigue fracture of the neck of the femur and a fracture dislocation (not described to date) of the hip joint with fracture of the posterior acetabular margin and avulsion of a femoral head fragment.

[Alternatives to endoprostheses of the hip and knee joint].

Total hip and knee replacement is one of the most successful operative procedures in orthopaedic surgery. With respect to greater life expectancy, however, loosening of the implant and revision surgery with all its sequelae must be taken into account. Therefore joint preserving procedures resp. arthrodesis of the hip and knee joint are still of great importance. The best long term results can be achieved by operations which can improve the biomechanics of the joint. Operative procedures which aim at the biology of the joint only lead to temporary pain relief. Arthrodesis of the hip and knee joint should always be considered in young patients with no possibility for joint preserving techniques.

[Sintering behavior of vertebral body fractures during treatment with the 3-point brace].

The following conclusions were drawn from a follow-up study of 62 patients who had been treated with the three-point brace for vertebral fracture: It was found that the only statistically significant factor influencing an increased "sintering" is the additional involvement of the posterior wall of the vertebral body. Final results are not significantly influenced by preexisting osteoporosis, amount of primary deformation, accompanying lateral compression, age and sex, period of wear of the three-point brace, and follow-up examination interval. We can conclude from the results that "sintering" of the vertebral bodies after fracture is mostly completed already 6 weeks after the accident. No reduction of vertebral fractures was observed. The remaining deformation of the vertebral body must, therefore, be considered in connection with the patient's age in all cases of early functional or repositioning or immobilising treatment.

[Backache and low back pain. Diagnosis and treatment].

In patients with back pain there is only a poor correlation between clinical and radiological symptoms. Therefore the interpretation of radiological findings is only possible with respect to the natural history of the disease. Indication for radiological examination is given for early diagnosis and treatment of malformation of the spine; diagnosis and treatment of functional disturbances of the spine with back- and low back pain and diagnosis and treatment of diseases which affect the stability of the vertebrae.

[Consequences of hemarthrosis for the knee joint (author's transl)].

The presence of an hemarthrosis generally is a sign of a particularly severe joint lesion. Arthrous alterations appearing post-traumatically can therefore be due to the extent of the joint lesion as well as to the accompanying hemarthrosis. A group of 24 young adult rabbits was submitted to morphologic examinations in order to find out in which way an isolated hemarthrosis can be responsible of degenerative cartilage alterations. At intervals of eight days each, 3 cm3 of their own blood were injected into both knee joints of the rabbits. One knee joint was immobilized, the other was mobile. The results prove that the hemarthrosis represents a considerable risk with regard to the trophism of the concerned joint. A direct enzymatic damage to the joint cartilage alone or in combination with trophical alterations appearing later on can lead to a formation and aggravation of arthrosis. The degenerative alterations are increased in an immobilized joint. Bearing in mind these alterations, the conclusion is drawn for clinical practice that the hemarthrosis has to be considered when establishing both diagnosis and therapy plan (punction, joint lavage, drainage, immobilization as early as possible).

[Changes in the shape of the lumbar vertebral column in patients with legs of unequal length (author's transl)].

Little is known about how static scoliosis is related to asymmetries of the lower extremities. In 117 patients we could find typical reactions of the juvenile and adult spine upon leg length discrepancy. In children scoliosis is long-curved, in adults it is short-curved and located in the presacral segments. The different behaviour can be referred to rotatory mechanics of the facet joints. The juvenile spine is especially susceptible to structural deformities with vertebral rotation. The deformities mostly develop at the vertebral arch structures. They are dependent upon the degree and duration of leg length discrepancy. Hence asymmetries of the articular facets can be considered as minimal torsion deformities.

[Radius head resection as a reconstructive measure of the elbow joint].

The authors followed up 55 patients to check on the clinical manifestations prevailing ten years after resection of the head of the radius with particular reference to the pattern presented in the radiocarpal joint. Particular attention was paid to functional and roentgenological changes in the regions of the elbow and wrist. Overall results were unsatisfactory, although a clear improvement in function could be seen in old fractures of the head of the radius, although the function did not improve beyond that achieved by primary resection of the head. No convincing improvement was obtained in old dislocations of the radial head (most of them being accompanied by additional injuries) by a resection of the head of the radius. With the progress of time after the operation, the overall result deteriorated, depending on the likewise slowly progressing proximal dislocation of the radius and hence negative influence exercised on the biomechanics in the distal radioulnar or wrist joint. On the basis of these results it is recommended to attempt radial head preserving treatment as the initial measure in fractures of the head of the radius, i.e. to promote osteosynthesis as far as ever possible.

Strain on the interarticular stress distribution. Measurements regarding the development of spondylolysis.

In discussion the aetiology of spondylolysis, attention has been increasingly directed to the biomechanics of the lumbar vertebral column and the lumbo-sacral transition. The theory of a traumatic induction of spondylolysis was tested with stress distribution measurements on the vertebral column of cadavers. Using stretch measuring strips it was possible to investigate, the strain exerted upon the interarticular portion of the intact lumbar vertebral column preparation. A particular strain upon the pars interarticularis occurs through hyperextension, axial stress and torsion of the lumbar vertebral column. It is worth to be mentioned that force transmission within the vertebral arch structures is influenced by the positioning of the vertebral arch joints. Sagittaly positioned vertebral arch joints lead to a greater strain exerted upon the interarticular portion. Our results have to be compared with those of others, who are of the opinion that spondylolysis is a condition resulting from a fatique fracture.

[The early radiological findings of idiopathic necrosis of the femoral head in adults (author's transl)].

Idiopathic necrosis of the femoral head in adults occurs in about 2 to 3% of all cases of degenerative hip disease. Advanced necrosis with collapse of the head offers a difficult therapeutic problem in younger patients. Early radiological diagnosis is therefore important. The typical, early radiological findings were studied in a hundred patients with idiopathic necrosis of the femoral head. They consist of an area of sclerosis at the junction of the head and neck, with a periosteal response on the inferior margin of the neck. The relationship of these changes with mechanical stress and defective blood supply is discussed. Signs of more advanced necrosis are band-like sclerosis at the point of demarkation and sub-chondral decalcification of the femoral head. Recognition of the early radiological features improves the chances of successful treatment, particularly in the lower age group.

[Metabolic diseases and idiopathic ischemic necrosis of the femoral head in adults (author's transl)].

100 patients suffering from osteonecroses (including 95 with necroses of the head of the femur) were subjected to comprehensive clinical, roentgenologic and chemical laboratory examination. A triangle was established from the range of factors considered as pathogenetically important by various authors. These three outstanding factors were: disturbance of lipid metabolism, found in 41% of the patients, deviation of the liver function data, and, finally, abuse of alcohol as discovered on the basis of the case history. The statistically significant correlation of these factors makes it probable that there is actually a pathogenetic chain abuse of alcohol--hyperlipoproteinemia--arteriosclerosis--osteonecrosis. No specific importance of the uric acid and glucose metabolism could be established, since the metabolic disturbances associated with these processes are mostly linked with the observed hyperlipoproteinemia. Deliberations regarding the genesis of necrosis of the head of the femur must also take into account, over and above the relation between metabolic disturbances and stenosing vascular processes, the presence of a locally predisposing factor, such as mechanical strain or the state of enzymatic supply of the osseous tissue.

[Long-term follow-ups in adult idiopathic necrosis of head of femur (author's transl)].

28 patients were subjected to follow-up examination after having suffered for more than 10 years from idiopathic necrosis of head of femur. The high incidence of the bilateral affection of the hip joint as well as the occurrence of multiple osteonecroses are correlated with distrubances of lipid metabolism. Under conservative treatment the disease takes a progressive course, which, however, can remain subclinical for a long time. Good postoperative results can only be expected from intertrochanteric corrective osteotomy which often leads to a regeneration of the head of femur. Caution is recommended before confirming the indication for athrodesis or total endoprothesis because of the risk of disturbances of consolidation or loosening of the implant.
