[Prognostic factors in bronchopulmonary neoplasms].

The paper aims to illustrate the current state of knowledge regarding the prognostic factors of lung cancer. The disappointing results obtained using therapeutic modes (an overall 5-year survival rate of 15%), together with the considerable variability of the natural history of lung cancer, should be attributed to a group of prognostic factors, of varying importance, which can be subdivided into three categories: those concerning the characteristics of cancer, those inherent to the patient's conditions and those related to the type of operation. The paper underlines that histotype, TNM staging, the assessment of the nuclear degree and ploidy class play a correct and real prognostic role, and that only surgical treatment, if indicated, can offer a real life expectancy (40% 5-year survival rate in operated patients).

Evaluation of drug consumption and quality of pharmacological therapy in the Friuli Venezia Giulia Region (Italy).

A program for automatic reading of medical prescriptions has been introduced in Friuli Venezia Giulia (Italy), to control the quality of medical care and the physicians' prescribing methods in office-based medical practice. Data on the prescribing practice of general practitioners, drug consumption and costs are collected, and then general practitioners are confidentially informed, through patient-specific medication profiles, on the number and costs of their prescriptions. The first data often showed inappropriate prescription practice on the part of doctors. Through this program an approach based on quality assurance principles is made possible to improve prescribing practice. Being "peer review" activities, bereft of fiscal or punitive ends, they are more readily acceptable by physicians who feel directly involved in improving the use of drugs, with positive connotations for containing costs.

[Current aspects and controversies in reconstructive surgery after esophagectomy: personal experience].

The authors report their series of 184 esophagectomies performed both for benign (44) and malignant (140) diseases. On the basis of their experience and supported by Literature data, it is believed restoration of digestive continuity offers two main points of discussion: choice of the intestinal tract for the interposition and type of anastomosis to perform. The nature of the lesion and its level seems to play the most important role in the choice.

[Reconstruction of the thoracic wall using a Marlex sandwich. Presentation of a clinical case].

The case of a 78-year-old male with a large, firm, not tender mass of the left anterolateral region of the chest is reported. Previously he had been treated with cyproterone acetate for a year, being affected by a locally unresectable prostate carcinoma. At CT-scan of the chest mass appeared solid, extending to the pleural cavity with compression of the lung and erosion of third to fifth rib. Operation consisted in complete removal of the mass with "en bloc" resection of involved ribs. Chest wall was reconstructed by means of a Marlex mesh sandwich. Histology revealed a carcinoma, probably originating from the breast. The role of antiandrogenic chemotherapy in the development of the tumor and the use of plastic mesh for chest wall defects are discussed.

Prophylactic and therapeutic role of rioprostil in NSAIDs induced gastroduodenal lesions.

In a randomized, double-blind, placebo-controlled, prospective trial, the activity of rioprostil, a new prostaglandin E1 alcohol analogue, on non-steroidal anti-inflammatory drug-induced gastroduodenal mucosal lesions is studied in 30 patients with rheumatic diseases. Both treatment regimens, rioprostil, 150 micrograms t.i.d., and rioprostil, 200 micrograms b.i.d., for a period of 12 weeks, significantly reduce the rate of mucosal lesions in all patients when compared to placebo. Furthermore, rioprostil achieves a reduction in gastrointestinal symptoms without enhancement of joint disease activity, and significantly reduces antacid intake. Therapy is safe and well tolerated by all patients. It can be concluded from this study that prostaglandins could play an important part in the prevention and treatment of gastrointestinal damage due to NSAIDs.

[Chronic idiopathic neutropenia: a benign hematological disorder].

Clinical data in 23 consecutive patients with chronic idiopathic neutropenia are reported. During a long-term follow-up (4 to 11 years), none had leukemia or autoimmune diseases. In particular, in no case did serious recurrent infections develop despite severe neutropenia. Immunological studies showed the presence of antibodies to neutrophils in a unique case and of the marker make-up of K lymphocytes in other two with chronic T8 lymphocytosis and associated neutropenia. The immunological features of this syndrome is briefly discussed. Chronic idiopathic neutropenia, even in the presence of an immunological imbalance, is a benign haematological disorder and does not need any treatment.

[Chloroquine-resistant Plasmodium falciparum malaria imported into Italy].

A case of chloroquine-resistant falciparum malaria imported into Italy from Angola is reported. It represents a further demonstration that R III resistance to 4-aminoquinolines has spread to countries of the Central Africa Atlantic Coast with important implications for the treatment of patients and individual chemoprophylaxis.
