[Significance of different modes of induction and different dosages of droperidol on the stability of blood circulation in neuroleptanalgesia (author's transl)].

The advantages and disadvantages of a higher dosage of droperidol were examined in 581 patients. It was found that an initial low dosage induces a later increase of blood pressure. Furthermore it became evident to what extent different modes of induction are apt to strengthen or to weaken the effect of droperidol. It could be observed that induction with thiopental (3 mg/kg) leads to a maximum lowering effect of droperidol medication on blood pressure. Moreover, the apparently better results with ketamine are critically discussed.

[Changes of intraocular pressure in healthy-eyed patients under neuroleptanalgesia in abdominal surgery (author's transl)].

Ocular pressure was measured in 60 patients under neuroleptanalgesia with three different modes of induction (N2O, ketamine, thiopentone), during abdominal operations lasting at least one hour. The effect of premedication, of three different induction agents and of decurarisation on intraocular pressure were tested. It became apparent that after premedication and during the induction stage a decline of intraocular pressure (IOP) occured but did not last longer than 60 minutes. Decurarisation had no influence on the IOP. The causes of these changes and discussed.

[Minor eye operations on infants under local anesthesia].

A procedure with local anesthesia is possible after premedication with Thalamonal and atropine in some special cases. The child is sufficiently sedated and remains co-operative. Four cases are reported in order to illustrate how such a procedure leads to satisfactory results.

[Changes in the internal cuff pressure in intubated patients (author's transl)].

In 50 intubated patients the changes of pressure in the interior of the cuff were recorded during various methods of narcosis for 30 min. The results were evaluated statistically and interpreted with respect to the difference in the gas mixture and time. A linear increase of inner pressure in the cuff was established. Further modified experimental series are proposed.

[The influence of enflurane on intraocular pressure (author's transl)].

Various forms of inhalation anaesthesia with enflurane were carried out in 30 patients. An attempt was made to eliminate all the factors influencing the intraocular pressure (IOP), and to record the behaviour of blood pressure, pulse rate and IOP with the gradual increase of concentration of enflurane. Results are discussed and the use of the inhalation anaesthetic enflurane is recommended for intra-ocular operations.

[On ketamine-monoanaesthesia for operations for strabismus in childhood (author's transl)].

In 58 children below the age of 12 with strabismus operations were performed under anesthesia with Ketamine. Continuous and discontinuous modes of application were selected with varying premedication. Blood-pressure and pulse-rate, the occurence of oculo cardial reflex (OCR) and narcotic requirement were registered and discussed with regard to premedication and specific modes of application.

[Experience with the use of cystometry and simultaneous cystometry and urethrometry to clarify urinary incontinence in women (author's transl)].

Cystometry and simultaneous cystometry and urethrometry were conducted in 81 incontinent female patients (46 Grade I, 35 Grade II, according to the Ingelman-Sundberg Scale). The tonometric criteria for stress incontinence were presented and discussed by statistically comparing the pressure parameters for continent and incontinent patients. The results of both examinations indicated that, in almost all cases, the cause of incontinence (bladder, bladder obstruction or combination of both) can be determined with certainty. This method is well suited as a routine procedure to clarify urinary incontinence in women.

Single-dose cefodizime as infection prophylaxis in abdominal surgery: a prospective multicenter study.

This prospective pilot study was performed to evaluate the efficacy, safety and tolerability of a single dose of cefodizime as infection prophylaxis in patients undergoing major abdominal surgery. One hundred forty-nine patients received 2g cefodizime, a third-generation cephalosporin, 30 min before abdominal surgery. The mean operation time was 116 +/- 66 min. Eighteen patients received metronidazole additionally. In 8/149 patients (5.4%) the final outcome was considered to be a treatment failure. 94.6% of the patients had no signs of infection. The long half-life of cefodizime allows a single-dose perioperative prophylaxis even in abdominal surgery lasting 3 to 4 h. Cefodizime can be regarded as a safe antibiotic prophylaxis with few side effects.

Disseminated intravascular coagulation (DIC) and rhabdomyolysis in fulminant varicella infection--case report and review of the literature.

Primary infection with varicella-zoster virus usually is a mild, self-limiting childhood illness. However, certain rare but potentially life-threatening complications can be associated with the disease. Adults and immunosuppressed patients are at increased risk for these events. We report a case of a patient on chronic immunosuppressants with fulminant varicella infection complicated by rhabdomyolysis and disseminated intravascular coagulation. Mechanisms of muscle damage in viral diseases might be direct invasion of skeletal muscle and/or induction of harmful cytokines. Aggressive fluid therapy, alkalinization of urine and supportive measures correcting electrolyte imbalances, hypothermia and hypoxemia should result in preservation or complete restoration of renal function. Disseminated intravascular coagulation occurs in conjunction with various diseases and may range from mild laboratory abnormalities to fulminant lethal thrombosis and bleeding. Apart from elimination of the causative process therapeutic strategies are still highly disputed.
