[Dry eye syndrome in patients with biliary cirrhosis].

Ophthalmic state of 18 women patients between 40 and 69 years of age with primary biliary cirrhosis (PBC) was studied. Tear secretion was studied by Schirmer I., break up time and conjunctival and corneal bengal rose staining tests. In 15 cases serious degree of keratoconjunctivitis sicca has been revealed. Xanthelasma of the lid was found in three patients. Visual functions were intact. Kayser-Fleischer ring was absent in every case. There was no connection between the duration or laboratory characteristics of the PBC and the severity of the keratoconjunctivitis sicca. In nine patients of the cases requiring tear substitution eye drops of methylcellulosum available in Hungary were insufficient. Hydroxypropyl-methylcellulosum drops of eye gel containing peptides of low molecular weight eliminated the symptoms in these cases. The authors emphasize that PBC should be considered as a possible underlying disease in women over 30 years with severe dry eye syndrome.

[Determination of hepatitis C antibodies, using the Abbott and the Ortho anti-HCV EIA kits, in chronic liver diseases and patients under hemodialysis for chronic renal failure].

The diagnostic usefulness of anti HCV EIA test of Abbott and Ortho companies were compared. The anti HCV levels determined in the sera of 173 patients with chronic liver diseases and of 17 haemodialysed kidney patients. 109 of 190 (57%) sera were found to be negative and 81 (43%) positive determined by Abbott kit, while 127 (67%) were negative and 63 (30%) were positive by the Ortho kit. Positive results in patients with chronic liver disease were found in 66 patients by Abbott and 58 patients with Ortho Kit. Distribution of positive cases according to the diagnosis is as follows: 29 (57%) out of 51 chronic hepatitis, 17 (61%) out of 28 cirrhosis. Conflicting results were obtained in 20 cases of 190 (11%) when 17 sera were positive by Abbott and negative by ORTHO, and 3 sera were negative by Abbott, and positive by Ortho. The samples close to the cut off and with low positivity with conflicting results were checked again by the neutralization HCV EIA Abbott assay. We found the Abbott HCV EIA test more sensitive in our excellent for screening of large numbers of samples, we recommend to confirm the positive results by a neutralization type test.

[Decreased serum osteocalcin level in non-alcoholic and alcoholic chronic liver diseases].

Serum level of osteocalcin (OC) is believed to be a specific biochemical parameter of bone formation. Decreased serum OC has been reported in alcohol-intoxicated subjects, in patients with primary biliary cirrhosis and in patients with chronic alcoholic liver disease. The question was, whether lower OC level could be detected in patients with nonalcoholic and non-cholestatic chronic liver disease. The serum OC was measured by RIA developed in our laboratory. Results were compared to age and sex matched controls. Decreased OC level was found in 35 out of 47 (74%) patients with non-alcoholic and non-cholestatic liver disease as chronic persistent hepatitis, chronic active hepatitis, fatty liver and cirrhosis, in 21 out of 26 (80%) patients with alcoholic liver disease and in 8 out of 15 (53%) primary biliary cirrhosis. None of the patients had elevated value. There was no correlation between the decreased OC level and the duration or severity of the liver disease and the laboratory parameters as bilirubin, AST, ALT, alkaline phosphatase, albumin, prothrombin, and serum 25-OH-D3 vitamin level. Decreased OC was found also in the patients without cirrhosis. The possible causes are discussed. Relying upon these findings it is supposed that chronic liver disease by itself can influence the osteoblast activity also by some unknown mechanism.

Early complications of gastric resection.

Analyzing the patient material of 506 patients with gastric resection, authors deal with the early complication of resection, its diagnosis and treatment. They review the complications, their frequency and mortality in their own material. After operations made for tumour, complications occurred more frequently than after interventions because of ulcer. Total mortality was 10.2%. In order to reduce mortality they call attention to the possible prevention of complications and their careful management.

[Detection of antimitochondrial antibodies by immunodiffusion, ELISA and immunofluorescence methods].

Ouchterlony immunodiffusion and ELISA were used for the detection of antimitochondrial antibodies. The results were compared with those of immunofluorescence measurements. The occurrence of antimitochondrial antibodies was investigated in 20 patients with primary biliary cirrhosis, in 30 with autoimmune disease and in 28 with porphyria cutanea tarda. Sonicated rat liver mitochondria fractions were used as antigens. Among the patients with primary biliary cirrhosis, Ouchterlony immunodiffusion revealed antimitochondrial antibodies in 18 cases, whereas antibodies were not found in the other groups of patients, except for 1 case of systemic lupus erythematodes. ELISA demonstrated that in all 20 patients with primary biliary cirrhosis where immunofluorescence revealed antimitochondrial antibodies, there was an increase in the quantity of antibodies against mitochondrial inner membrane fractions, and in 7 cases even against the outer membrane fractions. These 7 were the most serious cases clinically. These methods allow the differentiation of antimitochondrial antibody subtypes and promote the establishment of the diagnosis.

[Autonomic neuropathy in chronic liver diseases].

Autonomic neuropathy has been evaluated by various cardiovascular bedside tests in 99 patients with chronic alcoholism (33 alcoholics without liver disease, 33 patients with fatty liver and 33 with cirrhosis), in 10 patients with primary biliary cirrhosis, in 12 patients with cirrhosis of other origin, and in 40 healthy controls. Parasympathetic integrity was evaluated by beat-to-beat variation during deep breathing (6 min), Valsalva manouver and standing up, sympathetic function by blood pressure response to standing up and to sustained handgrip test. Autonomic reflex damage was found in all groups examined. Patients with alcoholic cirrhosis exhibited the most severe alterations. Our results suggest, that chronic hepatopathy itself presents a pathogenetic factor of autonomic neuropathy. Autonomic failure has to be considered as a possible cause of symptoms in liver diseases with all its prognostic consequences.

Effect of intravenous calcium load on the serum calcium level in postmenopausal osteoporosis (a study of the pathogenesis, and diagnostic use of the test).

Analysis of calcium tolerance in suggested to represent a valuable diagnostic aid in osteoporosis, particulary in the menopause. The serum calcium level was found to exceed 11.0 mg/dl 60 min after the intravenous injection of 3.6 mg per kg body weight of Ca++ in all patients with osteoporosis, while the level was normal at that point of time in every subject without osteoporosis, including patients with bone disease other than osteoporosis. Administration of norandrosterone decanoate or dehydroepinandrosterone to patients with menopausal osteoporosis resulted in normalization of the post-load hypercalcaemia. Calcium tolerance of menopausal patients without osteoporosis was not affected by dehydroepiandrosterone.
