Adenosine A1 antagonists. 3. Structure-activity relationships on amelioration against scopolamine- or N6-((R)-phenylisopropyl)adenosine-induced cognitive disturbance.

The effects of a variety of adenosine A1 and A2 antagonists on N6-((R)-phenylisopropyl)adenosine (R-PIA)- and scopolamine-induced amnesias were investigated in rodents in order to clarify the role of adenosine receptors in learning and memory. Some of the selective adenosine A1 antagonists exhibited antiamnesic activities at several doses where they did not induce an increase of spontaneous locomotion. These results suggest that the blockade of A1 receptors is more important than that of A2 receptors in learning and memory. Detailed studies of structure-activity relationships of adenosine A1 antagonists in two amnesia models demonstrated that there were three types of adenosine A1 antagonists: (A) Compounds 3-5 (8-substituted 1,3-dipropylxanthines) ameliorated the shortened latency in both models. (B) Compounds 7-11 (8-substituted 1,3-dialkylxanthines) and 19-21 (imidazo[2,1-i]purin-5(4H)-one derivatives) ameliorated the shortened latency in the (R)-PIA-induced amnesia model but not in the scopolamine-induced amnesia model. (C) Compounds 14-16 ameliorated the shortened latency in the scopolamine model but not in the (R)-PIA model. Aminophenethyl-substituted compounds C did not exhibit adenosine A1 antagonism in vivo presumably due to rapid metabolism. The dramatic change in the activities of A and B could not be explained by their simple pharmacokinetic differences because both types of compounds showed clear blockade of central adenosine A1 receptors in the (R)-PIA model. 8-(3-Dicyclopropylmethyl)-1,3-dipropylxanthine (5) (KF15372) was chosen for further studies and is currently under preclinical development as a cognition enhancer.

5-HT3 receptor antagonists. 2. 4-Hydroxy-3-quinolinecarboxylic acid derivatives.

A series of 4-hydroxy-3-quinolinecarboxylic acid derivatives (6) and 4-hydroxy-2-oxo-1,2-dihydro-3-quinolinecarboxylic acid derivatives (7) were designed and synthesized as 5-HT3 receptor antagonists. Molecular modeling studies suggested that the 3-carbonyl moiety in 6 was almost coplanar to the plane of an aromatic ring, but in 7 there was a 30 degrees deviation. 4-Hydroxy substitution in quinoline derivatives enhanced affinity for the 5-HT3 receptors, and endo-N-(8-methyl-8-azabicyclo[3.2.1]oct-3-yl)-4-hydroxy-3- quinolinecarboxamide (6f) exhibited the most potent activity in the Bezold-Jarisch (B-J) reflex test (ED50 = 0.1 micrograms/kg, iv) among quinoline derivatives 6. Although 4-hydroxy-2-oxo-1,2-dihydro-3-quinolinecarboxamide derivatives (7a) exhibited higher affinity (e.g., 7d: Ki = 0.48 nM) for the 5-HT3 receptors than ondansetron (Ki = 7.6 nM) or granisetron (Ki = 2.1 nM), these amides showed less potent activity in the B-J reflex test than the reference compounds. Interestingly, the ester derivatives 7c, 7f, and 7h eliminated affinity for the 5-HT3 receptors. These unusual structure-activity relationships and the deviation of the 3-carbonyl moiety from the plane of an aromatic ring suggest that the active conformation of 7a might be different from the proposed one for the preceding 5-HT3 antagonists. Thus, 6f was chosen for further studies. No receptor binding for a variety of ligands was significantly antagonized by 6f. Comparing the ratios of the ED50 value in the B-J reflex test (rat, iv) with the LD50 value in acute lethal toxicity (mouse, iv), 6f was proved to have a 600-fold wider margin of safety than ondansetron. Compound 6f dose-dependently attenuated both the incidence and frequency of emetic episodes induced by cisplatin in the dog (ED50 = 14 micrograms/kg, iv) more potently than ondansetron (ED50 = 210 micrograms/kg, iv). Compound 6f (KF-20170) is now under further investigation as a drug for treating gastrointestinal disorder.

Unique features of the cauda epididymidal epithelium of hibernating bats may promote sperm longevity.

Measurements of extremely high osmolalities in cauda epididymidal fluids of hibernating bat species led to an investigation of the junctional complex morphology of the epithelium of this sperm storage site. Freeze fracture replicas revealed the presence, at certain times of the year, of a tight junction architecture that resembled that traditionally thought to be exclusive to the blood-testis barrier, the strongest permeability barrier in the body. It is hypothesized that seasonal establishment of these specialized Sertoli cell-like tight junctions is necessary to the maintenance of the high osmotic state of the luminal environment, allowing for the prevention of dilution of its contents by paracellular routes and its protection from bursting under the osmotic pressure contained within.

Demonstrations of a B-cell population that regulates the immune response in spleens of mice infected with herpes simplex virus type I.

A population of B cells that regulates the immune response was demonstrated in splenic mononuclear cells (SMNC) from mice infected with herpes simplex virus type 1 (HSV). SMNC, obtained from mice 3 days after HSV infection at a dose of 100 LD50, exhibited a reduction in the proliferative response of naive SMNC stimulated with various lectins or allogeneic lymphocytes in a 5-day mixed lymphocyte reaction. Cocultivation of naive SMNC with phagocytic cell-free SMNC (M phi-MNC) from infected mice resulted in the inhibition of lymphocytic blast transformation stimulated with various lectins. These cells were characterized as nylon-wool adherent cells that were eliminated by treatment with anti-Ig antiserum, but not anti-Thy 1.2 monoclonal antibody or anti-asialo GM1 antiserum, followed by complement treatment. In addition, the suppressor cell activity was not demonstrated in M phi-MNC obtained from HSV-infected CBA/CaHN-xid/J mice, which contain a congenital B-cell deficiency. These results suggest that, in addition to suppressor T cells, a population of B cells, which can inhibit lymphocyte proliferations upon stimulation with lectins and alloantigens, might be generated in spleens of mice following HSV infections.

Solitary intracranial chondroma of the convexity dura: case report.

We present a rare case of chondroma originated from the dura mater of the cerebral convexity in a 16-year-old girl. Radiologic findings are reported with emphasis on computed tomography and magnetic resonance imaging scans, and histogenesis is briefly discussed.

Immunomodulating activities of orally administered SMANCS, a polymer-conjugated derivative of the proteinaceous antibiotic neocarzinostatin, in an oily formulation.

Immunomodulatory effects of oily formulated SMANCS, a polymer conjugated derivative of the proteinaceous antibiotic neocarzinostatin, after oral administrations to mice was investigated. The oral administrations of SMANCS, dissolved in medium-chain triglyceride containing phosphatidylcholine and polyglycerine dioleate (oily SMANCS), resulted in: (1) augmentation of NK cell activity in naive mice, (2) activation of macrophage cytostasis in naive mice, (3) enhancement of the generation of cytotoxic T-lymphocytes in mice immunized with allogeneic lymphocytes, (4) production of circulating interferon in naive mice, and (5) increase of delayed-type hypersensitivity response in mice immunized with sheep red blood cells. The degree of immunomodulation orally stimulated with oily SMANCS was similar to that of the immunomodulation induced by i.v. or i.p. administrations of aqueous formulated SMANCS (aqueous SMANCS). Although SMANCS is a protein drug that may be digestable by various enzymes present in the stomach, in the present study immunomodulating activities of SMANCS were clearly demonstrated when an oily formulation of the compound was administered to mice orally. Since aqueous SMANCS administered parenterally and oily SMANCS administered orally exhibit the same immunomodulatory activities and the former has demonstrated antitumor activity in man and animals, the latter may possess this same antitumor activity.

Syrian hamster embryo cell lines useful for detecting transforming genes in mouse tumours: detection of transforming genes in X-ray-related mouse tumours.

The Syrian hamster embryo cell lines, SHOK and MC-1, were used as recipient cells for DNA transfection assay to detect transforming genes in experimental mouse tumours. A mouse repeat sequence was utilised to check whether each transformed focus included mouse genomic DNA in the Hamster background. We investigated five mouse tumours that are related to X-ray radiation, and detected activated c-K-ras, c-mos, and c-cot oncogenes which induced foci of hamster cells. These results show that SHOK and MC-1 cells have unique properties for detecting transforming genes in experimental mouse tumours.

Inhibition of burn-associated suppressor cell generation by glycyrrhizin through the induction of contrasuppressor T cells.

The inhibitory effect of glycyrrhizin (GR), an anti-inflammatory Chinese herbal drug extracted from licorice roots, on the generation of suppressor T cells in thermally injured mice (TI-mice) was investigated. The burn-associated suppressor T cell (BTs cell) activity was demonstrated in splenic mononuclear cells (SMNC) from mice 2 to 8 days after thermal injury when suppressor cell activity was assayed in a one-way mixed lymphocyte reaction. However, when TI-mice were treated with GR, SMNC harvested 6 days after thermal injury showed no suppressor cell activity. This activity of GR demonstrated a dose-response effect, with a dose of 10 mg/kg exhibiting peak levels of the activity. Since GR had no direct inactivating activities against BTs cells in vitro, the inhibitory effect of SMNC, derived from TI-mice treated with GR, on the activity of BTs cells was examined in the same mixed lymphocyte reaction system, and the results showed that the SMNC from GR-treated mice 6 days after thermal injury counteracted the activity of BTs cells. The type of cell responsible for this inhibition of BTs cell activities was a CD3+, L3T4+, Vicia villosa lectin-adherent T cell with the same phenotypic properties previously exhibited by contrasuppressor cells. These results suggest that GR may regulate the generation of BTs cells through the induction of contrasuppressor cells. Since there are many reports describing septic infections due to the appearance of BTs cells in postburn patients, it may be possible to apply GR or blood preparations containing contrasuppressor cell populations induced by GR in healthy volunteers into immunosuppressed burn patients to avoid infections.

Adenosine A2a receptor modulation of electrically evoked endogenous GABA release from slices of rat globus pallidus.

Adenosine A2a receptors have been localized to GABAergic striatopallidal neurons, but their functional role is unknown. To address this question, the modulation of endogenous GABA release by adenosine A2a receptors was examined in slices of rat globus pallidus. The selective adenosine A2a receptor agonist CGS-21680 (3.0-10 nM) significantly increased electrically stimulated release (overflow) of GABA, with 10 nM CGS-21680 resulting in a 44% increase compared with the control. Both the nonselective adenosine receptor antagonist 8-phenyltheophylline (10 microM) and the selective A2a receptor antagonist KF-17837 (100 nM) abolished the CGS-21680-induced increase in GABA overflow. Higher concentrations of CGS-21680 (0.10-1.0 microM) decreased GABA overflow by approximately 25%. 8-Phenyltheophylline (10 microM) antagonized these effects, whereas KF-17837 (100 nM) did not, suggesting actions of CGS-21680 on other adenosine receptors at these concentrations. These results demonstrate that activation of adenosine A2a receptors augments electrically stimulated release of GABA from globus pallidus slices and suggest a mechanism by which adenosine may modulate GABAergic output from the striatopallidal efferent system.

Nonuniformity of AH intervals during stimulation at different left atrial sites.

Studies in humans have found left atrial stimulation via the coronary sinus (CS) to elicit significantly shorter atrium-His (AH) intervals as compared to right atrial stimulation, but whether pacing at different left atrial sites (anterior vs posterior left atrium, i.e., far distal vs proximal CS) affects the AH interval has not been studied. Hence, in 22 patients, we compared the effects of stimulation from various atrial sites, including anterior high right atrium (HRA), distal CS, mid-CS, and proximal CS, on: stimulus-atrium (SA), AH, and stimulus-His intervals on the His bundle electrogram. Paced cycle length differed for each patient (range 900-350 msec, mean 532 +/- 140 msec), but conduction intervals from different atrial sites were compared using identical cycle length in each patient. The mean SA intervals were 34 +/- 10 msec, 57 +/- 10 msec, 44 +/- 11 msec, and 32 +/- 8 msec with stimulation, respectively, from HRA, distal CS, mid-CS, and proximal CS (each significantly different except for HRA vs proximal CS). The mean AH intervals were 123 +/- 23 msec, 104 +/- 28 msec, 95 +/- 15 msec, and 90 +/- 18 msec with stimulation, respectively, from HRA, distal CS, mid-CS, and proximal CS (each significantly different except for mid-CS vs proximal CS). In 13 patients, the discrepancy in AH intervals during distal versus proximal CS stimulation was > or = 15 msec; in 9 patients this difference was only < or = 10 msec, considered within the range of measurement error. Thus, in a significant portion of patients, discrepant AH intervals were demonstrated during stimulation from the distal versus proximal CS. These previously undescribed observations suggest that electrophysiological studies on atrioventricular nodal conduction that involve left atrial stimulation must take into account actual location of the stimulation site (anterior or posterior) in order to properly interpret the findings.

"Paradoxical" AH shortening caused by proximal coronary sinus stimulation during orthodromic reciprocating tachycardia.

INTRODUCTION: During extrastimulation or entrainment of orthodromic atrioventricular (AV) reciprocating tachycardia (ORT), the atrium-His (AH) interval as measured at the His-bundle recording site is expected to lengthen due to extrastimulation-dependent or pacing rate-dependent slowing of AV nodal conduction by impulses that penetrate the tachycardia circuit. We report 6 patients in whom the AH interval "paradoxically" shortened during ORT in response to extrastimulation and rapid pacing from the proximal coronary sinus. METHODS AND RESULTS: Accessory pathway location was right anterior (1 patient), right anteroseptal (1 patient), and left anterior (4 patients). Cycle length of ORT was stable (variation < or = 5 msec) and ranged from 325 to 410 msec. During ORT, extrastimulation and rapid pacing were performed from the proximal coronary sinus and the right atrium. Extrastimulation from the proximal coronary sinus late in diastole caused significant shortening of AH interval in all patients by a mean of 18 +/- 3 msec (range 15 to 20 msec). AH shortening was demonstrated without a change of either the timing or morphologic appearance of the low septal right atrium at the His-bundle recording site. This phenomenon was not observed during right atrial extrastimulation. Rapid pacing from the proximal coronary sinus at cycle lengths of 305 to 390 msec (i.e., 15 to 20 msec shorter than the cycle length of each ORT) again demonstrated shortening of AH interval in all patients by a mean of 15 +/- 3 msec (range 10 to 20 msec). By contrast, rapid pacing from the right atrium demonstrated classical AH prolongation at any paced cycle length. CONCLUSION: AH shortening without a change of either the timing or morphologic appearance of the low septal right atrium at the His-bundle recording site confirms the existence of a distinct posterior atrial input to the AV node. In this setting low septal right atrial activation is not requisite for AV nodal conduction. Whether activation of the low septal right atrium is essential for, or contributes to, AV nodal conduction of atrial impulses from locations other than the proximal coronary sinus needs to be determined.

Incidence and clinical features of renal tubular acidosis-1 in urolithiasis.

To elucidate the frequency and clinical picture of renal tubular acidosis-1 (RTA-1) in nephrolithiasis, the acid-loading test was performed in 474 patients with calcium-containing stones. RTA-1 was detected in a total of 11 patients (6 men and 5 women, 2.3%). The incidence of RTA-1 in female patients tended to be higher than in male patients (3.2 vs. 1.9%). One male patient had the complete form, the others had incomplete form. There was a tendency that RTA-1 patients were younger than non-RTA-1 patients in men, and the former were older than the latter in women. The percentages of positive family history and positive past history were 27 and 45%, respectively. Stones were single in 7 cases and multiple in 4 cases. They were unilateral in 10 cases, and bilateral in 1 case. Hypercalciuria was detected in 2 of the 11 cases, hyperuricosuria was present in none of the 11 cases, and hyperoxaluria in 2 of 8 cases examined. Stones were composed of calcium oxalate in 2 cases, calcium phosphate in 2, and calcium oxalate mixed with calcium phosphate in 3.

Effects of interleukin-1 on syntheses of alkaline phosphatase, type X collagen, and 1,25-dihydroxyvitamin D3 receptor, and matrix calcification in rabbit chondrocyte cultures.

The effect of IL-1 on expression of the mineralization-related phenotype by chondrocytes was examined. In cultures of rabbit growth plate chondrocytes, IL-1 beta at 0.1 ng/ml caused 95% decreases in alkaline phosphatase activity, alkaline phosphatase mRNA levels, the incorporation of 45Ca into insoluble material, and the calcium content during the hypertrophic stage. These effects of IL-1 beta were dose-dependent and were observed in 24-48 h. Furthermore, IL-1 beta suppressed increase in cell size and the syntheses of 1,25-dihydroxyvitamin D3 receptor and type X collagen, other markers of hypertrophy, but had little effect on the synthesis of total protein including type II collagen. The inhibition of calcification was observed only when chondrocytes were exposed to IL-1 before the onset of calcification: IL-1 treatment from the mineralization stage had a marginal effect on 45Ca incorporation into insoluble material. These results suggest that IL-1 inhibits chondrocyte hypertrophy and the onset of calcification in ossifying cartilage.

1,4:3,6-Dianhydrohexitol nitrate derivatives. I. Synthesis and antianginal activity of alkylpiperazine derivatives.

A series of 5-deoxy-5-(4-substituted piperazin-1-yl)-1,4: 3,6-dianhydro-L-iditol 2-nitrates was prepared and evaluated for oral anti-ischemic activities. Inhibition of lysine-vasopressin-induced T-wave elevation in the electrocardiogram (ECG) of rats (angina pectoris model) served as a primary assay. Optimum activity was observed for the compounds with the aryl-heteroatom (O,S, or N)-propyl group. Among them, the phenylthiopropyl-substituted compound 13 exhibited the most potent activity. Furthermore, intraduodenal administration (i.d.) of 13 tended to decrease left ventricular end-diastolic pressure (LVEDP) in a propranolol-induced heart failure model (dogs) and showed a potent protective effect against reperfusion arrhythmia in rats. Thus, 13 (KF 14124) is under further study as an orally active nitrate.

1,4:3,6-Dianhydrohexitol nitrate derivatives. II. Synthesis and antianginal activity of aryl- or arylcarbonylpiperazine derivatives.

A series of 5-(4-aryl- or 4-arylcarbonylpiperazin-1-yl)-5-deoxy-1,4: 3,6-dianhydro-L-iditol 2-nitrates was prepared in order to obtain orally active, nitrate-type vasodilators with reduced side effects. Our drug design was based on a small reduction in the lipophilicity compared to that of 5-deoxy-5-[4-(3-phenylthiopropyl)piperazin-1-yl]-1,4: 3,6-dianhydro-L-iditol 2-nitrate (1, KF14124). Compounds 4h (aryl = benzimidazol-2-yl), 4i (arylcarbonyl = nicotinoyl), and 4w (arylcarbonyl = 3-furoyl) showed potent anti-ischemic activity in a lysine-vasopressin-induced angina pectoris model (rats), and their structure-activity relationships are discussed. Compound 4i exhibited potent vasodilation of the coronary artery in anesthetized dogs and also exhibited potent preload reduction in a heart failure model (dogs) as compared with isosorbide dinitrate (2), nicorandil (3), and KF14124 (1). Furthermore, 4i showed much weaker acute lethal toxicity and less central nervous system depression than 1 in mice. Thus, 4i (KW-3196) is under development as a vasodilator and a drug for treating angina pectoris.

Phorbol myristate acetate inhibits the bradykinin-induced L-nitro-arginine insensitive endothelium-dependent relaxation of bovine coronary artery.

The effects of L-nitro-arginine (LNAG) and phorbol myristate acetate (PMA) were studied in bradykinin-induced relaxations in bovine coronary arteries. In the presence of indomethacin (10 microM), neither LNAG (100 microM) nor PMA (0.1 microM) inhibited the bradykinin-induced relaxations in coronary arterial rings contracted with prostaglandin F2 alpha. However, simultaneous application of LNAG and PMA almost completely abolished the bradykinin-induced relaxation. In a sandwich-method, endothelium-intact coronary arteries (donor vessels) were treated with LNAG or with PMA for 30 min and then placed in close apposition to a denuded ring (assay vessel). Pretreatment of the donor vessels with LNAG, but not with PMA, almost completely abolished the bradykinin-induced relaxations in the assay vessel. In contrast, treatment of the assay vessel with PMA or with LNAG had no effect. These results suggest that bradykinin-induced endothelium-dependent relaxation of bovine coronary artery depends on both the release of nitric oxide and other endothelium-derived relaxing factor(s), which is an extremely labile substance(s), or a non-diffusible factor(s). PMA seems to inhibit the production and/or the release of the latter substance(s).

[Determination of oxalic acid by high performance liquid chromatography].

In order to determine a reliable method for measuring oxalic acid, we carried out a procedure using high performance liquid chromatography. This method proved to be excellent in the case of tea and beer. In the case of urine, however, it had some problem from the aspect of reproducibility, although as far as linearity measurement, dilution test and recovery rate were concerned, accurate results could be obtained. Nevertheless, this method was thought to be unsuitable for routine clinical use, at least at present, for the reasons that it took a long time to perform the measurement, and because of the high cost of the column used in the procedure. The value of the oxalic acid as determined by this method in 24 hr urine from 12 healthy male volunteers was in the range of 17-47 mg/day (average +/- standard deviation: 25.4 +/- 9.0). To correct for their differences of physical constitution, dividing oxalic acid value by body weight rather than by urinary creatinine or body surface area was found to give a more precise value of the norm. From this calculation, it was thought that the normal range of oxalic acid value was 0.6 mg/kg or less.

[Non-Hodgkin's T cell lymphoma associated with marked eosinophilia].

A case of non-Hodgkin's T cell lymphoma (diffuse lymphoma, large cell type) associated with marked eosinophilia and pleurisy in a 57-year-old male is reported. The leukocyte count was 12.5 x 10(3)/microliters and eosinophil count was 53% and the absolute count of 6.6 x 10(3)/microliters. The patient's serum and pleural effusion fluid, containing abundant lymphoma cells, showed eosinophil colony stimulating factor (Eo-CSF) activity. Conditioned medium (CM) prepared from patient's T cells (T-CM) produced Eo-CSF and this was enhanced by interleukin-2 (IL-2) stimulation. We demonstrated that the patient's serum contained a significant amount of interleukin-5 (IL-5) and the patient's T-CM, particularly after IL-2 stimulation contained a significant amount of granulocyte-macrophage colony-stimulating factor (GM-CSF). These findings suggest that Eo-CSF produced by neoplastic T cells or normal T cells activated by tumor antigen stimulated the production of eosinophils in this patient and that both IL-5 and GM-CSF might play a role in Eo-CSF activity.
