[Sequential scintigraphic evaluation in renal transplantation (author's transl)].

In a series of 57 renal transplantation of a sequential scintigraphy was carried out within the tenth day from surgical operation. According to the various scintigraphic findings and on the basis of the possible acute renal failure, the patients were subdivided into 7 groups and the data were correlated with clinical and hystological findings.

Cytochalasin B and water transport. A scanning electron microscope study of the toad urinary bladder.

A morpho-functional study of the effects of cytochalasin B (CB) on Na and water transport was made in amphibian epithelia. The functional studies confirmed the dissociation of the natriferic and hydrosmotic effects of vasopressin in toad urinary bladders exposed to CB and showed in addition that the block of the hydrosmotic effect was reversible and could still be induced in epithelia maximally stimulated with the hormone. Scanning electron microscopy revealed that CB, per se, did not alter the apical surface of the bladders. An almost total loss of microvilli of granular cells was seen, however, if CB was associated with vasopressin and an osmotic gradient. The results suggest two points: a) the block of the hydrosmotic flow induced by CB is due to factors beyond the apical membrane; b) microfilaments may be important mechanochemical transducers in the chain of events leading to the hydrosmotic effect of vasopressin.

[Experimental studies on the role of suture materials in the healing of anastomoses of the colon. Mechanical and histological study].

The relation between suture material and consolidation and healing in operations on the colon was investigated by using silk and catgut in one group of disinfected stray dogs subjected to right or left hemicolectomy and resection of the transverse portion, and a synthetic reabsorbable material (polyglycolic acid) in another. The following parameters were assessed following removal of the segment containing the anastomosis after various periods of time: resistance to biomechanical tests, stenosis of the lumen, extent and duration of inflammation and regeneration at the edge. Reabsorbable material was associated with less overall inflammation and healing was more rapid. Consolidation of the suture was not significantly different in the two groups.

A method of continuous renal capillary microperfusions of the same area at different solute concentrations.

The microperfusion pump described in this paper allows to perfuse the same peritubular capillary area on the renal surface with different test solutions. Solutions can be changed by means of a microcannula inside the pump which for this purpose can be moved near to the tip of the pipette. Concentration effect relations of various agents may thus be obtained on the single nephron level in vivo. This is illustrated on the example of the inhibitory effect of acetazolamide on proximal tubular reabsorption. Inhibition of this tubular function has been assessed by means of repeated determinations of split droplet half times.

The hydrosmotic effect of vasopressin: a scanning electrom-microscope study.

Scanning electron-microscopy (SEM) was used to investigate the hydrosmotic effect of vasopressin on the apical surface of urinary bladders of toads Bufo marinus. Bladders were mounted on glass chambers and water fluxes were monitored with an optical method. Tissues were fixed in 2% glutaraldehyde and processed for SEM. Three types of cells were seen on the surface of control bladders:large polygonal (granular) cells, with blunt microvilli; smaller (mitochondria-rich) cells, with longer microvilli; goblet cells. Neither exposure of the bladders to a large osmotic gradient nor exposure to vasopressin in the absence of a gradient altered appreciably the epithelial surface. In contrast, the combination of vasopressin and an osmotic gradient resulted ina conspicuous diminution of the blunt microvilli. However, the small cells with longer microvilli remained unchanged. Identical results were seen with cAMP or theophylline in the presence of an osmotic gradient. These findings suggest that the hydrosmotic effect of vasopressin is mainly exerted on the granular cells of toad bladder and confirm observations made by others with the electron-microscope.

[Thyroid surgery in assisted local anesthesia].

To evaluate the advantages of thyroidectomy under assisted local anesthesia, 35 cases operated on from January 1998 to February 1999 were reviewed. The patients were studied in ambulatory setting and adequately informed on the program of thyroid operation under local anesthesia. Multinodular goitre was present in all the cases. In 12 cases, it was intrathoracic. The morning of operation, all the patients underwent to pre-operative sedation and, in operative room, to local anesthesia with Mepivacaine 1% and adrenaline 1,200,000 U. The operations were performed with a mean of 30 cc of local anesthetic. During operation, in none case the conversion to general anesthesia was necessary. No mortality and morbility were registered. In the majority of cases, an analgesic was necessary meanly 4 hours after operation. The evening of operation in all the patient oral nutrition was restored. Twenty-nine patients were discharged from the hospital 48 hours after surgery. Eight days after operation, surgical recovery was evident in all the patients reviewed in the out patients setting. The advantages of thyroid surgery under assisted local anesthesia are outlined.
