An analytical framework of physician productivity and variation in utilization.

The utilization of medical services by patients is an important determinant of doctor productivity, but this factor does not appear to have been given much attention in previous studies. In order to answer the question of why is there a wide variation in doctor output at low level medical facilities in China, an analytical framework of doctor productivity and utilization is developed. The simulation model is used to produce data that can be analyzed by such a framework. Great uncertainty about patient flows is one reason for the average lower and varying doctor productivity in lower level health facilities. Until uncertainty can be reduced, more flexibility is needed at the lower level to cope with changing utilization patterns and patient characteristics. The management by doctors of non-patient care activities (preventive programmes, medical research, teaching, and administration) is crucial to any approach to using doctor resources more effectively and efficiently.

[Revelations of the origin of Chinese nation from clustering analysis and frequency distribution of HLA polymorphism in major minority nationalities in mainland China].

This paper is a joint report on the analysis of polymorphism of HLA class I, II and III antigens in Chinese major minority nationalities (Miao, Buyi, Mongol of Inner Mongolia, Man, Hui, Tibetan and Uygur), and southern and northern Han samples as a part of the workshop organized by the 11th International Histocompatibility Workshop and Conference (IHWC) held on 6-13 November 1991 in Yokohama. Clustering analysis showed that Miao and Buyi cluster with the southern Han and Mongol, Man, Hui and Tibetan cluster around the northern Han, suggesting that the Chinese nation comprises northern and southern two major populations. Uygur clusters with a large group of Caucasoid samples from very far point. Frequencies of HLA alleles of Caucasoid origin (A3, B8 and others) decrease eastwards and southwards, and those of HLA alleles of Southeast Asia Mongoloid origin (B46 and others) decrease northwards. The gradient distribution of HLA allele frequencies suggests the unceasing migration and mutual amalgamation of our ancestors since the remote pre-historic times. From the fact that Chinese nation comprises northern and southern two major populations, this paper discussed the possible revelations of the origin of Chinese nation.

Antimicrobial prophylaxis in colorectal surgery.

This article summarises the findings of a systematic review of the literature concerning antimicrobial prophylaxis before colorectal surgery. From analysing 152 clinical trials certain conclusions can be drawn, such as the importance of ensuring that prophylactic therapy is active against both aerobic and anaerobic bacteria; and, single dose treatment can be as effective as a full course of antibiotics.

The role of electronic journals in reducing publication bias.

Publication bias poses a threat to the validity of medical research. It is unlikely that conventional paper journals can solve the problem of selective publication of studies with striking results because of their space limitation and need to be newsworthy. Peer-reviewed electronic journals without limitation of space might provide a solution to this paradox. To maintain the integrity of medical publication, editorial policy needs to be changed to accept clinical trials for publication, based only on the methodological criteria and not on the impact of their findings.
