Hypertension secondary to bradycardia: blood pressure regulation under the effect of impaired cerebral blood flow and bradycardia.

Ninety-two patients of advanced age were studied in whom arterial hypertension was associated with chronic bradycardia, the cessation of bradycardia after pacemaker implantation was followed by a significant fall or permanent normalization of the high blood pressure. In three cases cerebral blood flow was found low, but it increased when the heart rate was normalized on pacing. In order to check the results obtained in humans, cerebral blood supply was restricted by carotids ligation, in some animals the vertebral artery of one side was also ligated. During obstruction of cerebral blood flow, bradycardia induced by av block or vagal stimulation was followed by an increase in arterial blood pressure. It is suggested that disturbances of cerebral blood flow associated with bradycardia have a decisive part in the pathogenesis of hypertension in old age.

Changes in the pacemaker function of the sinus node after closure of atrial septal defect. Temporary and stable iatrogenic sick sinus syndrome.

The alterations in the clinical symptoms and the pacemaker function of the sinus node following open heart operation were studied in 26 patients suffering from atrial septal defect. The most characteristic electrophysiological change was the prolongation of the sinus node recovery time as consequence of surgical repair of the atrial septal defect. The clinical symptoms of the sick sinus syndrome could be detected in all 26 cases after the open heart surgery. In 16 patients the symptoms of iatrogenic sick sinus syndrome (SSS) were only transitory, while in 10 patients a stable iatrogenic SSS developed after the operation. In the basis of these data the pathological mechanism of the iatrogenic SSS is discussed.

Effect of the stellate ganglion blockade upon the electrical systole (Q-T interval) of the heart.

The effect of the Novocain blockade of the stellate ganglion upon the duration of the electrical systole (Q-T) of the heart was studied in 38 patients. Basic Q-T was normal in 17 patients; in these cases stellate ganglion blockade did not influence the Q-T interval. Of the 21 cases with Q-T prolongation, in 10 the Q-T interval became considerably shorter upon the effect of blockade. In the 11 cases that did not respond to the blockade, ECG repolarization was regular, and there was no arrhythmia. Of the 10 responsive cases with Q-T prolongation, in 9 the repolarization was pathological, and in 6 there was no chronic arrhythmia. The authors discuss the role of the sympathetic nervous system in certain forms of Q-T prolongation.

[Effect of electric cardiac stimulation in experimental supraventricular tachycardia].

Experimental supraventricular tachyarrhythmia was produced in dogs by the local application of aconitine on the left auricle. The effect of various types of stimulations (right auricular, right ventricular, double-delayed right auricular and right ventricular-right auricular double-delayed stimulation) was studied for the suppression of supra-ventricular (overdrive) tachyarrhythmia. It was found that the electric stimulation of the heart is an effective method of suppressing tachyarrhythmias, though there is a significant difference in the anti-arrhythmic effects of the electric stimulations of various types. For suppression of the supraventricular tachyarrhythmia caused by aconitine, delayed double-right auricular stimulation seemed to be the most effective with respect to the right ventricle. The bioelectric features of aconitine arrhythmia and the mechanism of the antiarrhythmic effect of the electric stimulation of the heart are discussed.

[The innominate artery syndrome].

During to the peculiar morophology and symptomatology of the haemodynamic disturbance following occlusion of the innominate artery, the anonyma syndrome ought to be treated as an independent type of the cerebro-vascular syndrome. The anonyma syndrome is generally a part-symptom of obliterating arteriosclerosis of multiple localization and has therefore an unfavourable prognosis. After a brief survey of the literature the clinical data of patients operated for innominate artery occlusion are reported together with the surgical results.

The effect of stimulation of the stellate ganglion on excitability of the heart.

Stimulation of the stellate ganglion leads to a shortening of the effective refractory period of the heart, while the electrical systole duration decreases in normal heart condition only secondarily in consequence of the increase in heart rate. In myocardial focal ischaemia stellate ganglion stimulation leads to an increase in the electrical systole time, the refractory period of the heart significantly shortens and the repolarization of the heart becomes inhomogenous. Consequently, the arrhythmogenic effect of a sympathetic excitation manifests itself mainly in myocardial ischaemia.

[Probability of electrode dislocation: the importance of threshold measurements during pacemaker implantation (author's transl)].

Myocardial stimulation thresholds were measured during 774 pacemaker implantations with endocardial electrodes. It was found that myocardial thresholds were higher when using electrodes with greater surface area and bipolar ones. Early electrode dislocations were more frequent at higher myocardial thresholds. There was no relationship between frequency of late electrode dislocations and the value of myocardial thresholds. The importance of measurement of myocardial stimulation threshold - as a guarantee of successful pacemaker therapy - is emphasized.

Heart block: histological and clinical features.

The clinical types of A-V-block and the localization of the underlying lesion to the conduction pathway have been examined in 25 patients. On the evidence of the post mortem studies the lesions were of the following types: 1) Necrosis or fibrosis of ischaemic origin: 13 cases; 2) lesions of inflammatory character: 5 cases; 3) nonspecific lesions; 6 cases; 4) malignant infiltration: one case. Correlations between the site of lesion and the clinical type and pattern of block could be demonstrated in a large proportion of the cases. The characteristic features of the individual types of block and their localization are discussed and a review of the literature is given.

Effect of ethyl apovincaminate on the cerebral circulation. Studies in patients with obliterative cerebral arterial disease.

The effect of ethyl apovincaminate (RGH-4405, Cavinton) on the cerebral and systemic circulations has been studied in detail in ten cases of cerebrovascular disease. 10 mg doses of Cavinton were given as infusion within 4-6 min; circulatory tests were carried out prior to administration of the drug and 3-6 min after. The principal results showed the following: On Cavinton cerebral vascular resistance was strongly reduced, while cerebral fraction of cardiac output significantly increased. On acute effect of the drug arterial mean pressure slightly decreased but cerebral blood flow nevertheless increased in general. Total vascular resistance also decreased but this decrease was less marked than that registered in cerebral vascular resistance.
