Fingerprint profiles in lymphocytic vacuoles of mucopolysaccharidoses I-H, II, III-A, and III-B.

Fingerprint (FP) profiles in vacuolated lymphocytes of mucopolysaccharidoses I-H, II, III-A, and III-B are a numerically rare, but possibly consistent finding as they have not been seen in vacuolated lymphocytes of other non-neuronal lipofuscinosis (NCL) lysosomal diseases. Their nosologic significance is not clear, but they may be as non-specific as tubular inclusions in lymphocytes and they are identical to those FP profiles seen in juvenile NCL.

[The problem of documentation in traumatology].

An exact documentation is a necessary demand of modern medicine. Different methods of the registration of information, for example the conventional sick-paper, the punch card and the electronic data processing documentation are opposed with their pros and cons. The own experiences of the surgical hospital of the Friedrich-Schiller-University with the documentation of injured patients is shown and explained. The text points to the importance of the electronic data processing registration and evaluation of the many facts of the injured patients concerning the therapeutic, epicritical, but also epidemic and legal questions.

Ultrastructurally abnormal mitochondria in the pituitary oncocytoma.

A pituitary adenoma in a 67-year-old man was characterized by abundant mitochondria and identified as an oncocytoma, which clinically and histologically appeared as a chromophobe adenoma. In addition to the numerous mitochondria within the neoplastic cells, structurally abnormal mitochondria were also present. Compared with other pituitary oncocytomas reported in the literature, abnormally structured mitochondria appear rare among the mitochondrial population of pituitary oncocytomas.

[Fatal sniffing of heroin (author's transl)].

Sniffing of heroin could be demonstrated in four acute deaths of drug addicts by radioimmunological analysis of the nasal mucosa. In addition, the influence of other drugs (benzodiazepines, barbiturates) or chronic diseases (hepatitis, cardiomyopathy), and acute alterations (pneumonia, myocarditis) were responsible for the fatal course. Different routes of application of drugs, especially heroin, are cited from literature and discussed.

[Biochemical methods for the determination of a clinical protein catabolism].

1. 20 patients before surgery received enteral nutrition for three days (12 g nitrogen, 1800 Kcal). Nitrogen and urea excretions in urine during the second and third day were determined. Eleven patients had a negative nitrogen balance (-2,7 and -2,4 g/day). In these patients urea production rates were 21,1 and 20,1 g/day. An urea production rate exceeding 15 g urea/day is probable an indication for a protein catabolism. The reason for this catabolic state seems to be a decreased protein utilisation (49 and 47 percent) as the result of a metabolic stress situation. This metabolic stress was determined according the stress index (Bistrian). The patients were in a stress situation comparable to postoperative stress (+3,7 and +3,9). The determination of urea production rate and catabolic index seems a suitable tool for defining a catabolic state. 2. 3-met-histidine excretion in urine were measured in seven patients postoperatively. In different periods saline or aminoacids solutions (5% alanine) were infused. During alanine administration protein (+49%)--and 3-met-histidine excretions (+50%) increased. It is not possible to state a catabolic situation out of the 3-met-histidine excretion, because an increased excretion may result from a stimulated protein synthesis in muscle tissue or from an increased muscleprotein wasting. 3. Free amino acid pools in plasma and muscle tissue were analysed in patients with severe illness of liver and pancreas. The free amino acid pattern differed from healthy volunteers. In patients with liver disease significantly increased concentrations of phenylalanine, tyrosine and methionine were found. In patients with acute pancreatitis highly abnormal pattern of intracellular amino acids occurred with decreased concentrations of glutamine, cysteine, histidine, lysine, arginine and ornithine. The highly significant decreased concentrations of glutamine (p less than 0,01) indicate a catabolic situation of these patients. A quantification of the severity of the catabolic state out of amino acid concentrations is not possible.

[Changes in glucose, fat and hormone balance induced by the postoperative administration of alanine solution].

Seven patients undergoing cholcystektomy received a 5 pevcent solution of alanine (90 mg/kg/h) during a period of 8 hours on the 1. postoperative day. Stimulation of gluconeogenesis was observed with increase of glucose from 5,9 +/- 0,2 to 6,3 +/- 0,2 mmol/l. Initially the level of ketone bodies decreased (acetoacetate 70 +/- 23 to 17 +/- 6 mumol/l) but again increased to start values during infusion of alanine. Insulin increased twofold from 7,6 +/- 1,4 microU/ml to 14,6 +/- 4,0, and had a constant level during infusion as did glucagon, which increased from 561 +/- 70 to 608 +/- 74 pg/ml. Nitrogen balance during infusionsperiod was almost zero (0,04 +/- 0,09 g/h minus) showing a significant difference to balances calculated from periods before and after infusion of alanine (0,57 +/- 0,07 and 0,53 +/- 0,06 g/h deficit respectively). Direct influence on peripheral glucose alanine cycle and increased gluconeogenetic substrate seem to be responsible for the nitrogen sparing effect of alanine.

[The definition of clinical catabolism by the urea production rate and its correlation with cellular immunity].

In 38 preoperative patients the production of urea was estimated and the cell-mediated immunity was measured. 10 patients were catabolic and had negative skin tests, all of them were inoperable. 15 patients were catabolic and had positive skin tests five patients were inoperable. 12 patients were not catabolic and had positive skin tests, two were inoperable. One female patient was not catabolic and had negative skin tests, but was operable. The catabolic state can be assessed by the urea production rate and you can give prognostic and therapeutical references in correlation with the delayed hypersensitivity.

[Urea production and cell-mediated immunity as indicators of preoperative catabolism (author's transl)].

In 38 preoperative patients the production of urea was estimated and cell-mediated immunity was measured. Ten patients were catabolic and had negative skin tests, all of them were inoperable. Fifteen patients were catabolic and had positive skin tests, five patients were inoperable. Twelve patients were not catabolic and had positive skin tests, two were inoperable. One female patient was not catabolic and had negative skin tests, but was operable. The catabolic state can be assessed by the urea production rate and one can give prognostic and trophotherapeutical indications in correlation with the degree of cellular immunity.

[Congenital cysts of the liver (author's transl)].

After reviewing the literature, case reports of 15 patients with congenital cysts of the liver are given. Surgery was done in these cases because of cholangitis, jaundice, suspected tumor. Cysts were removed totally, anastomized, punctured or supplied with a window, or lobectomy of the liver was done depending upon localisation, size, number and contents of the cysts.

[Cimetidine for treatment of stress-induced ulcer haemorrhage (author's transl)].

After gastroscopic demonstration of a definite source of bleeding (gastric or duodenal erosions or ulcers) 56 stress patients were treated with 1,200 mg cimetidine intravenously per day (constant infusion or 200 mg intravenously 4-hourly) in an open prospective study. Not considering 2 false indications because of arterial arrosion 46 patients were treated successfully with cimetidine. Bleeding could not be arrested in 7 cases and 2 of these succumbed, one of them despite repeated operations. In one patient bleeding was arrested; however, operation was necessary in the end due to perforation. Cimetidine has proved to be of value in conservative treatment of stress induced ulcer haemorrhage and is used increasingly prophylactically.

The ultrastructural variability of non-specific lipopigments.

Abnormally structured lipofuscin granules found in various cell types of different conditions not related to the neuronal lipofuscinoses (NCL), emphasize the ultrastructural diversity of non-specific lipofuscin morphologically marked by paracristalline inclusions, pseudo-fingerprint enclosures, and stacks of parallel filaments, all of them embedded in the granular matrix of the lipopigments. Though lipofuscin consisting of the regular granulo-vacuolar ultrastructure was also present in many cells of the same tissues, our findings suggest differences, though of unknown biochemical nature, in lysosomal catabolism resulting in varying lipopigment structures within similar cellular compartments.

[Panorama change in alcohol-criminality. Analysis of blood alcohol findings evaluated in the year 1965 and 1975 (author's transl)].

Evaluation of 8834 blood alcohol findings from individuals in police custody and suspect of criminal actions (3434 relate to the year 1965, 5400 to 1975). Classification of the findings according to the following aspects: blood alcohol concentrations in general and with regard to sex, profession, age and nature of the suspected offense--the findings of the two years (1965 and 1975) are compared and the individual results presented. The analysis suggests that: In young people of any age-group the absolute number of alcohol delicts shows a considerable increase during the last 10 years (1965--1975). In adults, with the exception of the 31--40 year age-group, there was no significant increase in these offences. Comparing the results of the individual delicts in 1965 and 1975 there was no significant difference in the level of the blood alcohol-concentration-groups. Unskilled workers were more frequently involved in nearly all alcohol-related crimes. In 1965 6,6% and in 1975 7,9% of alcohol-related offenses were committed by females. Compared with other districts in 1975 alcohol-criminality has increased significantly in the new suburban colonies of Hamburg (built since 1965) (public housing, high-rise housing).
