A 40-year-old woman with chronic symptoms of gastroesophageal reflux had a 1.5-cm filling defect in the distal esophagus on esophagogram. Endoscopy revealed distal esophagitis and, immediately above a hiatal hernia, a pedunculated polyp on a short stalk, which was removed by snare cautery. The polyp was an adenoma composed of tubular glands and covered by intestinal-type epithelium. Serial distal esophageal biopsies confirmed Barrett's metaplasia with areas of specialized columnar epithelium with goblet cells. These findings suggest that the full expression of neoplasia in Barrett's esophagus includes the development of adenomatous polyps as well as dysplasia and adenocarcinoma.

It is reported on phosphate-triester synthesis, isolation and sequencing of five tetradecadeoxyribonucleotides containing the modified bases 5-fluorouracil and 5-methylcytosine. The modified nucleotides can be exactly determined in the oligonucleotide chain by solid-phase MAXAM/GILBERT-sequencing. By combination of the oligonucleotides new hemi-modified DNA-duplexes were constructed which serve as model compounds for investigating the interaction with the EcoRII restriction and modification enzymes.

Paget's disease may present in the tibia without involvement of either end of the bone. Such unusual diaphyseal lesions are invariably lytic and, rarely, may be multifocal. They almost always involve the anterior cortex of the bone and usually have a characteristic 'V-shaped' margin at one or both ends. Expansion of the bone is common. This unusual appearance is frequently an early manifestation of the disease under the age of 40 years. Awareness of this appearance is important in order to prevent incorrect diagnosis and unnecessary biopsy or surgery.

Pituitaries from 38 human fetuses and 5 microdissected hypothalamic areas from each of 14 fetuses were assayed for vasopressin and oxytocin. The fetuses were between 15 and 34 weeks of gestation. Vasopressin and oxytocin were found in all the pituitary glands and all the hypothalamic areas studied. A trend of increasing content of both hormones was found in the pituitary gland towards the end of gestation with a marked increase in vasopressin/oxytocin ratio. The human fetus synthesizes oxytocin and vasopressin at least from 15 weeks of gestation and the presence of both hormones in the hypothalamus is not limited to the supraoptic and paraventricular nuclei.

The study of pain by analysis of the postoperative consumption of opiates shows a significant influence of the operation and the personal factor N. High N-scores and long operations lead to a significant increase of the postoperative consumption of opiates. Fixed combinations for opiate medication are as useless as a time-fixed method for the giving of these drugs. The individual preparation and medication should always be preferred.

The coupling of 8-aminomethylene blue to oligonucleotides via poly-L-glutamic acid linker using carboxy-anchor groups will be described. The introduction of carboxy-anchor groups into oligonucleotides proceeds both during automated synthesis using 6-(ethoxycarbonyl)hexyl 1-O-phosphoramidite and by reaction of 5'-amino-functionalized oligonucleotides with succinic anhydride. O-(N-Succinimidyl)-1,1,3,3-tetramethyluronium tetrafluoroborate was used as activating reagent for binding of poly-L-glutamic acid to the carboxylated oligonucleotides. The successful 5'-carboxylation and poly-L-glutamic acid coupling were proven both by polyacrylamide gel electrophoreses and HPLC. 8-Aminomethylene blue in its leucoform was covalently coupled to the oligonucleotides in the presence of water soluble carbodiimide.
