Since plasma is largely involved in autoimmune diseases, plasma exchanges have been used to treat these patients. In the first part of this paper the authors review the indications for plasmapheresis, which they divide into two groups: autoimmune diseases proper, and diseases with autoimmune phenomena. In the second part, the results available in the literature are analyzed. In many cases, it is extremely difficult to form an opinion, since the methods vary from one group of workers to the other. Unquestionably, improvement was observed in bullous pemphigus and pemphigoid, in Goodpasture's syndrome, in glomerulonephritis with autoantibodies and in myasthenia. In other cases, dramatic improvement was sometimes observed (notably in some cases of acute polyradiculoneuritis), but the small number of patients in each series makes it impossible to separate the effects of plasma exchanges and those of the immunosuppressive treatment frequently administered concomitantly.
The authors report a case of severe hypertriglyceridemia (148.5 mmol/l) in a 27-year-old woman admitted for coma of unknown origin. Initial investigations revealed ketoacidosis, pancreatitis and noncardiogenic pulmonary edema. The diabetes was unknown. Ketoacidosis was rapidly controlled. The hypertriglyceridemia was corrected by one course of plasma exchange (4,400 ml) during which the patient returned to consciousness. The patient recovered without any sequelae. Only 2 similar cases, treated by plasma exchange, have been reported in the literature until now.
The authors report one case of Pyoderma gangrenosum whose unusual cervical localization was particularly bothering. Besides, the patient, an 18-year-old girl, had clinical symptoms of Crohn's disease confirmed by radiological investigations and bowel pathology. A benign monoclonal gammapathy and an auto-immune haemolytic anaemia were also found. After four plasma exchanges Pyoderma healed rapidly, Crohn's disease stabilized and biological abnormalities disappeared. Comments and review of the literature are reported. It appears that such a case is quite exceptional, as far as clinical, biological data and therapy (plasma exchange) are concerned.
Granulocytes have been transfused systematically for several years owing to the utilisation of cells separators. It seems profitable to test these granulocytes from a functional standpoint since they are centrifuged and then collected in plastic bags. Several function tests are used and demonstrate that the quality of these granulocytes is on the average diminished without altering them significantly. Morphological studies complementing these qualitative studies demonstrate membrane fusions and partial degranulation.

The screening of zoster's antibodies has been performed in blood donors population and persons convalescing from herpes zoster. The results show that 25% of blood donors and 83,5% of convalescents have respectively a titer of CF antibody of 8. Plasma units were pooled and the pool had a final titer of CF antibody of 16. Varicella zoster immunoglobulin prepared by alcohol fractionation had a fluorescent antibody titer of 1 024 and a CF antibody of 512. These results are discussed and compared with those of literature.
Although T and acquired B polyagglutinabilities are not exceptional, simultaneous occurence of the two types is a rarer phenomenon. Ten days after an open heart surgery operation, the patient, age 55, had an infectious syndrom : two strains, Clostridium perfringens and Peptococcus variabilis were isolated. Simultaneously, her red cells were found to be polyagglutinable. The association of T and acquired B polyagglutinabilities could be demonstrated by serological studies of the patient's red cells, and by in vitro transformation of normal red cells using culture supernatants obtained from the two isolated strains.
After reviewing data from the literature concerning the effects of normovolaemic haemodilution on cardiac output and regional flow rates, the authors illustrate these concepts by a personal study involving the haemodynamics of 10 subjects undergoing operation in normovolaemic haemodilution. Removal of blood was compensated simultaneously by modified liquid gelatin in electrolytic solution (Plasmion) until the haematocrit was 0.30. Measurements were performed before haemodilution in patients in a steady state (anaesthetised, intubated, normoventilated), at the end of haemodilution, at the end of the operation, then 4 hours after recovery. There was no variation in blood pressure and heart rate, showing that normovolaemia was maintained. At the end of haemodilution, cardiac index increased from 3.10 to 3.84 l.min-1.m-2 (0.0517 to 0.0638 l.s-1 . m-2) (p < 0.005); stroke volume increased from 70 to 83 ml (p < 0.005); systemic resistance fell from 1585 to 1262 dynes. s.cm-5 (158.5 to 130.4 kPa.s.l-1) (p < 0.005); arterial oxygen content decreased from 191.1 to 152.1 ml.100 ml-1 (8.535 to 6.793 mmol.l-1) (p < 0.005), whilst oxygen transport was unchanged. These various haemodynamic measurements showed no significant changes at the subsequent times when they were measured, values remaining close to those obtained at the end of haemodilution. The results confirm the fact that norvolaemic haemodilution is accompanied by a fall in systemic vascular resistance with an increase in cardiac output. Regional circulations are thus improved. Since oxygen transport is unaffected, the oxygenation of peripheral tissues is ensured normally.

The efficacy of zoster immunoglobulin in prophylaxis of varicella and herpes zoster is reported by many authors. The screening for zoster's antibodies is performed in the blood donor population and in persons convalescing from herpes zoster. The results show that 25 per cent of blood donors and 83.5 per cent of convalescents, respectively, have CF antibody titer of l6. Varicella zoster immunoglobulin prepared by alcohol fractionation has a fluorescent antibody titer of 1,024 and a CF antibody titer of 512. These results are discussed and compared with those of the literature.
