Subcortical correlates of differential classical conditioning of aversive emotional reactions in social phobia.

BACKGROUND: Conditioning processes have been proposed to play a role in the development of anxiety disorders. As yet, the neurobiologic correlates of emotional learning have not been fully understood in these patients. Accordingly, brain activity was studied in subcortical and cortical regions involved in the processing of negative affect during differential aversive classical conditioning. METHODS: Twelve patients with social phobia and 12 healthy control subjects were presented with paired conditioned (CS; neutral facial expressions) and unconditioned stimuli (US; negative odor vs unmanipulated air). Functional magnetic resonance imaging (fMRI) was utilized to examine regional cerebral activity during habituation, acquisition,a nd extinction trials. Activity was measured with echo-planar-imaging (EPI), and signal intensity in individually defined anatomic regions were analyzed. RESULTS: Subjective ratings of emotional valence to the CS indicated that behavioral conditioning occurred in both groups. The presentation of CS associated with negative odor led to signal decreases in the amygdala and hippocampus of normal subjects, whereas an opposite increased activation in both regions was observed in patients. Regional differences were not found during habituation and extinction. CONCLUSIONS: Results suggest that conditioned aversive stimuli are processed in subcortical regions, with phobic patients differing from control subjects.

The evolution of minimally invasive valve surgery--2 year experience.

OBJECTIVES: The aim of the study was to evaluate the evolution of Port-Access minimally invasive mitral valve surgery to a robot assisted video assisted solo surgery approach. METHODS: One hundred and twenty-nine patients with non-ischemic mitral valve disease underwent 3D-video assisted mitral valve surgery via a 4 cm right lateral minithoracotomy using femoro-femoral bypass and endoaortic clamping. Transcranial Doppler and continuous transesophageal echocardiography were used to monitor placement and positional stability of the endoclamp. After the initial series (group I, n = 62), a simplified solo surgical technique using voice controlled robotic assistance for videoscope guidance was used in the last 67 patients (group II). RESULTS: After an initial learning curve and modifications of catheter design, the procedure could be steadily redefined and simplified. In the last 67 patients, the procedure was completed without the need for an additional assistant as 'solo surgery'. The mitral valve was repaired in 72 and replaced in all other patients. Duration of bypass and clamp time steadily improved during our study and in the most recent 67 patients average 107 +/- 34 and 48 +/- 16 min, respectively. The voice controlled robotic arm (AESOP 3000, Automated Endoscope System for Optimal Positioning) provided a stable and precise video image with excellent exposure of all valvular and subvalvular structures. Hospital mortality was high in the early series (mean survival 88.7% at 804 +/- 35 days; 95% CI: 735-873) and partially procedure related (aortic dissection in two patients). In group II, hospital mortality has declined to 3.0% (mean survival 97.0% at 568 +/- 12 days; 95% CI: 553-600). CONCLUSION: Port-Access minimally invasive mitral valve surgery has evolved to be a reliable video assisted technique with reproducible results. Surgery can now be performed almost in the same time as with conventional techniques. Robotic assistance has enabled a solo surgery approach.

Identification of the EGF/EGF-R system in the oviduct and endometrium of pigs in early stages of pregnancy and early conceptus.

In the oviduct and endometrium, higher mRNA amounts encoding for EGF were found on day 6 and 12 in cyclic and on day 6 in pregnant pigs compared to signals on day 1. Reproductive state related changes could also be detected for TGFalpha mRNA in cyclic and pregnant pig oviduct but not in the endometrium. EGF-R protein concentration was higher (P < 0.05) in oviductal membranes on day 1 of the pregnancy (42.6 +/- 16.5 fmol mg(-1) protein) compared to day 6 and 12 (21.6 +/- 6.0 fmol mg(-1) protein and 17.1 +/- 3.8 fmol mg(-1) protein). In the endometrium EGF-R protein concentrations increased (P < 0.05) on day 1 (14.7 +/- 4.8 fmol mg(-1) protein) to day 6 (29.0 +/- 6.8 fmol mg(-1) protein) and day 12 (27.5 +/- 7.0 fmol mg(-1) protein) of pregnancy. The mature EGF-R protein (170 kDa) was verified in oviductal and endometrial membranes of all pregnant pigs investigated. A biologically intact EGF-R could be detected in all samples by means of autophosphorylation assay. Weak EGF mRNA signals were found in the expanded and filamentous blastocysts. No TGFalpha transcripts could be amplified during the embryonic stages. Only low amounts of EGF-R mRNA could be detected in zygotes and in filamentous blastocysts.

A tissue-like culture system using microstructures: influence of extracellular matrix material on cell adhesion and aggregation.

Special microenvironmental conditions are required to induce and/or maintain specific qualities of differentiated cells. An important parameter is the three-dimensional tissue architecture that cannot be reproduced in conventional monolayer systems. Advanced tissue culture systems will meet many of these demands, but may reach their limits, especially when gradients of specific substances over distinct tissue layers must be established for long-term culture. These limitations may be overcome by incorporating microstructures into tissue-like culture systems. The microstructured cell support presented consists of a flat array of 625 cubic microcontainers with porous bottoms, in which cells can be supplied with specific media from both sides of the tissue layer. Permanent cell lines and primary rat hepatocytes have been used to test the culture system. In order to define reproducible conditions for tissue formation and for cell adherence to the structure, several ECM (extracellular matrix) components were tested for coating of microstructured substrata. The described tissue culture system offers great flexibility in adapting the cell support to specific needs.

[Detection of malignant focal hepatic lesions. Comparison of ultrasonography, computerized tomography during arterial portography, delayed computerized tomography and magnetic resonance imaging].

AIMS: This study was performed to compare the sensitivity of ultrasonography, computerized tomography during arterial portography, delayed computerized tomography, and magnetic resonance imaging to detect focal liver lesions. Forty three patients with primary or secondary malignant liver lesions were studied prior to surgical intervention. METHODS: The results of the imaging studies were compared with intraoperative examination of the liver, intraoperative ultrasonography and pathology results (29 patients). In the non-operated (14 patients) group, we compared the number of lesions detected by each technique. RESULTS: One hundred and forty six lesions were detected. There was 84% sensitivity with computerized tomography during arterial portography, 61.3% with delayed scan, 63.3% with magnetic resonance imaging and 51% with ultrasonography in operated patients. In patients who did not undergo surgery, magnetic resonance imaging was more sensitive in detecting lesions. CONCLUSIONS: In operated and non-operated patients series, CT during arterial portography had the highest sensitivity, but magnetic resonance imaging had the most consistent overall results.

[Wound healing and hyperbaric oxygen. Experimental study of the angiogenesis phase in the rat].

Hyperbaric oxygen therapy (HBO) is widely reported as highly favourable to wound healing. The experimental models generally used to investigate its effects are difficult to set up and reliable quantification of the results obtained is rarely achieved. The underlying pathophysiological mechanisms occurring during HBO remain poorly understood and its mode of application for clinical practice is difficult to standardise. Our study was carried out to assess the contributions of oxygen and hyperbaric pressure on the initial steps of wound healing. It was based on qualitative and quantitative analysis of the development of the angiogenic process in a granulation tissue bud, using animals implanted with fibrin chambers, an in vivo model initially described by Dvorak in guinea pigs. In our study, rats were further submitted to HBO (OHB group) or hyperbaric air (Air-HB group) treatments. The control group (Control group) consisted of rats maintained in the treatment tank under normal atmospheric conditions. Nine specific parameters were determined and analysed during the course of the angiogenic process by classical histological techniques. The vascular density and the height of the bud were particularly examined at day 7, 14 and 21 following chamber implantation. At D7 the neovessel density and bud height were significantly higher in OHB group than in Air-HB or Control groups, thus confirming the beneficial effects of this treatment on the initial steps of wound healing. Nevertheless, the results reported herein also suggest a possible inhibitory effect of hyperbaric therapy alone on this very early process, although the pathophysiological significance of this effect on wound healing remains to be determined.

[Detection of airborne cultivatable microorganisms from compost sites--emissions and imissions].

The statement presented here gives an overview and assessment of the procedures and concepts currently used for the collection and determination of airborne, culturable microorganisms at sources of emission within composting plants and in their near vicinity. The paper focuses on "classical" methods, which involve cultivation as an intermediate step for the determination of viable, airborne microorganisms. The theoretical and practical requirements on such methods are discussed. Results and experiences from recent investigations are described.

Differential amygdala activation in schizophrenia during sadness.

Several studies have reported impaired emotion processing in schizophrenic patients. However, the corresponding functional cerebral correlates of such impairment have not been fully understood, leaving the neurobiological basis of their affective symptoms unknown. Functional magnetic resonance imaging (fMRI) was utilized to examine brain activity in subcortical and cortical regions of 13 medicated male schizophrenic patients and 13 matched healthy controls during happy and sad mood induction. Results show brain activity in the amygdala of normal controls during negative affect, which is in line with previous neuroimaging findings. Unlike controls, schizophrenic patients have not demonstrated amygdala activation during sadness despite matched ratings to normal controls indicating a similar negative affect. Recognizing that structural abnormalities exist in the amygdala of schizophrenic patients, our results provide new evidence of functional abnormalities in the limbic system.

Induction of 5-hydroxytryptamine release by tramadol, fenfluramine and reserpine.

Tramadol is a centrally acting analgesic with several modes of action. Enhancement of 5-hydroxytryptamine release contributes to its actions. We investigated in which way tramadol induces 5-hydroxytryptamine release. Rat brain frontal cortex slices were preincubated with [3H]5-hydroxytryptamine, then superfused using conditions which impaired either carrier mediated release or exocytosis. Tramadol (10 and 100 microM), fenfluramine (1 microM) and reserpine (10 microM) enhanced the basal release of [3H]5-hydroxytryptamine. In the presence of a high concentration of 6-nitroquipazine effects of tramadol were reduced and those of fenfluramine abolished. Effects by reserpine were enhanced, indicating that [3H]5-hydroxytryptamine depletion was counteracted by reuptake. When NaCl was replaced by LiCl, tramadol did not affect [3H]5-hydroxytryptamine release, fenfluramine induced a small and reserpine a marked facilitation. Omission of CaCl2 did not alter fenfluramine and reserpine effects while those by tramadol were reduced. It is concluded that tramadol induces both carrier mediated 5-hydroxytryptamine release as well as exocytosis.

fMRI reveals amygdala activation to human faces in social phobics.

Functional magnetic resonance imaging was used to determine the activation of the amygdala while seven social phobics and five healthy controls were exposed to slides of neutral faces as well as aversive odor stimuli. The amygdala was selectively activated in the social phobics during presentation of the face stimuli. The data show for the first time that the amygdala is active in human phobics when they are exposed to potentially fear-relevant stimuli. Further research is needed to determine the extent to which overactivation of the amygdala precedes or is a consequence of phobia.

Effects of treating young boars with a GnRH depot formulation on endocrine functions, testis size, boar taint, carcass composition and muscular structure.

A study was designed to determine the effects of triptorelin (Decapeptyl C.R.), a GnRH-agonist in a depot formulation, on androst-16-en-3-one (androstenone), LH, oestradiol and testosterone and on carcass composition and muscle structure in boars. Treatment with this highly potent GnRH analog ensured peptide release for 4 weeks when given to 18 animals at 135 days of age (group 1). Control animals (n = 20, group 2) received a saline injection. Blood samples (n = 10) were collected by venipuncture from each boar from 7 days before hormone treatment to 28 days after treatment. During the treatment period, three samples of back fat were taken by biopsy to determine the levels of androstenone and skatole. Testis size was determined at slaughter at 165 days of age. The treatment of boars with triptorelin increased concentrations of LH, oestradiol-17 beta, and testosterone for one day, followed by a decrease to base-line concentrations. The result of sensorial testing for boar taint detection at slaughter showed a slight reduction in this trait (0.76 vs. 0.99). The concentration of androstenone in the body fat of all treated animals was lower than the generally accepted limit of 0.5 microgram/g for the absence of boar taint, compared with the concentrations from 20 control animals among which 13 showed levels higher than this limit. The hormonal treatment significantly reduced the weight and size of the tests. There was no effect on skatole levels in backfat (0.141 vs. 0.150 microgram/g). The GnRH-agonist did not influence the carcass composition of treated boars. The muscle meat percentage was not significantly altered in the experimental group (51.39 vs. 50.50%). Overall, these results indicate that the use of this form of GnRH-agonist in a depot formulation offers a new possibility for reducing concentrations of androstenone which is mainly responsible for undesirable boar taint.

[Gastrointestinal tuberculosis--an almost forgotten clinical picture].

Incidence of gastrointestinal tuberculosis increases in the western world. Exact epidemiological data about this rare extrapulmonological disease are not available in Hungary. Three advanced cases with intestinal tuberculosis have been reported, all of them recognised during surgery due to complications. The clinicopathological correlations are briefly reviewed, and the importance of early diagnosis is emphasized.

Estimated susceptibility to asymptomatic secondary immune response against measles in late convalescent and vaccinated persons.

Serological evidence indicates that measles virus (MV) could circulate in seropositive, fully protected populations. Among individuals fully protected against disease, those prone to asymptomatic secondary immune response are the most likely to support subclinical MV transmission. The serological characteristics of protected subjects who developed secondary immune response after reexposure to measles have been described recently [Huiss et al. (1997): Clinical and Experimental Immunology 109:416-420]. On the basis of these data, a threshold of susceptibility was defined to estimate frequencies of secondary immune response competence in different populations. Among measles, late convalescent adults (n = 277) and vaccinated high school children (n = 368), 3.2-3.9% and 22.2-33.2%, respectively, were considered susceptible to secondary immune response. A second vaccination did not seem to lower this incidence. Even when estimates of symptomatic secondary immune response (e.g., secondary vaccine failure) were taken into account, susceptibility to subclinical secondary immune response was still 5-8 times higher after vaccination than after natural infection. Although viral transmission between protected individuals has never been directly demonstrated, the data describe a population in which protected but infectious persons could potentially be of epidemiological importance.

Control of endoaortic clamp position during Port-Access mitral valve operations using transcranial Doppler echography.

The technique of computer-aided transcranial Doppler echography for continuous control of cerebral perfusion during minimally invasive mitral valve operations using the Port-Access system is described. Temporary displacement of the endoclamp in the aortic arch (brachiocephalic trunk, left carotid artery) is followed by cerebral embolic signals and a sudden decrease in the blood flow velocity in the middle cerebral arteries.

A simplified immunoassay based on measles virus recombinant hemagglutinin protein for testing the immune status of vaccinees.

Simplified tests based on recombinant antigens are considered to be important for monitoring immunity against measles virus (MV). The hemagglutinin protein (H) is the main target for neutralising and protective antibodies. We produced a recombinant MV-H protein, in a high-yield mammalian expression system based on the Semliki Forest virus replicon. The antigenicity of this recombinant protein was investigated with monoclonal antibodies and its suitability for measuring the immune status of vaccinees was tested in a large cohort by ELISA (H-ELISA). The results were evaluated against neutralisation (NT) and hemagglutination inhibition (HI) titers and MV-specific IgG measured in a commercial whole-virus based ELISA (MV-ELISA, Enzygnost). The H-ELISA correlated better with HI (r=0.78) and NT titers (r=0.80), than the MV-ELISA (HI, r=0.58; NT, r=0.59). In contrast to the MV-ELISA, the H-ELISA detected no false-positive sera (P < 0.02) and the number of false-negative sera was significantly lower in the H-ELISA than in the MV-ELISA (4/378 vs. 15/378; P < 0.025). The performance of the H-ELISA did not deteriorate significantly when, instead of background corrected net values, uncorrected raw O.D. values of the H-antigen were considered, or when early time points (30 min) were evaluated. These results demonstrate that the recombinant H-ELISA detects efficiently non-immune individuals among vaccinees, despite their relatively low MV-antibody levels. A simplified format with single value measurements did not result in loss of sensitivity or specificity and its performance compared favorably with commercial ELISAs based on whole virus.

Balkan endemic nephropathy, the haematopoietic system and the environmental connection.

We previously reported the detection of an increased subpopulation of cytotoxic T lymphocytes in patients with Balkan (endemic) nephropathy (BEN) and in area controls (individuals free of clinical syndrome but born in a BEN endemic area and having a family history of BEN). Extending the flow-cytometric analyses to other populations of peripheral blood leucocytes, we found a decrease in the proportion of B lymphocyte subset and an increased proportion of eosinophils in BEN patients and in area controls. Although these numerical alterations cannot be categorically linked to the aetiopathogeny of the disease, it is presumed that they can be induced by the same factor(s) causing the kidney damage, through a direct haemato- and lymphotoxic effect.
