Analysis of cytogenetic damage induced in CHO cells by the pyrethroid insecticide fenvalerate.

A synthetic pyrethroid insecticide, fenvalerate, was tested for its ability to induce chromosome aberrations (CA) and sister chromatid exchanges (SCE) in CHO cells. Fenvalerate was assayed both in the presence and in the absence of a rat liver activation system (S9-mix). Our results indicate that fenvalerate in the presence of S9-mix is able significantly to increase the frequency of CA, while in the SCE test this increase occurred both in the presence and in the absence of S9-mix. In addition, fenvalerate affected the cell cycle, causing a decrease in the mitotic index (MI) and in the proliferative rate index (PRI).

Neutralizing-enhancing monoclonal antibody recognizes the denatured glycoprotein of viral haemorrhagic septicaemia virus.

Two monoclonal antibodies (MAbs), 1H10 and 1F10, have been selected against the denatured glycoprotein of the viral haemorrhagic septicaemia virus (VHSV) of salmonids by immunoblotting. Three reference VHSV serotypes (F1, F2, 23.75) and five VHSV isolates from either different host species (trout, salmon, barbel) or geographical locations in Spain reacted with both MAbs by ELISA. In vitro neutralization of all VHSV serotypes but not infectious haematopoietic necrosis virus (IHNV) was obtained only with MAb 1H10. However, when the MAb 1H10 (but not the non-neutralizing Mab 1F10) concentration was below 100 micrograms/ml rather than neutralizing VHSV infectivity, plaque counts increased 2-3-fold. MAb 1H10 is unique in that it has both, an in vitro enhancing infectivity effect (not described yet for VHSV) and it recognizes denatured G protein in contrast to other previously described neutralizing MAbs against VHSV.

Validation of the medical expert system PNEUMON-IA.

The present study validates the expert system PNEUMON-IA. The aim of PNEUMON-IA is assessing the etiology of community-acquired pneumonias from clinical, radiological, and laboratory data obtained at the onset of the disease. Validation was performed using data from medical records of 76 patients with proven clinical diagnosis of pneumonia. The etiological diagnoses provided by PNEUMON-IA were compared to those established by five specialists unrelated to the development of the expert system. For each etiological possibility, both PNEUMON-IA and the experts provided a causal possibility, expressed as a linguistic label (i.e., "almost impossible"). Linguistic labels were then converted to numeric values. In the majority of cases, an etiological diagnosis was unavailable to be used as a gold standard. To overcome this limitation, distances between arrays of etiological possibilities given by specialists and by PNEUMON-IA were considered as an agreement measure between diagnoses. Cluster analysis based on those distances was used to classify PNEUMON-IA among experts. Results showed the same differences between specialists and PNEUMON-IA as among the specialists themselves. The method used to validate PNEUMON-IA could prove useful to assess the performance of expert systems in fields where no gold standard is available.

[Nosocomial fungemia caused by Candida parapsilosis].

We review 27 episodes of nosocomial fungemia due to Candida parapsilosis over a 6 year period, compared to a control group of 27 episodes of nosocomial fungemia due to Candida albicans. During the study period, C. parapsilosis accounts for 23% of all yeast isolated from blood-cultures. Fungemia due to C. parapsilosis was more frequently seen in males (23/4). More than half of the cases (15/27) presented in the postoperative period. In 89% of cases the patients were under total parenteral nutrition and 81% had received broad-spectrum antibiotics. In 41% of cases, the source of the fungemia was unknown, and in another 41% of cases was related to an iv line infection. Direct attributable mortality to C. parapsilosis infection was 11%. When compared to the control group, nosocomial fungemia due to C. parapsilosis occurs in patient with more prolonged courses of total parenteral nutrition, and also was related with less frequent development of septic shock and lower attributable mortality.

Automatic search for maximum similarity between molecular electrostatic potential distributions.

A new computer program has been developed to automatically obtain the relative position of two molecules in which the similarity between molecular electrostatic-potential distributions is greatest. These distributions are considered in a volume around the molecules, and the similarity is measured by the Spearman rank coefficient. The program has been tested using several pairs of molecules: water vs. water; phenylethylamine and phenylpropylamine vs. benzylamine; and methotrexate vs. dihydrofolic acid.

The free nucleocapsids of the viral haemorrhagic septicaemia virus contain two antigenically related nucleoproteins.

A protein of 34 kDa (Nx) was induced in vitro by the infection of fish cell cultures with the rhabdovirus agent of viral haemorrhagic septicaemia (VHS) of the trout. This protein only appeared as a major component in concentrated or intracellular labeled VHS virus but not in purified VHS or in the related infectious haematopoietic necrosis virus. That Nx protein is antigenically related to the nucleoprotein of purified virus was shown by its reaction with four anti-nucleoprotein monoclonal antibodies (at least 3 of them reacting non competitively against different epitopes) and by immunoprecipitation with polyvalent international reference sera. The Nx protein was shown to be specifically associated with free non-infective particles isolated by ultracentrifugation which were confirmed to be nucleocapsids by electron microscopy.

Stimulation of adherent cells by addition of purified proteins of viral hemorrhagic septicemia virus to trout kidney cell cultures.

Purified proteins of the virus causing viral hemorrhagic septicemia in the trout were added to cultures on semisolid medium of leukocytes obtained from either healthy or immunized rainbow trout. Adherent cells were specifically stimulated by the glycoprotein of the viral spikes and, to a lesser extent, by the nucleoproteins. In contrast, a specific memory response was associated more with the nucleoproteins than with the glycoprotein when leukocytes from trout immunized with the virus were employed. These results suggest the necessity of employing both proteins in subunit vaccination trials and the possibility of using this assay to select the proper epitopes for genetically engineered proteins during subunit vaccine development.

[Ten-year survival of a cohort of 736 patients with a first myocardial infarction. Sex-related differences?].

The survival of a cohort of 736 patients (617 men and 119 women) with a first acute myocardial infarction is reported. All patients were admitted and diagnosed at a referral center of six areas of the province of Girona, Spain. The results of a follow-up period of 10 years are analyzed. The overall 10-year survival rate was 55% (57% in men and 43% in women). The survival curves for males and females were significantly different (p = 0.0001). In the acute phase of infarction, the mortality amongst women was higher than amongst men. Women suffered from myocardial infarction at an older age than men. When the study population was stratified by age groups, no statistically significant differences in the survival rates between men and women were observed. We conclude that age is the confounding variable that all other survival-related variables have to be adjusted for. Sex was not found to be a determinant factor for long-term survival after myocardial infarction. The worse hemodynamic condition amongst women prior to the infarction may account for the greater mortality in the acute phase observed in females.

Toxicity of fractions obtained from the legume species Astragalus lusitanicus Lam. lusitanicus.

Extracts of Astragalus lusitanicus Lam. lusitanicus, a European legume very common in southwestern Spain and Portugal, caused in mice acute toxicity, with convulsions and death in a few min, and delayed death after (10-15 days) with first symptoms appearing 6-8 days after a single i.v. dose. The toxic compounds could be separated by column chromatography or methanol solubility, but were unstable and could not be isolated in pure form. Plant toxicity could not be attributed to the previously described compounds pinitol, isormhamnetin glycosides or saccharinic acid lactone, which did not show toxicity in mice after i.v. administration.

Effects of long-term treatment with metoprolol and hydrochlorothiazide on plasma lipids and lipoproteins.

In order to evaluate the effects of one-year antihypertensive treatment on plasma lipids and lipoproteins, 65 patients whose diastolic blood pressure was in the range 95-120 mmHg were randomly allocated to groups that received either hydrochlorothiazide or metoprolol, or both drugs when the response to one of them was insufficient to control blood pressure. Blood pressure was effectively reduced in all groups. Patients on hydrochlorothiazide showed a significant increase (P less than 0.01) in low-density lipoprotein cholesterol (LDL-C) after 3 months of treatment. A significant increase in triglycerides was observed after 6 and 12 months, together with a decrease in high-density lipoprotein cholesterol (HDL-C) after 12 months (P less than 0.05) of treatment in patients on metoprolol. In patients treated with both hydrochlorothiazide and metoprolol, total cholesterol increased after 3 (P less than 0.001) and 6 months (P less than 0.05), triglycerides increased after 6 (P less than 0.01) and 12 months (P less than 0.01), and LDL-C increased after 3 (P less than 0.05), 6 (P less than 0.001) and 12 months (P less than 0.01) of treatment, respectively. In 61% of the patients, three or more lipid parameters were affected during the study period. We conclude that long-term antihypertensive treatment with hydrochlorothiazide, metoprolol, and particularly with both drugs, can induce lipid effects that deserve recognition, because in some cases these might counteract the possible benefit of a reduction in blood pressure on the prevention of coronary heart disease.

[Epidemiology and course of craniocerebral injuries in children in Cantabria].

We present the epidemiology and the outcome of the patients younger than 15 years-old who were admitted to our hospital during 1986 with the diagnosis of head injury. One hundred and fifty-five met the required inclusion criteria, this offers an incidence of head injury in children of 139/100,000. The boy/girl ratio was 2.1/1. The commonest causes were traffic accidents (45%), mainly those knocked down or on bicycles, and falls (41%). According to the Glasgow Coma Scale 88.5% of the head injuries were slight, only 11.5% being moderate or severe. The outcome was good in 97%. There were two intrahospital deaths, that happened in the 8 patients with severe head injury, giving a mortality rate of 25% for this group; and 5 deaths out of hospital. Thus, the global mortality secondary to head injury for children was 6.3/100,000 and year. These data show: a) the sanitary importance of the head injury in children; b) the necessity of a separate study of the head injury in children since the precipitating causes and the clinical outcome are specific; and c) the shortcomings in the urgent out of hospital medical attention, as indicates an out of hospital mortality rate of 71% which is well above that of other developed countries.
