Analysis of a polytef granuloma mimicking a cold thyroid nodule 17 months after laryngeal injection.

A 65-year-old female who received laryngeal injections with polytef (Teflon) paste for treatment of right vocal cord paralysis first noticed a small, right anterior neck mass 17 months later. Clinically this mass slowly progressed in size and was felt to be a recurrent carotid body tumor, but at surgery, 22 months after injection, a circumscribed nodule on the anterior right lobe of the thyroid was found and histologically diagnosed as a foreign body granuloma. Scanning electron microscopy (SEM) and energy-dispersive x-ray analysis (EDXA) of material in this lesion was identical to that of polytef paste, which is a mixture of pyrolyzed polytetrafluoroethylene and glycerine. The possibility that deaggregated smaller particles in the polytef paste were able to migrate from the injection site resulting in this neck mass is considered.

[Antireflux plastic repair of the cologastric anastomosis in a female patient operated on by retrosternal esophagocoloplasty].

Esophageal atresia without fistula is a rather uncommon malformation (about 8% of all esophageal atresias); its surgical correction is sometimes difficult because of the long gap between the two pouches. When a delayed anastomosis cannot be performed, it is necessary to perform a colon interposition. In one patient operated with retrosternal esophageal-colon-plasty a second operation was needed to correct a symptomatic reflux causing dysphasia and severe aspiration pneumonia. The distal end of the colon was tapered around a chest tube; then a submucosal gastric tunnel was prepared, and the tapered colon was pulled through it and anastomosed to the gastric lumen. The child showed no evidence of gastrocolic reflux in the three years following the operation and is today healthy. Radiologic and scintigraphic examinations, performed ten months after the operation, showed the good canalization of the interposed colon and the continence of the antireflux anastomosis.

Expression of ABO blood group, hematopoietic, and other cell-specific antigens by cells in the human cornea.

Because of controversy surrounding the expression of ABO blood group (ABH) antigens (which may act as transplantation antigens) in human cornea, we examined the expression of these antigens in a panel of normal corneas using a highly sensitive avidin-biotin complex immunoperoxidase technique. ABH antigens were expressed by the corneal epithelium from all donors in a pattern consistent with their red blood cell phenotype, but were absent from stroma and endothelium. We also examined the same corneas for cells expressing hematopoietic cell markers in an effort to determine which marrow-derived cells may be contributing (as "passenger leukocytes") to the immunogenicity of corneal allografts. Scattered cells positive for a pan-hematopoietic cell marker (T29/33) were present throughout all corneas, but no consistent expression was noted of markers for B lymphocytes, monocytes, or NK cells. Adult corneas showed occasional T lymphocytes, predominantly in the limbus. Finally, we looked for the expression by these corneas of a variety of cell-specific and structural antigens, to identify possible relationships between corneal cells and analogous cell populations. We found that corneal endothelial cells expressed one vascular endothelial antigen, (HuEE12), but not another (Factor VIII related antigen), suggesting a possible relationship between corneal endothelium and its immunologically important vascular counterpart. Various structural antigens were found in predicted associations with corneal endothelial, stromal, and epithelial cells. These findings cast new light on the antigens involved in corneal allograft reactions and the immunologic nature of some constituent corneal cells.

[Sacrococcygeal teratomas: diagnosis and treatment].

The Authors present ten cases of sacro-coccygeal teratomas observed during the last ten years at the G. Gaslini Children Institute, Genova. Two cases was diagnosed in the pre-natal period. Diagnostic methods, histologic aspect and surgical treatments are discussed.

[Mesenteric cystic lymphangioma: a case report].

Intraabdominal lymphangiomas are rare. A five-year-old girl with abdominal lymphangiomas was successful treated by excision of the mass and resection of short segment of small bowel. We present a case report.
