[Carotid chemodectoma: a clinical case report and review of the literature].

Carotid chemodectoma (CC) is a very rare neoplasia. It originates from Type 1 main cells of the carotid body and affects both sexes almost in the same proportion (mostly in the fourth and fifth decades). It can be on a familiar basis (5-10%) and in these cases it is more frequently multicentric, being sporadically found in association with other paragangliomas. CC is a slow-growing neoplasia, locally aggressive and it can give metastases to regional lymph nodes and surrounding anatomical structures. Once surgically removed, it can reoccur in a small percentage of patients. Early diagnosis is of the utmost importance and surgeon's skill is fundamental as well as awareness of the real nature of the disease. Surgical treatment of CC is difficult and demanding, for the close relation of CC to the neighbouring neurovascular structures. The authors report a case of CC radically operated on. Clinical parameters are analysed as well, and Literature is reviewed.

[Epstein-Barr virus meningoencephalitis. Considerations on a very unusual clinical manifestation in adults].

The authors report a case of meningoencephalitis occurring as a complication in an adult suffering from infective mononucleosis. As well as the clinical and serological aspects of the pathology, the authors analyse the main and most recent etiopathogenetic hypotheses responsible for neurological symptoms, and pay special attention to the indirect namely immunomediated action of the virus.

[Systemic venous thrombolysis with rt-PA in arterial embolism of the legs in the elderly].

Lower limb arterial embolism with blockage of a proximal artery is still a significant use of mortality, especially in elderly subjects. The excellent results achieved in acute myocardial infarction by the use of recombinant tissue plasminogen activator (rt-PA) have prompted us to apply this agent systemically i.v. in elderly patients with probably cardiogenic arterial embolism of the lower limbs. Out of 14 patients in our care, 9 have been submitted to this treatment after careful selection, and in 7 total or partial recanalization of the vessel has been achieved thus saving the limb involved. It can therefore be concluded, even on the basis of the small number of cases, that in selected cases thrombolytic management is possible, while avoiding the hemorrhagic complications attending arterial thrombolysis. This treatment can be applied in hospitals lacking the resources of angiographic diagnostics. The mechanism of action of rt-PA is such that at the doses administered critical changes of blood clotting parameters are not to be feared. Venous systemic thrombolysis can be performed mainly in elderly subjects whose precarious conditions may be upset by manoeuvres involving bleeding, sophisticated diagnostic or surgery.

[Radiotherapy of cancer of the uterine cervix and successive appearance of new malignant growth in the irradiated field].

Among the late complications of high-dosage pelvic radiotherapy for cervical cancer, anorectal and bladder malignancies are sporadically reported in the literature. In this study the possible relationships of previous radiotherapy with late appearance of neoplasms are analyzed and in particular post-radiotherapy interval, dosage and type or irradiation to the cervical area. The mechanisms or neoplastic transformation or irradiated tissues are also studied. Three clinical cases or women irradiated in the past because or cervical cancer and recently treated at our surgical Division for the appearance or four new pelvic malignancies in the irradiated field (three rectal and one bladder cancer are presented. In conclusion, the possible cause-effect relationships between previous radiotherapy and subsequent appearance of neoplasms are analyzed as well as the potential therapeutical consequences or considering these women high-risk subjects. The usefulness or recruiting women with history or previous radiotherapy for cervical cancer in a strict follow-up program (ultrasonography, cytology and endoscopy) is suggested in order to make an early diagnosis of the new pelvic malignancy with a better possibility or treatment.

[Clinico-pathological study of microcarcinoma of the thyroid].

We have analysed the results of surgical treatment for microcarcinoma of the thyroid (MCT). In sixteen patient clinical and follow-up data were retrospectively evaluated during a 35.1-month follow-up. Thyroid hyperfunctional state us was present in two subjects. A single nodule was detected by echotomography in 11 patients, while multinodular diffuse goitre was revealed in 3 patients. In the last two subjects, thyroid gland appeared completely normal at ultrasonography, despite laterocervical lymph node metastases. Fine-needle aspiration biopsy was performed in 6 patients and its diagnostic accuracy was high (83,3%). MCT was classified as "incidental" in 12 patients and "occult" in the remaining 4 patients. Eight subjects underwent total thyroidectomy and 8 hemithyroidectomy plus isthmectomy. No postoperative complications were recorded. In 10 patients MCT histotype was papillar adenocarcinoma, in 5 was follicular adenocarcinoma and in the remaining case it was medullary carcinoma. Goitre was associated in 75% of the cases. Only in a patient disease progressed to death because of hematogenous metastases. In conclusion, we believe that incidental MCT is a low-grade malignancy with a benign biological behaviour. Occult MCT is a potentially lethal disease. We did not observe differences in the long-term results between different surgical treatments of MCT.

[Plasma prolactin in childhood paroxysmal disorders].

The Authors discuss the role of plasma prolactin concentration as a marker to discriminate epileptic and non epileptic seizures. In 40 children was determined prolactin during the first seizure (30 feverish and 10 non feverish seizures). The analysis of the results shows that during non feverish seizures prolactin concentration in higher, but we don't know if the same seizures are the beginning of epilepsy. So plasma prolactin concentration is a parameter of moderate interest in diagnosis and prognosis in childhood paroxismal disorders.

[Role of p53 gene in colorectal neoplasms].

A prospective study on the role of the p53 gene in sporadic colorectal neoplasms is presented and the level of mutant p53 protein was measured in the tissue removed during colonoscopy from: patients previously operated for colorectal malignant neoplasms, patients with active neoplasms, first degree relatives and during regular checks. 72% of patients with an active tumour showed a positive p53 and 38% in follow-up checks. Longer follow-up periods and a major number of patients are necessary to assess the prognostic importance of the p53 protein.

[Intensive hospital monitoring of adverse reactions to benzodiazepines and neuroleptic agents].

BACKGROUND: The main aims of the programme were to highlight the incidence of adverse reactions to the drugs monitored and to define the risk/benefit ratio taking account of the main physiological and physiopathological variations of patients. This paper reports the results of the programme regarding to adverse effects correlated to the use of some psychiatric drugs (benzodiazepines and neuroleptics). METHODS: A total of 73 records were compiled for 64 patients treated with benzodiazepines and/or neuroleptics. RESULTS: A very high mean incidence of adverse events was documented (48%) without any severe undesirable effects. 92% of patients treated with neuroleptics reported adverse events. Haloperidol, which caused adverse effects in 80% of patients, revealed mild or moderate forms of parkinsonism (15%), spasm (15%), rigidity (10%), akathisia (5%), reversible postural hypotension (10%), temporary reduction of the visual field (10%), delayed menstrual flow (5%), xerostomia (10%), excessive sweating (10%) and sialorrhea (10%). All the patients treated with clozapine showed adverse effects including postural hypotension (29%), persistent tachycardia (14%), sialorrhea (29%), excessive sweating (14%) and akathisia (14%). Spasms (25%), rigidity (25%) and akathisia (25%) were correlated to the use of clothiapine, whereas postural hypotension was referred to clopenthixol. 44% of patients treated with benzodiazepines showed undesired effects. 20% of those taking chlordemethyldiazepam showed somnolence (33%), sedation (22%) and dysar-thria (44%). Prolonged sedation was observed in 30% of all patients treated with lorazepam. Prazepam was correlated with adverse effects in 75% of cases. No adverse event was documented with bromazepam. CONCLUSIONS: A higher incidence of adverse events was documented than literature data. Further periods of intensive monitoring will be required to obtain a greater quantity of data from the intensive monitoring of adverse events through the MIO '97 programme.
