Morbus Bowen. A description of the disease in 617 patients.

Between 1943 and 1983 a total of 617 patients were diagnosed to have morbus Bowen (m.B.). Eighty percent of the patients were over 60 years old at the time of diagnosis. Three-fourth had their disease on sun-exposed areas (head, neck and hands). Almost 1/4 had concomitant basal cell epithelioma (c.b.) and 1/10 squamous cell carcinoma (c.s.). Approximately 1/5 had multiple m.B. tumours. Effective therapy was curettage followed by cauterization, excision, or X-ray.

Bowen's disease and internal malignant diseases. A study of 581 patients.

In a retrospective study of 581 patients with a diagnosis of Bowen's disease (BD) treated over a 40-year period, we traced patient records to identify later diagnoses of nonskin cancer. Fifty patients had nonskin cancer, as against an expected number of 40, but this difference was not significant. The lack of association was equally true for BD on sun-exposed and non-sun-exposed skin. Our findings support the view that BD is not a skin marker for internal malignant disease.

Dapsone in the treatment of cutaneous lupus erythematosus.

Thirty-three patients with chronic discoid lupus erythematosus (DLE) who had received dapsone for 1-27 months were reviewed. The dapsone treatment gave excellent results in 8 patients (24%), some effect in 8 patients (24%), while no response was seen in 17 patients (52%). Of the patients who had earlier been treated with hydroxychloroquine (HCQ), 76% responded excellently to the treatment. Of 6 patients who received a combined dapsone and HCQ therapy, 2 responded well. Six patients (18%) terminated the dapsone treatment because of side effects, none of which were serious. It is concluded that dapsone might be an alternative or a supplement to HCQ in the treatment of DLE, when the latter causes adverse reactions or fails to be effective. However, we find that HCQ remains the drug of choice when systemic treatment is needed.

Abnormal lymphocyte response to u.v. radiation in multiple skin cancer.

The lymphocyte response to u.v. radiation (254 nm) was investigated by two different methods in 29 unselected patients with multiple epidermal cancer. The u.v.-induced DNA synthesis was determined as the increase in incorporation of [3H]thymidine in irradiated cells compared with nonirradiated cells after incubation for 2 h. The u.v. tolerance was measured as the u.v. dose necessary for 50% reduction in phytohemagglutinin-stimulated lymphocyte proliferation. Patients with both squamous cell differentiated tumors and basal cell carcinomas had very high u.v.-induced DNA synthesis values (p less than 0.01, when compared with patients with basal cell carcinoma only and p less than 0.005, when compared with controls). The u.v. tolerance in patient lymphocytes was considerably lower than in control lymphocytes (p less than 0.001), with the lowest values occurring in patients with clinical sun intolerance (p = 0.05, when compared with the remaining patients). These investigations may be of predictive value in skin carcinogenesis.

15 years' experience with treatment of basal cell carcinomas of the skin with curettage.

At the Department of Dermatology in the Finsen Institute we have since the beginning of the 1960's been especially interested in the treatment of skin tumors. Curettage is used in combination with a number of other treatment modalities and, therefore, it has been important to assess the value of curettage as the only therapy of basal cell carcinomas (b.c.c.). Gradually, curettage has come to be the normal treatment of b.c.c. of the skin in very large patient materials. The curettage is supplemented with X-ray therapy in cases where the demarcation in the depth is uncertain, while electrocauterization is used if the limits between a tumor and the surrounding skin are not sharp. 3 patient groups have been followed at regular intervals, and the results of these are described and discussed. The groups comprise firstly 525 patients with solitary tumors, secondly a special group consisting of 180 b.c.c. of the eyelid region and thirdly a group of 673 patients with multiple b.c.c. Considering the benign character of the tumors, their frequent localization on the face, the low age of many patients, and especially the nice results from the cosmetic point of view, it is concluded that curettage as the only treatment of b.c.c. ought to be a part of the therapeutic routine in a modern Department of Dermatology.

The treatment of psoriasis with 0.1% domoprednate (a D-homocorticosteroid) and 0.1% betamethasone valerate ointment. A double-blind, randomized trial.

In 85 patients with psoriasis vulgaris the efficacy and tolerance of domoprednate and betamethasone valerate were studied in a double-blind, randomized trial lasting 4 weeks. Complete or satisfactory remission was obtained in 36% in the domoprednate group and in 53% in the betamethasone group. This difference is not statistically significant (p greater than 0.10). 3 patients on each treatment experienced some local irritation. No laboratory abnormalities were found during the study.

Patients' acceptance of etretinate therapy. A retrospective survey of long-term etretinate therapy in chronic keratotic and pustular skin diseases.

From January 1976 to April 1982 etretinate was used to treat patients with morbus Darier (25 patients), pustulosis palmo-plantaris (72 patients), psoriasis (79 patients), and hyperkeratotic eczema of hands and feet (eczema keratoticum; 41 patients). Dosage of etretinate ranged between 0.5 and 1.0 mg/kg/day. In a retrospective survey at April 1, 1982, we observed a beneficial effect of the drug in all patients with morbus Darier, in 85% of patients with pustulosis palmo-plantaris, in 69% with psoriasis, and in 93% with eczema keratoticum. Many patients experienced side effects. Within 9 months more than half of the patients had stopped treatment primarily due to side effects, except patients with morbus Darier, whose median duration of treatment was 34 months with a range from 7 to 60 months. We will recommend that etretinate is used during short-term or intermittent courses of therapy, except in patients with morbus Darier, where a low dosage of etretinate is able to give a beneficial clinical effect.

Androgen Antagonists

A randomized trial of etretinate (Tigason) in palmoplantar pustulosis.

5 patients with palmoplantar pustulosis (PPP) were randomized to 8 weeks of daily treatment with either oral etretinate, 1 mg/kg b.w. or placebo. Good or moderate effect was obtained in 18 or 20 patients on etretinate compared o 6 of 21 patients on placebo (p less than 0.001). Etretinate proved to be significantly superior to placebo with regard to influence on the individual symptoms and signs of pustulosis. All patients on etretinate experienced some side effects from the mucous membranes, but they were generally mild. Treatment was discontinued after 4 weeks in 3 patients for reasons unrelated to treatment, in 4 for lack of effect (all on placebo) and in 2 for side effects (both or etretinate). Etretinate is a good alternative to other systemic treatments of PPP.

Chondrodermatitis nodularis chronica helicis treated with curettage and electrocauterization: follow-up of a 15-year material.

During the 15-year period from 1965 to 1979 a total of 142 cases of chondrodermatitis nodularis chronica helicis were diagnosed. 32% were women, a higher proportion than in earlier materials. The treatment was principally curettage followed by electrocauterization. 78 patients were re-examined after an average lag of 7.1 years. The relapse rate was 31%. This simple surgical technique seems equal to the more elaborate procedures in respect of recurrence rate and more satisfactory from a cosmetic point of view.

Cyproterone acetate in the treatment of acne vulgaris in adult females.

22 adult females with therapy-resistant acne vulgaris were treated for 12 months with Diane, a drug containing cyproterone acetate and ethinylestradiol. Treatment was withdrawn in 7 patients because of side-effects of lack of of effect. In the remaining 15 patients, the treatment had extremely promising results, from 70 to 90% improvement of the acne. In a remarkably high number of patients, the androgen production, measured by the urinary excretion of fractional 17-ketosteroids, was elevated. None of these patients had signs of endocrinological diseases, in particular no cases of hirsutism of Stein-Leventhal syndrome were found. The current concept of the course of acne is that the conversion in the skin of testosterone to dehydrotestosterone is increased. The finding of an elevated urinary excretion of androgenic substances in this group of acne patients indicates that the pathogenesis is far more complicated.
