[Intra-operative EEG surveillance during carotid artery desobliteration].

In 35 patients intraoperative EEG-monitoring during carotid desobliteration under general anesthesia was performed. 15 of these patients had preoperatively focal EEG-changes, coinciding frequently with a cerebral infarct or with carotid changes also contralateral to the side of the desobliteration. 18 patients had during the carotid clamping EEG-changes (slowing of frequency and decrease in amplitude) or a worsening of preoperative changes. 15 of these patients had a complete recovery of these changes after unclamping. The EEG changes do occur more frequent in patients who had a cerebral infarct or bilateral carotid changes preoperatively. Since the EEG-changes during the carotid clamping do signal a critical lowering of the brain perfusion, an ischemic etiology has to be assumed. The use of a shunt during the procedure to prevent irreversible ischemic damage seems therefore to be justified.

[Peroperative evoked potentials in neurosurgery].

The application of somatosensory evoked potentials in different fields of neurosurgical procedures are presented. SSEP are used for the intra-operative control in aneurysm surgery, during a.v. malformation surgery specially in the posterior fossa and in the operative treatment of large acoustic neurinomas. Finally the authors describe their technic of anterior spinal cord stimulation in transthoracic approach in spinal cord traumatic lesions. In this patients proven intra-operative neuronal continuity allows good prognosis of functional recovery.

[The value of visually evoked response in diagnosis of multiple sclerosis ].

We investigated the reliability of visually evoked responses (VER) in 56 cases in diagnosing multiple sclerosis. We found 100% pathological VER in proved cases and 71% in probable/possible cases. Patients with a duration of illness less than one year had in 80% pathological VER, whereas in other conditions only 17% were pathological. Structural changes are usually found in definite multiple sclerosis patients with a disease progress lasting more than 1 year. Pathological VER are an important diagnostic aid in multiple sclerosis, especially in doubtful cases, whereas structural investigations are primarily useful in the exclusion of other conditions.

[The anterior tibial syndrome (author's transl)].

The anterior tibial syndrome is caused by primary or secondary ischemia of the tibial muscles group. Acute onset of intensive pain, swelling of the muscles and reddening of the skin are the clinical features of the syndrome. The muscles enzymes are raised. The definite diagnosis is established by the typical "silent-EMG". Surgical decompression within 24 hours after onset of the symptoms is of essential prognostic importance.

[Value of somatosensory evoked potentials in diagnosis of transverse lesions of the cord (author's transl)].

3 cases with incomplete spinal transection are presented here and the importance of the technique of somatosensory evoked potentials is demonstrated. This method is apt to verify extension and localisation of somatosensory pathway lesions. There are no risks for the patient and the examination can be repeated any time. As a functional diagnostic procedure it does not compete with radiological technique, but is of additional diagnostic value.

[The locked-in-syndrome (author's transl)].

On the basis of two cases some problems of the locked-in-syndrome are discussed. In most cases the cause of the syndrome is an infarct in the ventral part of the pons due to an occlusion of the vertebral or basilar artery. There is a total loss of movement exept for the movement of the eyes in all directions and the eye-closing. Although vigilance is almost not disturbed the complete loss of movement often results in a false diagnosis of cerebral coma. The EEG is an important diagnostic aid.

[Comments on epileptic psychosis (two case reports) (author's transl)].

Nosological interpretation and therapy of epileptic psychosis is difficult, since psychotic and epileptic activities are of quite different time relationships. Case reports of two patients suffering from psychosis and showing generalized spike-wave paroxysms, without evidence of clinical seizures, are presented. The problems of diagnosing epileptic psychosis under these circumstances and the therapeutic approach are discussed.

[Advantages of the initial therapy of acute schizophrenia with large doses of droperidol/A comparative study (author's transl)].

The treatment of acute schizophrenic disorders with large doses of droperidol was compared to clozapine and clopenthixol in a special study. Apart from the clinical protocol, psychometric test for the assessment of social behaviour and thought disorders were used. The advantages of the treatment with droperidol, especially in highly disturbed acute schizophrenic patients, are shown and discussed by means of the results of psychological tests and clinical experience.

[Tiapride in the treatment of irreversible tardive dyskinesia--initial experiences].

Twelve patients on a chronic psychiatric ward with NL-induced permanent TD were treated with 300 mg tiapride daily for at least 3 weeks. Clinical assessment of TD was performed with AIM-Scale before and after 3 weeks' therapy, showing a significant effect of treatment. These preliminary findings were controlled in a blind study, showing again a significant therapeutic effect of tiapride.
