[Rhabdomyosarcoma of soft palate. A case on purpose].

The rabdomiosarcoma (RMS) are infrequent tumors. They are principally described in infancy and located in 35% of the cases in head and neck. The nasopharynx localisation is relatively rare, being in these cases the tongue, palate and oral mucosa the preferent places of establishment. Classically the patient presented very low standard healing with surgery and radiotherapy. The introduction in the middle 70 of systematic chimiotherapy as complementary treatment, improved the survival rate in large scale. In this article the case of an adolescent patient, who presented a RMS at the level of the soft palate, the diagnostic procedure and the therapeutic decision adopted, after revision of the last studies at this respect, are described.

[Malignant external otitis. A pattern to bear always in mind].

Malignant external otitis (M.E.O.) is a severe and rare affection, consisting in an infection of the external auditory canal (EAC) whit extension and compromising the structures of the skull base, produced mainly by Ps. Aeruginosa in patients affected by diabetes type II of long evolution and or deterioration of the immunity system. We present a case of a patient affected by this pathology, commenting the caracteristics of this disease, how we arrived to the diagnosis, and the evolution of the case with an exclusively medical treatment.

[Metastases in both internal auditive meatus of nasosinusal melanoma].

Primary malignant melanoma of the nasal and paranasal sinusses is rare and represent 3% of malignant nasosinusal tumors. The prognosis of this tumor is poor. The evolution is unpredictable; there is very commonly a series of local recurrences which may be well controlled, followed by remote metastases remaining long quiescent. There are no statistical difference in local control or survival between patients receiving surgery alone and those receiving surgery and radiotherapy. Chemotherapy is used for metastatic forms. We present a case of a patient with a primary malignant melanoma of the nasal mucosa. Tree years after diagnosis we found intracranial metastases in left and right auditive internal meatus.

[Big profound cervical abscess in a diabetic patient].

With antibiotics and an increasing dental care and hygiene, the profound neck spaces infections have decreased their incidence. Nevertheless we can found them in the clinical practice, and they present a big morbidity and severe complications. We present in this paper a case of profound cervical abscess in a diabetic patient, discussing the clinical and therapeutics aspects.

[Brain abscess secondary to rhinosinusitis. Therapeutical modalities. Exposition of one case].

Brain abscess (BA) as complication of sinusitis represent around 3% of the cases. Other suppurative intracraneal complications such as epidural abscess, subdural empyema and meningitis are more common than BA. The frontal and ethmoid sinuses form an integral part of anterior and to a lesser extent middle skull base to which the dura is intimately related. Thrombophlebitis of veins associated with paranasal sinuses is considered to be the main route of intracraneal spread of infection. However, osteitis is an uncommon mechanism that typically involves frontal sinus. Chronic sinusitis more than fulminant course of acute sinusitis is often responsible for BA. Epidemiology of BA has changed with the increasing incidence of this infection in inmunocompromised patient and the decreasing incidence related to sinus infection. Mortality rate of BA were around 35-55% before CT scan era and with surgical excision as the rule of surgical treatment.

[Giant cholesteatoma of the external ear canal after myringoplasty].

Cholesteatoma are specially localized in middle ear and mastoid process, although sometimes is the external ear canal which shows the disorder. Usually its a appearance is spontaneous. We report a rare case of gigantic cholesteatoma which origin was the previous otologic surgery done because of the inclusion of a bit of epithelial tissue inside the surgical incision. Similar outcomes justify periodical checking after ear surgery with outlining a tympanomeatal flap.

[Otogenic cerebral abscess: a persistent problem].

A young man with otitis media and cholesteatoma of the left ear developed secondary cerebral abscess. The clinical debut was non-specific, with headache, mild fever, and mild persistent otalgia in spite of early antibiotic treatment. Studies revealed a cerebral abscess, so ENT surgery in collaboration with the neurosurgery department was decided. This case illustrates that clinical manifestations in such cases can be mild and highlights the need to exclude this type of serious pathology.

[Serum immunoglobulin levels in tonsillectomized patients. An unsolved mystery].

In a prospective study of 117 patients scheduled for tonsillectomy, with or without adenoidectomy, immunoglobulin levels were measured preoperatively and 1 to 6 months after intervention. Small deficiencies in IgG levels were observed, but levels were within normal limits. We have attempted to clarify the controversial topic of the immunological disturbances that may appear after this common intervention.
