Fallopian tube patency demonstrated at ultrasonography.

Fallopian tube patency was assessed in 24 infertile patients by hysterosalpingography (HSG) and ultrasonographic examination of the pouch of Douglas following transcervical injection of a sterile isotonic solution of NaCl. The presence of fluid in the pouch, after the injection, was taken to indicate tubal patency. The results of the HSG and the ultrasonographic diagnosis as to the presence of at least unilateral tubal patency were concordant in 21 patients. Pitfalls consisted of fluid accumulation in periadnexal adhesions, edema in the bowel wall, and spill of the injected saline into a large hydrosalpinx. Ultrasonography is advocated as the initial examination in assessing infertility in young women. If tubal patency is demonstrated, the patient should be recommended a six month trial period, to become pregnant, before invasive procedures are initiated.

Radiologic aspects of calcaneal fractures in childhood and adolescence.

One hundred and forty-three calcaneal fractures in 140 patients under the age of 19 years were reviewed. Of the total number, 75 per cent were in males. Eighty fractures were seen in females under 13 years of age and in males under 16 years of age. Fifteen (19%) of these fractures were initially not recognized. Four projections were available in the majority of the cases, and were reviewed: lateral, axial, straight dorsoplantar and oblique dorsoplantar views. The various types of fractures had different courses and localizations. The optimum demonstration of a fracture in a certain projection will therefore be dependent on the type of fracture present. It is clinically difficult to foresee a specific type of calcaneal fracture, and consequently it is recommended that all four views should be obtained routinely following trauma to the calcaneal region in patients of the pertinent age groups.

Inadvertently performed hysterosalpingography during early pregnancy.

A review of 6,225 hysterosalpingographies disclosed four cases of inadvertently performed examinations during early pregnancy. These radiologic misadventures were in three cases performed because of oligomenorrhoea and in one case because of a misjudged decidual bleeding. The radiologic findings and the outcome of these pregnancies are presented. Precautions to avoid this radiographic error are discussed.

Therapeutic value of hysterosalpingography with lipiodol ultra fluid.

Hysterosalpingography (HSG) with lipiodol ultra fluid was performed in 294 infertile women with a normal ovulatory temperature curve and at least two years of infertility and a partner with normal sperm. In 21 per cent, pregnancy occurred within 6 months after the examination, and about one third of the women with a normal finding or with intraperitoneal adhesions at HSG conceived. The pregnancy rate was especially high in the first two cycles after HSG. The spontaneous pregnancy rate was 8 per cent, and the difference between this and the total number of pregnancies must be attributed to a therapeutic effect of the procedure. Previous pelvic inflammatory disease was present in 40 per cent of those who did not become pregnant, while only 11 per cent of those who conceived had previous inflammation. Of the women without previous gynecologic disease 30 per cent conceived.

Granulocyte chemotaxis. Chemoattractive properties of a synthetic tripeptide are inhibited by radiographic contrast media.

The under agarose method for evaluation of leucocyte chemotaxis was used to investigate the effect of radiographic contrast media (CM) on granulocyte locomotion. The CM tested had no chemoattractive properties. CM inhibited N-fmlp, a synthetic formylated Met-tripeptide, which is a strong chemotactic agent and an analogue to chemotatic peptides produced by bacteria. The inhibition of N-fmlp was most pronounced for diatrizoate. Equiosmolal saline was not so inhibitive. Therefore, some part of the inhibition was caused by factor(s) other than hyperosmolality inherent in the CM solution.

Influence of radiographic contrast media on phagocytosis.

To evaluate the influence of radiographic contrast media (CM) on human polymorphonuclear leucocytes (PML), the ability of these cells to ingest latex particles after in vitro exposure to five different radiographic contrast media was investigated. All CM inhibited the phagocytic properties of PML. The inhibition was dose dependent. The inhibitory effect was partly due to hyperosmolality but CM specific inhibition was also evident.
