[The "open abdomen" method of treatment of postoperative peritonitis].

The authors stress the therapeutical difficulties encountered in patients with severe postoperative peritonitis, necessitating multiple interventions for the solution of viscero-peritoneal problems, and of the abdominal wall. The authors' personal experience is presented with the method of the "opened abdomen", which is applied abroad under the improper name of "coeliostomy". The authors describe their procedure, and also describe a complementary method, of the socalled "semi-open abdomen", which is in fact a narrowing of the surgical wound with threads of inxodable wire. Four of six patients hospitalized in an extremely serious condition have been recovered.

[Duodenal biliary ileus].

A clinical observation is presented, of a patient that underwent emergency surgery for biliary ileus located in the duodenum. The particularity of the case consisted in the presence of a double bilio-digestive fistula: between the cholecyst and the stomach, and between the cholecyst and the duodenum. The rarity and the clinical characteristics of this syndrome described by Bouveret are stressed. The aspect and the extension of the lesions found during surgery have made necessary to apply an exceptional surgical solution which consisted in cholecystectomy and antrectomy with gastro-jejunal anastomosis, excluding the sutured duodenum from the digestive pathway.

Medullosclerosis, contralateral adrenalectomy and splanchnicosympathectomy in severe essential arterial hypertension and thromboangiitis. Part II.

Operations were performed in 58 patients with severe essential hypertension and in 105 patients with Buerger's thromboangiitis. No postoperative deaths were recorded. The mean adrenalinemia value decreased by 70.91% and that of noradrenalinemia by 10.34%. Systolic pressure decreased by about 30--50 mm Hg and the diastolic pressure by 10--30 mm Hg. The delayed results (1--20 years) were good in 62% of the patients with IIIrd-degree eye lesions and in 17% of the patients in the organofunctional stage II, in 17% of the patients in stage II--III of restricted necrosis, and in 4% of the patients in gangrenous stage III.

[The clinico-evolutionary and therapeutic aspects in diastatic perforation of the cecum].

We describe five cases of diastatic perforation of cecum due to colonic cancer obstruction with secondary diffuse peritonitis, all in elderly patients with many organics dysfunctions and, most important a late arrival for hospitalisation (over 48 h). The surgical treatment aimed for the perforation and the obstruction has permitted the realization--in emergency--of: cecostomy on site of perforation, with multiple peritoneal drainage; subtotal colectomy with ileosigmoid anastomosis; cecum perforation sutures, with left colostomy, opened per primam; cecostomy on site of perforation with left colostomy, drainage. We underline the severe postoperative evolution, with only one survival be cause of the fast pace evolution of toxico-septic shock by pyostercoral peritonitis due to perforation.
