[Indeterminate form of Chagas' disease. A polymorphic disease].

PURPOSE: To evaluate the association between the indeterminate form of Chagas' disease and abnormalities in non-invasive cardiovascular propedeutics. PATIENTS AND METHODS: Twenty-two patients in the indeterminate form of Chagas' disease, 17 female, with ages between 25 and 50 years were submitted to studies of: vectorcardiography, echocardiography, stress test, dynamic electrocardiography and scintigraphic studies (scintigraphic ventriculography and cardiac mapping with pyrophosphate of technetium), in a period of two months after first consultation. RESULTS: The analysis shows the percentual of altered exams: vectorcardiogram--68.2% (conduction disturbance--22.7%); echocardiogram--15.8% (apical hypokinesia); cardiac mapping 61.8%; scintigraphic ventriculography--68.2%; stress test--72.2% (reduced delta PS--50%, ventricular arrhythmia--50%); and dynamic electrocardiographic--50% (ventricular arrhythmias). We could observe also that the great majority of the patients has more than three exams altered (63.6%). The study of relation between methods, shows only association between reduced delta PS and ventricular arrhythmia, with concordant results in 72.7%. CONCLUSION: Indeterminate form of Chagas' disease appears to be a really chronic form, may be of attenuate aggressive potential, with indeterminate evolution and with polymorphic clinical exteriorization seeing only through invasive and non invasive propedeutic techniques.

[Mitral valve prolapse in infective endocarditis. Incidence and characteristics].

UNLABELLED: PURPOSE valve prolapse (MVP) cases, complicated with infective endocarditis (IE), seen at a cardiologic reference hospital. METHODS: The MVP cases were diagnosed within a series of 276 cases of IE without previous cardiac surgery with clinical-echocardiographic criteria. We emphasised in our study; a) incidence; b) age and sex distribution; c) risk factors for IE; d) clinical-bacteriological data; e) echocardiographic features; f) therapeutic aspects; f) evolution and complications. RESULTS: a) We diagnosed 40 (14.4%) cases of MVP; b) mean age of 40.6 years, with 29 men (72.5%); c) dental procedures without prophylactic antibiotic-therapy in seven patients (17.5%), mitral regurgitation in 33 (82.5%), mitral valve redundancy in six (15%); d) fever was found in all patients (100%), cachexia was diagnosed in eight (20%), polyarthritis in six (15%) and congestive heart failure in six (15%). A systolic murmur of mitral regurgitation was found in all the patients and a nonejective click was encountered in four (10%), a Streptococcus was cultivated in 30 (75%), Staphylococcus in three (7.5%) Haemophilus sp in three (7.5%) Peptostreptococcus products in one (2.5%) and negative in three (7.5%) patients; e) echocardiographic signs of vegetation in 23 (57.5%), mitral regurgitation in 33 (82.5%), rupture of mitral chordae in 11 (27.5%) and mitral redundance in six (15.2%); f) 37 (92.5%) patients were medically treated and three (7.5%) required surgery; g) we registered in the hospital evolution: stroke in 10 (25%) and noncerebral mycotic aneurysm in 2 (5%) cases, two patients died (5%), one due to a hemorrhagic complication after surgery of bioprosthesis implantation and another of rupture of a cerebral mycotic aneurysm. CONCLUSION: The MVP was frequent in the population of patients with IE with of patients with mitral regurgitation, and male with mean age greater than the other cases. We observed low incidence of surgical need (7.5%) and lethal outcome (5%) but frequent (25%) neurological complications.

[Predictive value of programmed ventricular stimulation after myocardial infarction].

PURPOSE: To find out whether is there a relation between electrophysiology laboratory ventricular arrhythmias through programmed ventricular stimulation, and the occurrence of relevant clinical events, particularly, sudden death, in patients victims of first episode of myocardial infarction. PATIENTS AND METHODS: Twenty-seven patients (all males) who suffered non-complicated first myocardial infarction, with age varying from 47 to 70 (mean 54 +/- 6) years were prospectively and consecutively studied. Upon consent, patients were at first submitted to conventional cinecoronaryography and to programmed ventricular stimulation utilizing the S2, S3, S4 protocol, 30 days after infarction. Moreover, patients were submitted to continuous electrocardiographic (Holter system), exercise test, and radioisotopic studies for left ventricle ejection fraction (phase I). The last three studies were subsequently repeated at 6 and 12 months (phases II and III), respectively. RESULTS: All patients had ventricular arrhythmia induced, and according to the kind of response, patients were grouped into: group I--repetitive ventricular response, RVR, nine (33.5%) patients; group II--non sustained ventricular tachycardia. NSVT, eight (29.5%) patients; group III--sustained ventricular tachycardia SVT, ten (37%) patients. The patients of three groups, when submitted to electrocardiographic exercise test and Holter system studies, revealed complex arrhythmias in all phases. One patient of group II suddenly died at home, and another was injured with syncope, whereas in group III, two suddenly died, one at home and the other at the admission to the hospital, after an acute myocardial infarction. All patients but one of group I, who had a non-fatal reinfarction, showed preserved ventricular function. CONCLUSION: Holter system and electrocardiographic exercises test revealed complex dysrhythmia in the tree phases of the study, independently of the induced response; C--Holter system and electrocardiographic exercise test studies revealed no relation. between sudden death and the kind of ventricular induced response or complex arrhythmia. Ventricular arrhythmia induced with S2, S3 and S4 protocol in patients with preserved ventricular function seems to indicate results without predictive value for cardiac sudden death.

[Infective endocarditis: analysis of 300 episodes].

PURPOSE: Study of clinical features and etiologic agents, treatment and mortality of patients with infective endocarditis (IE). PATIENTS AND METHODS: 300 episodes of IE occurring in 288 patients, ages ranged between 0.2 and 78 (mean 30.76) years; 185 (62%) episodes occurred in males. RESULTS: a) etiologic agents: viridans group streptococci in 93 (31%) episodes, enterococci en 21 (7%), group D-non enterococci in 19 (6%) (13 S. bovis), other streptococci in 14 (5%), Staphylococcus aureus in 59 (20%), Staphylococcus epidermidis in 14 (5%), gram-negative bacteria in 16 (5%), gram-positive bacteria other than streptococci and staphylococci in 8 (3%), fungi in 4 (1%). The etiologic agents were not identified in 52 (17%) episodes; b) underlying cardiac diseases: valvular heart disease in 119 (40%) episodes, congenital heart disease in 37 (12%), prosthetic heart valves in 69 (23%), other heart diseases in 6 (2%). There was no evidence of previous heart disease in 69 (23%); c) treatment: surgical treatment was undertaken in 102 (34%) episodes. The frequency of surgical treatment in relation to the etiologic agents ranged between 1% (non-group D streptococci) and 62% (negative blood cultures). The frequency of operation in relation to underlying heart disease ranged between 17% (other heart diseases), 19% (congenital heart disease) and 54% (prosthetic heart valve); d) mortality: 78 (26%) patients died, 56 (28%) of the 198 submitted to medical treatment and 22 (21%) of the 102 submitted also to surgical treatment. The mortality in the different groups of etiologic agents ranged between 5% (non-group D streptococci) and 62% (gram-positive bacteria other than streptococci and staphylococci); in relation to the underlying heart disease, the mortality was 16% in patients with other heart disease, 19% in valvular heart disease patients, 21% in patients with congenital heart disease, 23% in patients without known heart disease and 43% in patients with prosthetic heart valves. CONCLUSION: The mortality associated with IE remains still high in spite of modern treatment; the mortality is different in relation to the different etiologic agents and in relation to the cardiac status before the IE.

[Endomyocardial biopsy and myocardial imaging with 67-gallium in the diagnosis of active myocarditis in children with dilated myocardiopathy].

PURPOSE: This study was designed to compare 67Ga imaging and endomyocardial biopsy (EB) in children with severe dilated cardiomyopathy (DC), as well as to evaluate the results in a group of patients with active myocarditis submitted to immunosuppressive therapy. PATIENTS AND METHODS: Forty-four pediatric patients with severe DC were studied. Twenty males and 24 females from 10 months to 15 year old (median = 2.6 years). All patients were submitted to a protocol including 67Ga uptake and EB. In patients submitted to immunosuppressive therapy these procedures were repeated after six months. RESULTS: In 32 patients (72.7%) the EB revealed presence of inflammatory process; 21 (65.6%) of these had a positive 67Ga uptake and 11 (34.4%) negative. Twelve patients with no evidence of inflammatory process in the EB, nine (75%) presented negative 67Ga uptake. However, when the intensity of myocardial inflammatory was analysed (mild, moderate and severe) and correlated with 67Ga imaging, was observed that the majority of patients with negative 67Ga uptake (11 patients) had mild inflammatory infiltration (nine patients). In this way the 67Ga uptake demonstrated a good correlation in the diagnosis of moderate and severe inflammatory process in children with DC. This is important because the use of immunosuppressive drugs is indicated only in these group. CONCLUSION: The 67Ga imaging is a noninvasive diagnostic method with a good sensitivity to the diagnostic method with a good sensitivity to the diagnosis of AM in children with severe DC, demonstrating to be very useful in the therapeutic approach.

[Late potentials in patients with myocardial infarction subjected to coronary thrombolysis].

PURPOSE: Analyse the behavior of the late potentials (LP) in patients submitted to thrombolysis with success. MATERIAL AND METHODS: Thirty-five patients with acute myocardial infarction, 32 (91.4%) male with ages varying from 33 to 68 (mean 52.6). Thrombolysis was obtained during cinecoronarography with intravenous infusion "in bolus" of doses of 50 mg, 60 mg and 70 mg of rt-PA, with a new bolus of 30 mg at 60 minutes after the procedure. A new angiographic study was performed 12-48 hours late. The high resolution ECG was taken with the ART system model 1200 EPX, before, after and 72 hours later. The presence of electrical activity in the last 40 ms of the QRS complex with less than 20 mu volts in amplitude and more than 35 ms in duration characterized the LP. RESULTS: LP was observed in 16 (46%) patients during the phase of arterial occlusion. LP was recorded only in 23% after recanalization and in 26% when the infarcted related artery (IRA) remain opened. The reduction in the LP was 43.5%. Only one patient with LP had threatening arrhythmia. CONCLUSION: In patients with demonstrated thrombotic occlusion of the IRA, the recanalization diminished the incidence of LP in 43.5% of the patients. Further studies are necessary to achieve the exact clinical importance of these findings.

[Incidence of dyslipidemia and its relation to coronary artery disease in Brazilian populations].

PURPOSE: To describe the incidence of dyslipidemia and its relation to coronary artery disease (CAD) in two brazilian populations. PATIENTS AND METHODS: 1.261 subjects were analyzed; 674, whose mean age was 53 years, came from a cardiology private office (population A) and 587, whose mean age was 37 years, were employees of an engineering company (population B). Cholesterol was determined by the enzymatic method Chod-pap and LDL calculated by the Friedwald's formula. RESULTS: CAD was present in 35.1% of population A and in 1.4% of population B. In population A total cholesterol and LDL were above the expected, in all age groups. About 70% of these patients had cholesterol greater than 200 mg% and LDL greater than 130 mg%, which are considered desirable levels; 28% of women and 44% of men had HDL less than 35 mg%. Conversely, in population B total cholesterol and LDL were slightly above normal only in patients greater than 45 years old. In addition, about 65% of population had cholesterol less than 200 mg% and LDL less than 130 mg%; even so, 28% of women and 48% of men had HDL less than 35 mg%. In population A, there were no significant differences between the percentages of patients with and without CAD, over a wide range of cholesterol values. Similarly, levels of LDL, HDL and triglycerides as well as the incidence of hypertension, smoking and diabetes did not differ between patients with and without CAD. CONCLUSION: There is a high incidence of dyslipidemia in these brazilian populations, especially among patients from private office. Further, the isolated measurement of lipid fractions does not discriminate between patients with and without DAC, suggesting that other factors are involved in the genesis of the disease.

[Endocarditis caused by Streptococcus bovis and colorectal neoplasms].

PURPOSE: To evaluate the association of Streptococcus bovis endocarditis with large bowel neoplasms. PATIENTS AND METHOD: Twenty episodes (19 patients) of Streptococcus bovis endocarditis (5.05%) selected from 396 episodes of endocarditis followed up from 1978 to 1990. The mean age was 57.68 years, 16 (84.21%) were males. Previous heart disease was not known in nine (45%). Previous manipulation that might induce bacteremia was identified in three patients. Symptoms related to neoplasia of the digestive system were not informed. The mean time of follow up (17 cases) was 33.76 months, standard deviation 27.37 months. Sixteen patients were submitted to colonic evaluation (barium enema in 11, colonoscopy in 5 patients, and both were performed in 5 patients). RESULTS: Bowel neoplasias were diagnosed in 12 (75%) of 16 patients submitted to colonic evaluation, one colonic adenocarcinoma in one (8.33%), patient, histologic benign polyps in eight (66.6%). Two patients are waiting for colonoscopic resection. In one patient the polyp was lost after colonoscopic resection. CONCLUSION: The high incidence of large bowel neoplasia in patients with Streptococcus bovis endocarditis indicates that evaluation of the large bowel must be performed on in patients with S. bovis endocarditis in order to diagnose neoplasias, even in the absence of symptoms of bowel disease.

[Pericardial disease erroneously diagnosed as endomyocardial fibrosis. Report of 2 cases].

Two patients who were sent to operation with diagnosis of endomyocardial fibrosis (EMF) turned out to have pericardial disease. EMF had been suspected on the basis of clinical history, electrocardiographic, radiologic and echocardiographic data. The hemodynamic studies were "typical", and included apex amputation, flat ventricular surface and mitral and tricuspid insufficiencies. Thus, ventriculography should not be considered specific, but rather suggestive of endomyocardial fibrosis.

[Histologic response of the myocardium to various immunosuppressor schedules in patients with dilated cardiomyopathy and diagnosis of myocarditis at endomyocardial biopsy].

PURPOSE: To analyse the histological evolution of endomyocardial biopsies from children with active myocarditis, submitted or not to immunosuppressive therapy. PATIENTS AND METHODS: Four groups of patients were compared, clinically treated as follows: group I--anticongestive drugs (4 patients); group II--prednisone (5 patients); group III--prednisone plus azathioprine (9 patients); group IV--prednisone and cyclosporine (5 patients). RESULTS: No patient from group I presented any histological improvement during a mean period of 9 months, while evident histological improvement occurred in 25% of patients from group I, 67% from group III and 80% from group IV. The microscopical aspect of resolving myocarditis was only observed in patients from groups III and IV, after treatment. CONCLUSION: The immunosuppressive therapy with azathioprine or cyclosporine plus prednisone leads to decrease of active myocarditis intensity in a higher proportion of cases than the treatment with only prednisone or no immunosuppressive drugs.

[Empiric therapy with amiodarone in patients with chronic Chagas cardiomyopathy and sustained ventricular tachycardia].

PURPOSE: To evaluate the efficacy and safety of long-term empiric amiodarone therapy in patients with recurrent Sustained Ventricular Tachycardia (SVT) and Chronic Chagasic Myocarditis (CCM). PATIENTS AND METHODS: Thirty-five patients with CCM and SVT, eighteen (51%) of them were refractory to other antiarrhythmic drugs. The Amiodarone loading dose was between 600 and 1200 mg/day, mean of 883 +/- 239 mg/day, from a period of one to four weeks. The maintenance dose was decreasing in the follow-up period, it fell down to 356 +/- 125 mg/day at the end of six to 80 (mean = 27 +/- 20) months. Cumulative, event-free interval curves were generated by the Kaplan-Meier method. Clinical variables were compared with the use of the Student t-test or by means of chi-square tests. RESULTS: The probability to suppress SVT was, 0.62 in 12 months, 0.56 in 24 months and 0.44 in 36 months, with regular use of amiodarone. The probability to the occurrence of sudden death was 0.0 in 12 months, 0.04 in 24 months and 0.11 in 36 months. The stratification of risk to clinical recurrence was significative to the left ventricular disfunction. All patients with functional class III or IV and LV ejection fraction less than 30% at radioisotopic ventriculography had clinical recurrence, while just 30% of patients with functional class I and II have got it (p less than 0.05). Fifteen (42.8%) patients had side effects. The treatment was discontinued in four patients (11.5%). CONCLUSION: The empiric treatment with amiodarone apparently was effective in patients with SVT and CCM and functional class I and II. Patients with functional class III and IV did not get benefits from the treatment. In these cases other therapy must be pointed out.

Two- to eight-year survival rates in patients who refused coronary artery bypass grafting.

One hundred and fifty patients with coronary artery disease (CAD) who refused bypass grafting were followed prospectively from 2 to 8 years. Mean age was 57 +/- 8 (standard deviation) years. Ejection fraction averaged 70 +/- 14%. Eight percent of patients had 1-vessel CAD and 92% had multiple-vessel CAD. Medical treatment included propranolol, nifedipine, isosorbide dinitrate, dipyridamole and aspirin. Annual mortality was 0% for 1- and 2-vessel CAD and 1.3% for left main equivalent disease, 3-vessel and left main CAD. Treatment significantly reduced the incidence of stable and unstable angina. Fifty-two patients (34%) had a second hemodynamic study 4.2 +/- 1.3 years after initial evaluation. Stenosis progression or new significant obstructions (greater than or equal to 70%) in previously normal coronary arteries occurred in 61% of 123 arteries studied, whereas new occlusions were observed in 12% of the arteries. Nonfatal acute myocardial infarction incidence was 8%. No significant changes occurred in ejection fraction. In conclusion, proper medical treatment in selected patients with advanced CAD but preserved ventricular function is associated with good long-term survival and remission of symptoms, although progression of coronary atherosclerosis does occur in some patients.

Effect of intentional hemodilution on platelet survival in secondary pulmonary hypertension.

Platelet regeneration time was assessed in 13 young adults with pulmonary hypertension and polycythemia secondary to congenital heart defects who underwent isovolemic hemodilution to improve clinical status and coagulation defects. The estimated platelet half-life in patients with Eisenmenger's complex was significantly shortened in comparison with normal subjects (3.8 +/- 1.9 vs 4.8 +/- 1.0 days, p less than 0.05). Hemodilution was carried out with no adverse effects, using low molecular weight dextran solutions. Lowering hematocrit from 61 to 50 percent resulted in a significant increase in platelet half-life from 3.8 +/- 1.9 to 5.7 +/- 1.8 days (p less than 0.02), which was followed by a marked rise in platelet count from 149 +/- 31 to 209 +/- 47 x 10(9) platelets/L (p less than 0.003). Arterial oxygen tension did not change significantly. These observations indicate that high hematocrit levels may have accounted for the shortened platelet survival and thrombocytopenia in these patients. Significant hemodilution may lead to a marked improvement in platelet abnormalities in patients with Eisenmenger's complex.

[Pneumonitis induced by amiodarone].

Fifteen patients, eleven males and four females, with amiodarone induced pulmonary disease were studied. Their ages ranged between 52 and 79 (mean = 64.0) years. 66% of the patients were taking a daily dose of 200 mg of amiodarone. The time elapsed between the initial dose and the diagnosis of the pneumonitis varied from 2 to 84 (mean = 23.3) months. Premature ventricular beats and recurrent episodes of paroxistic supra ventricular tachycardia were the most common indications for the use of the drug. The most frequent clinical complaints were progressive dyspnea and cough. Weight loss was observed in five patients, fever in six and chest pain in two. The most habitual thoracic physical sign was diffuse crepitation. Chest roentgenograms disclosed bilateral interstitial infiltrates in all patients, associated to pleural effusions in two. An increased diffuse uptake of 67 gallium citrate was observed in the nine patients to whom it was done. Lung function tests showed a pattern of restrictive ventilatory respiratory insufficiency and hypoxemia. Lung tissue specimens were obtained in ten patients, bronchoalveolar lavage in one and pleural fluid in one. The material was examined by light and electron microscopy. Amiodarone was discontinued in all patients and corticosteroids were introduced in thirteen. Five patients (33.3%) died, eight improved and two remained with radiographic scars.
