[Current trends in the diagnosis and therapy of anal pruritus. Review of 238 cases].

Perianal pruritus may be caused by a variety of factors. Of these, the most frequently occurring are proctological, infections, dermatological, dietary, and systemic. The results of a study carried out on 238 patients from 1980 to 1984 are reported. The survey was carried out with the aim of defining the frequency of pruritus in relation to the triggering cause, to sex and to age. It is emphasised that diagnosis is often difficult and requires the measurement of the following parameters: blood glucose levels, blood, nitrogen levels, ESR, haemochrome, hepatic functions, in addition to a gynaecological examination, analysis of faeces, proctocolonoscopy, skin biopsy and other tests. These are carried out to determine whether anal pruritus is a symptom of a systemic disease, or whether it is brought about by local causes. Treatment measures adopted are reported. Treatment is mainly symptomatic and aetiological for cases caused by local disturbances.

[Experience with hepatic hydatidosis. Review of 100 cases].

201 cases of hydatid cysts observed in 1964-1984 are analysed. This is followed by a report on 100 cases of hepatic cysts with details of the patients' sex, age, symptoms and occupation. Finally the surgical treatment adopted in response to the site and dimensions of the cysts is described in detail.

Crohn's disease of the esophagus. A case report.

A case of Crohn's disease localized in the inferior third of the esophagus with negative radiological evidence, positive endoscopic evidence, and aspecific signs of esophageal wall inflammation is reported. Evaluation of clinical remission of dysphagia and endoscopic features of the esophageal lesions following steroid therapy led to the diagnosis of Crohn's disease of the esophagus in the patient already operated on for a colonic localization. Data from literature are examined with emphasis on the difficulty of the diagnosis.

A clinical and epidemiological study on eclampsia in the obstetrics and gynecology department of Catania in the years 1978-1983.

From January 1978 to July 1983 22 patients have recovered from eclampsia in the Obstetrics and Gynecology Department - Vitt. Emanuele II Hospital - Catania University. 64% of the patients had hypertension in pregnancy combined with edema in 92.85% of the cases (13 cases) and with proteinuria in 18% of the cases (4 cases). Eclampsia manifested itself before labour in 28.57% (6 cases), during labour in 19.05% (4 cases), in postpartum in 19.05% (4 cases) and in the successive three days after delivery in 33.33% (7 cases). Perinatal death rate was 11.28% and in all the cases it was due to intrauterine fetal death. In 22 patients treated there was a case of acute pulmonary edema and a case of maternal death.

[Epidemiologic-statistical data on chest injuries: review of 181 cases].

The Authors report their casuistry about thoracic traumata (181 cases), they had the opportunity to observe from 1972 up to 1983. They illustrate the most interesting epidemiologic data, such as sex and average age of patients, causes of trauma, hurted anatomic formations of thorax, association with traumata in a different place. At last, as a conclusion, they remark the exitus occurred in 5.5% of thoracic traumata.

Use of type-specific antimyosins to demonstrate the transformation of individual fibers in chronically stimulated rabbit fast muscles.

Continuous stimulation of a rabbit fast muscle at 10 Hz changes its physiological and biochemical parameters to those of a slow muscle. These transformations include the replacement of myosin of one type by myosin of another type. Two hypotheses could explain the cellular basis of these changes. First, if fibers were permanently programmed to be fast or slow, but not both, a change from one muscle type to another would involve atrophy of one fiber type accompanied by de novo appearance of the other type. Alternatively, preexisting muscle fibers could be changing from the expression of one set of genes to the expression of another. Fluorescein-labeled antibodies against fast (AF) and slow (AS) muscle myosins of rabbits have been prepared by procedures originally applied to chicken muscle. In the unstimulated fast peroneus longus muscle, most fibers stained only with AF; a small percentage stained only with AS; and no fibers stained with both antibodies. In stimulated muscles, most fibers stained with both AF and AS; with increasing time of stimulation, there was a progressive decrease in staining intensity with AF and a progressive increase in staining intensity with AS within the same fibers. These results are consistent with a theory that individual preexisting muscle fibers can actually switch from the synthesis of fast myosin to the synthesis of slow myosin.

[Survey of the frequency of routine diagnostic tests in term pregnancies].

In 420 pregnant women who delivered during the third trimester at the "Istituto di I Clinica Ostetrica e Ginecologica", University of Catania, routine blood tests were performed in 95.5% of cases (401 women), urine tests in 95.5% (401), Hb-phoresis in 42.6% (179), HBsAg in 51.4% (216), TORCH and urine cultures in 37.8% (159), cardiotocograph in 73.1% (307), plasma RIA of E3 and/or HPL in 35% (147), maternal ECG and pseudocholinesterasemia in 83.6% (351) and 37.6% (158) respectively. No routine diagnostic tests were performed in 4.5% (19 women) of cases. Diagnostic tests are less frequently performed in women, plurigravida, and in pregnancies which follow a physiological course. Several diagnostic tests, except for TCTG and HBsAg, are performed more frequently in pregnancies with complications. Lastly, there is a marked difference between the number of diagnostic tests performed and the individual woman's place of residence.

[Cancer and pregnancy. Retrospective study on the frequency in 57,393 deliveries].

Between 1950 and 1984 out of 57.393 women who delivered at the First Department of Obstetrics and Gynecology, Catania University Medical School, Catania, Italy, 40 cases of malignant neoplasia were diagnosed with an incidence of one case in 1.434 deliveries. The most frequent neoplasias is cervix carcinoma (21 cases; 52.5%), followed by breast cancer (6 cases; 15%), ovarian cancer (4 cases; 10%) and leukemia (4 cases; 10%). There was very rare association with Hodgkin disease (2 cases; 5%), osteosarcoma (1 case; 2.5%), medulloblastoma (1 case; 2.5%), and skin melanoma (1 case; 2.5%). Since cancer of the uterine cervix is the most frequent neoplasia (one cases out of 2.733 deliveries), cervical smear should be performed during pregnancy in women that never performed it.

[Prevalence of endemic goiter in Sicilian pregnant women in the Alte Madonie region].

The "Alte Madonie" is a hill-mountain community in Sicily with a population of approximately 30,000 inhabitants. It includes the towns of Alimena, Bompietro, Blufi, Castellana Sicula, Fasano, Gangi, Geraci, Petralia Sottana, Soprana and Polizzi generosa. This is an area where goitre is endemic. Between January 1990 and September 1991 out of a population of 400 pregnant women examined at various stages of pregnancy, goitre was found in 1 out of 7.8 women (51 cases). 74.5% (38 cases) of those with goitre had undergone endocrinological tests, in all except two cases, a long time before the start of pregnancy. Three pregnant women (5.8%) were receiving suppressive therapy with L-thyroxine. 58.8% (30 cases) had undergone hormonal controls and laboratory data had revealed cases of hypothyroidism; in the remaining cases hypothyroidism had been excluded on the basis of medical examination. One patient, not included in this series who had received IVG, had been treated a few months before with strumectomy and radio-iodio-therapy for non-differentiated carcinoma which had developed on the goitre. In conclusion, these findings underline the high frequency of goitre in pregnant women from the "Alte Madonie", and the need for greater care in the prevention and treatment of this pathology. There is a clear need for integrated health education programmes and early diagnosis to be carried out in schools and work places. The role played by the gynecologist who first sees the pregnant women with goitre is vitally important, especially in the case of those patients which dedicate little time to the health of their own bodies either for cultural reasons or due to the demands of work both inside and outside the family.

[Ultrasound in the study of gynecologic neoplasms].

The authors discuss the state-of-the-art of ultrasound diagnosis for pelvic swellings, with special reference to uterine and ovarian cancer. They review the applications of Eco colour Doppler in gynecological oncology and discuss the protocols for the prevention and/or early diagnosis of gynecological tumours.
