Effect of a calcium antagonist, nifedipine, in exercise-induced asthma.

The nifedipine effect was studied in 8 extrinsic asthmatic subjects with exercise-induced asthma. Before the exercise the patients received, in a randomized double-blind manner, either 20 mg nifedipine, sublingually or sodium cromoglycate by inhalation on 2 separate days. Nifedipine and sodium cromoglycate in all patients inhibited the exercise fall in FEV1. No differences were found between the two drugs. Nifedipine is a potent antagonist of calcium ion influx in smooth muscle and secretory cells, and these studies suggest that it may inhibit release of mast cell mediators and reduce bronchial smooth muscle contractility in asthma.

Computerized study with 201T1 of the cold thyroid node.

Because of its physical and potassium-metabolic characteristics 201T1 is more suitable than 131Cs for radioisotopic studies of the cold thyroid nodule, with the further diagnostic possibility of quantitatively assessing intranodular behavior for a specific differentiation among different kinds of neoformations. Using a gamma-camera on line with a computer data processing device, sequential scintiscans were recorded for the first 20-30 min after i.v. administration of 15-20 microCi/kg of radiothallium; delayed sequences were taken at 40-60 min if intranodular uptake appeared. A quantitative appraisal was made of the differential 201T1 uptake-ratio between nodule and healthy thyroid tissue (density-index) and the multiparameter analysis of thyroid time/activity curves generated on the relative regions of interest (ROIs). This computerized study, in 120 out of 293 patients submitted to this radiothallium test, has shown a) diagnostic agreement between clinical-histological and radioisotopic findings in 76 out of 79 colloid-cystic or degenerative neoformations, in all 16 malignant and in 23 out of 25 hyperplastic benign nodules; b) significant statistical difference of the density-index in solid versus cystic but not between benign and malignant nodules; c) different 201T1 kinetics behaviour in different kinds of solid thyroid lesions with a satisfactory statistical difference of the radiothallium nodular disappearance-index.

[Controlled clinical study of the treatment of chronic bronchitis with guacetisal].

Controlled clinical research has been carried out on the activity and tolerance of a new active principle, guacetisal (Broncaspin) obtained from the esterification of acetylsalicylic acid with guaiacol, in the treatment of chronic bronchitis. The drug's therapeutic response was evaluated with respect to that of bromexine. Guacetisal was generally well tolerated. It had no unwanted side-effects on the main haematochemical parameters or on the function of organs and systems. It was found to have considerable therapeutic effectiveness, at times even superior to that of the control drug, with respect to general symptomatology and at respiratory system level. It produced early, lasting reduction in temperature, heart frequency, dyspnoea, duration of expirium and in the intensity and number of coughing attacks. It also led to an appreciable improvement in thoracic objectivity and the X-ray picture. Variations in respiratory functional parameters were of considerable interest and from these it is concluded that guacetisal exerts its polyvalent activity to a proportionately higher extent in the bronchial districts more seriously involved in the inflammatory process, inducing an elective improvement in bronchial permeability, a reduction in total pulmonary resistances--with consequent tendency for ventilation-perfusion relations to normalise--as well as an improvement in gas exchanges and patient oxygenation.

201Tl in the scintigraphic evaluation of the "cold" thyroid areas.

201Tl-chloride, which has a metabolic behaviour similar to that of potassium and cesium, has been used in 68 patients for the evaluation of thyroid nodules previously recognized as "cold" on 131I or 99mTc scans. All patients were re-examined with gamma-camera and/or sequential scintigraphic recordings during 60 min after i.v. administration of thallium. In some cases, simultaneous imaging and integral digital plot with 131I or 99mTc and 201Tl were performed. In all 12 malignant nodules, 201 Tl has showed a high uptake, while it did not concentrate in 47 benign nodules (cystic or macrofollicular adenomas); thallium uptake was nevertheless found in 10 solid neoformations in which histological pictures were negative for malignancy or atypical lesions. The computerized study of the 201Tl intranodular concentration, with the analysis of its dynamic function curves, seems to offer further possibility in differentiating and in a more objective evaluation of the "cold" areas on the thyroid scan.

Analysis of metabolic changes in revertants of antibiotic-resistant micobacteria.

In the present report evaluation of the possible metabolic changes in partially revertants of M. tuberculosis var. hominis, reisolated from strains at high levels of drug-resistance after treatment with mutagenic agents, was reported. Based on the data presented here, it must be concluded that no significant biological and biochemical differences in mycobacterial cells revertant from high mutants mono-resistant to four aminoglycosides (Sm, Kana, Vio, Neo) and p-aminonasalicylic acid (PAS) are to be found.

Mutational studies of the drug-resistance of M. tuberculosis with special reference to p-aminosalicylic acid, ethambutol, isoniazid and rifampicin.

In the M. tuberculosis H37Rv and in the human tubercular strains isolated from patients, the localization of genetic determinants of drug-resistance to antitubercular chemotherapeutic drugs in current use was studied. Data obtained through the treatment of the resistanct strains with acridine dyes and ethidium bromide, and the analysis of irreversible elimination of resistance markers support the hypothesis of an extrachromosomal locus for the p-aminosalicylic acid (PAS)-resistance and confirm a chromosomal mutation for the isoniazid (INH), ethambutol (EM), rifampicin (R/AMP)-resistances, in all the levels reached in training or selected in wild strains. In the PAS-revertants recovered after treatment with mutagenic agents, a loss by degrees of resistance levels was observed. In a mycobacterial strain biresistant to PAS and streptomycin (Sm) the Authors were able to find a loss of the mono-resistance to Sm only. The significance of these results is discussed in terms of plasmid (s) having separate mutational loci for PAS- and for the aminoglycoside-resistance. These results agree with the previous studies in the field of resistance of Mycobacteria to specific drugs.
