[A study on the psychological status of perioperative patients].

To evaluate the causes of psychic disturbances after operation, three different psychological tests, Spielberger's rating scale "The State-Trait Anxiety Inventory (STAI)", Maudsley Personality Inventory (MPI) and Baum test, were performed during perioperative periods in 25 patients who received simple total hysterectomy. The score of the A-Trait analyzed from STAI that is trait anxiety, did not change significantly, while the score of the A-State analyzed from that, that is state anxiety, decreased significantly in postoperative period. Especially, the score of the A-State showed gradual decreasing tendency during perioperative period in the patients, who were classified as N+ group in MPI, showing the tendency of nervousness. On the contrary, in the patients of N- group, showing no tendency of nervousness, the score of the A-State increased significantly immediately before operation. In Baum test, most of the patients showed some psychological disturbances even during the postoperative period. From the Baum test, it was shown that they had psychologic fragility and lability even postoperatively, in spite of stabilization of general status.

[A study on evaluation of circulating anticoagulant in borderline cases].

Detection of circulating anticoagulant (CA) is clinically important in patients with SLE, habitual abortion and other hemorrhagic and thrombotic disorders. We have studied CA in 83 patients with such disorders. Thirty-one of 83 (37%) had positive CA and 16 of 83 (19%) had negative CA, while 36 of 83 (44%) showed borderline pattern of CA. The experiment was undertaken to detect CA clearly in patients with borderline CA, by using diluted APTT or PT reagents. When we used 5% diluted reagent in APTT or 50% diluted reagent in PT, constant value of clotting time showed 1.8% or 1.2%, respectively. Seven of 10 with borderline CA cases tested here revealed positive CA by using diluted reagents. Furthermore 2 of remaining 3 with borderline CA cases showed positive anti-cardiolipin antibodies, suggesting of presence of lupus anticoagulants. These results suggest that CA in borderline cases should be considered to be positive and to be possibly derived from lupus anticoagulants.

Synthesis of new heparinoids with high anticoagulant activity.

New heparinoids were synthesized by the chemical method starting from ring-opening polymerization of anhydro sugar derivatives. Sulfation of synthetic (1----6)-alpha-linked 3-amino-3-deoxy-D-glucopyranan and its copolymers gave dextran-type heparinoids having a sulfamide group on the C-3 carbon of the sugar unit. Heparinoids with different sulfamide contents indicated that the anticoagulant activity (35.3-41.3 units/mg) is independent of the sulfamide content, while an increase in sulfamide content lowered the toxicity. Sulfation of (1----5)-alpha-D-xylofuranan and -ribofuranan provided furanan-type heparinoids the anticoagulant activities of which were higher than those of the corresponding sulfated pyranan-type polysaccharides (1----4)-beta-D-xylopyranan and -ribopyranan. The highest activity (69.1 units/mg) was shown by sulfated (1----5)-alpha-D-xylofuranan. The dextran-type heparinoid having a sulfamide group showed a high anticoagulant activity also in vivo and high lipemia-clearing activity.

Effect of splenectomy and immunotherapy on advanced gastric cancer associated with total gastrectomy.

Immunotherapy with levamisole (LMS) and combined splenectomy were performed in the cases of stages III and IV gastric cancer of the upper and middle stomach and associated with total gastrectomy. The cell-mediated immunity and the survival rates for both groups were compared with controls. Combined splenectomy was not effective for cases of stage III gastric cancer, but was effective for those in stage IV. Comparing the cumulative survival rates of the immunotherapy group with the controls, cases of stages III and IV gastric cancer without splenectomy, the survival rate of those with immunotherapy was higher than that of the control group in the cases of the stage III, but the result was reverse in cases of stage IV gastric cancer. Changes in the cell-mediated immunity correlated with the survival rates.

Detection of hypercoagulability by the measurement of the dynamic loss modulus of clotting blood.

The dynamic loss modulus of clotting whole blood was measured in thrombotic patients to characterize the physical properties of coagulation in the hypercoagulable state. The dynamic loss modulus was measured by a Sonoclot. Thrombotic patients consisted of 30 with deep vein thrombosis and 25 with arterial thrombosis. An accelerated increment rate of the dynamic loss modulus at the beginning of gelling was the characteristic of hypercoagulability. This characteristic occurred more frequently than other abnormalities in other tests for hypercoagulability (beta-thromboglobulin, antithrombin III and TEG). Only in deep vein thrombosis, a moderately positive correlation was noted between the increment rate of the dynamic loss modulus and the plasma fibrinogen level.

[Effect of immunochemotherapy on the cases of advanced gastric cancer stage III].

This study was conducted in patients with stage III gastric cancer who were admitted and gastrectomized in our Department of Surgery during July 1971 to March 1980. The control group consisted of 32 patients who were treated with mitomycin C and FT -207 during the first half of the period and the levamisole (LMS) group of 32 patients who were treated with the same regimen plus LMS during the latter half of the period. No significant statistical difference in their back ground factors was found. The LMS group was treated with 150 mg/day of LMS for 3 consecutive days every other week starting 3 days before operation. The follow-up study up to 30 postoperative months indicated an enhancing effect of LMS on the survival rate in patients with gastric cancer stage III undergoing curative resection. LMS proved to be effective on inhibition of the tumor growth when the tumor sizes ranged 4.0 to 7.9 cm, and the metastases were limited within the regional lymph node.

Immunochemotherapy of gastric cancer with levamisole.

In 156 cases of gastric cancer, levamisole (LMS) was administered at a daily dose of 150 mg for three consecutive days every other week. The administration was started 3 days before operation. This medication was repeated for more than one month. The survival rate up to two years after surgery was studied. The survival rate was not affected in patients with Stage I and II gastric cancer, but in patients with Stage III, the difference in the survival rate between the LMS group and the control group was significantly higher than that in the control group (p less than 0.05). In patients with Stage IV, the survival rate in the LMS group was higher than that in the control group although the difference was not significant. In patients of Stage III and IV, the effect of LMS on the survival rate was highest in cases with curative resection (p less than 0.01). In cases with noncurative resection, the difference between the LMS group and the control group was greatest (24.4%) 12 months after surgery but not significant (p less than 0.5), and also in cases without resection the difference between the two groups was greatest (20.3%) 12 months after surgery but not significant (p less than 0.2).

Two cases of pseudoaldosteronism (Liddle's syndrome) in siblings.

Two cases of Liddle's syndrome were found in a brother and sister. Both showed typical hypokalemic hypertension without hyperaldosteronism. These cases showed similar responses in various pharmacological tests and their symptoms of hypokalemic and hypertension were relieved by triamterene. And in a family survey, the father appeared to be affected. This seems to be the first report on this syndrome in Japan.
