Suppressive effects of genistein dosage and resistance exercise on bone loss in ovariectomized rats.

This study was designed to determine whether combined treatments with genistein dosage and moderate resistance exercise would exhibit synergistically preventive effects on bone loss following the onset of menopause. Forty-one 12 wk-old female SD rats were assigned to five groups: 1) Sham operated (Sham); 2) ovariectomized (OVX-Cont); 3) OVX received genistein (OVX-GEN); 4) OVX exercised (OVX-EXE); and 5) OVX treated with both genistein and exercise (OVX-GEN-EXE). All rats were fed a low Ca (0.1%) diet ad libitum. Daily genistein dosage was 12 mg/kg body weight. Exercising rats took 40 sets of 1-min run interspersed with 1-min rest with a 100 g weight on the back on an uphill treadmill at 20 m/min. The experimental duration consisted of the adaptation and treatment periods of 4 weeks each. Uterine weight in OVX-Cont, OVX-GEN, OVX-EXE and OVX-GEN-EXE decreased to about 15% of that in Sham (p < 0.001). The femoral BMD (mg/cm2; mean +/- SE), assessed by DEXA (Lunar), of OVX-Cont was significantly lowered to 206 +/- 5 by -9%, as compared to 226 +/- 2 of Sham (p < 0.001). The BMD of OVX-GEN, OVX-EXE and OVX-GEN-EXE were 217 +/- 2, 217 +/- 2 and 222 +/- 2, respectively, and genistein dosage and resistance exercise equally increased the BMD of OVX rats by 5% (p < 0.01). Combined treatment of genistein and exercise more successfully recovered their decreased BMD by 8% (p < 0.001). BMD of the fourth lumbar vertebrae in OVX-Cont was declined to 191 +/- 7 by -15%, as compared to 225 +/- 4 in Sham (p < 0.001). OVX-EXE and OVX-GEN-EXE gained the BMD by 6% to 205 +/- 4 and 203 +/- 3, respectively, as compared to that of OVX-Cont (p < 0.01). These results suggest the possibility that the combined treatment of genistein dosage and resistance exercise have more beneficial effects by acting rather independently than their separate trials on the prevention of ovx-induced bone loss in femurs.

[Multicystic sphenoethmoidal mucocele with intracranial extension presented with generalized convulsion: a case report].

A 53-year-old male presented with generalized convulsion. Magnetic resonance imaging and computed tomography revealed large sphenoethmoidal mucocele extending intracranially. Signal intensities between paranasal and intracranial portion of the mucocele were different. Preoperatively it was not obvious that intracranial portion of the mucocele communicated with paranasal portion of the mucocele. We approached the paranasal portion of the mucocele by transsphenoidal approach resulting in disappearance of the intracranial portion of the mucocele. Sphenoethmoidal mucocele with intracranial extension presenting with generalized convulsion was the first case in the literature. Transsphenoidal approach for sphenoethmoidal mucocele with intracranial extension is the first choice of surgical treatment, though neuroimaging shows that there is no communication between paranasal and intracranial portion of mucocele.

[An adult case of cerebellar mutism after removal of cerebellar hematoma].

Cerebellar mutism is a well documented complication of posterior fossa tumor surgery in children. But only one case presenting with cerebellar mutism after removal of a cerebellar hematoma in adults was reported previously. We present 64-year-old male developing cerebellar mustism after surgery for hypertensive cerebellar hemorrhage. A cerebellar hematoma was located in the right side dentate nucleus and cerebellar peduncle dominantly not extending to the cerebellar vermis. During the period of cerebellar mutism, single photon emission computed tomography revealed a decrease in the contralateral temporo-parietal cerebral blood flow. Cerebellar mutism in this case may be associated with the low blood flow in the contralateral supplementary motor cortex owing to injury to the dentate-thalamo-cortical pathway at the dentate nucleus and cerebellar peduncle. The relevant literature is reviewed.

Bisphosphonate therapy for bone metastases from breast cancer: clinical results and a new therapeutic approach.

BACKGROUND: We evaluated the usefulness of bisphosphonate (BIS) monotherapy, the safety of rapid infusion of BIS and the efficacy of BIS-sequential therapy for bone metastases from breast cancer. PATIENTS AND METHODS: Twenty-nine patients with bone metastasis or invasion were treated with BIS monotherapy. Each BIS (pamidronate 30 mg, alendronate 10 mg, or incadronate 10 mg) was infused over 30 minutes every two weeks a median of 12 times. RESULTS: With BIS therapy, five patients (17%) showed partial response of the bone lesions, and eighteen patients (64%) had pain relief. Of the nine patients treated with BIS-sequential therapy, one (11%) showed a partial response of the bone metastases, three (33%) had pain relief, and one (11%) showed a decrease in the serum tumor marker level. CONCLUSION: BIS therapy is effective against bone metastases from breast cancer, and rapid infusion of BIS is both safe and convenient for patients. BIS-sequential therapy can be a unique therapeutic option in some cases.

Prolonged swimming exercise training induce hypophosphatemic osteopenia in stroke-prone spontaneously hypertensive rats (SHRSP).

Stroke-prone spontaneously hypertensive rats (SHRSP) induce spontaneous osteoporosis. To elucidate the specific characteristics of bone metabolism, the SHRSP was compared with age matched Wistar-Kyoto (WKY) rats. We investigated the effects of prolonged swimming exercise training on bone mineral density (BMD) and metabolism in the SHRSP. Seven-week-old male SHRSP and WKY were divided into three groups; the sedentary control WKY group (n = 6, WKY), the sedentary control SHRSP group (n = 6, SP) and the swimming exercise training SHRSP group (n = 6, SWIM) (in pool with 60 min./day, 5 days/week for 12 weeks). The femoral BMD, bone mineral content (BMC), strength, Ca and P contents (%) of SHRSP were approximately 17, 27, 25, 20 and 9%, respectively, lower than that of WKY (p < 0.001). Serum alkaline phosphatase (AlP) had not changed between both of SP and WKY, but tartrate-resistant acid phosphatase (TrAcP) of SP approximately 3-fold higher than that of WKY (p < 0.05). Both serum calcium (Ca) and intact parathyroid hormone (i-PTH) were similar between SP and WKY. However, serum phosphate (P) of SP was approximately 18% lower than that of WKY (N.S.). These results suggested that SHRSP induces osteopenia by the bone turnover of the promoted osteoclast activity with disturbed phosphate homeostasis. On the other hand, the femoral BMD and strength were approximately 7% and 20%, respectively, decreased in the SWIM (p < 0.001), and femoral bone Ca and P contents (%) were also approximately 11% and 14%, respectively, lower than that of SP (p < 0.001). There were no significant difference between SWIM and SP on serum Ca, but serum P of SWIM was significantly lower than that of SP (p < 0.05). These results suggested that the prolonged swimming exercise training in the SHRSP induces more cruelly hypophosphatemia, and leading to osteopenia eventually. We conclude that SHRSP induces osteopenia with disturbance of phosphate homeostasis, and the prolonged swimming exercise in the SHRSP might deteriorate hypophosphatemia and osteopenia.

Immobilization on the day 14th does not disrupts the basic diurnal rhythm of bone resorption.

Weight bearing and physical activity are important mechanical stimuli to bone growth and metabolism, and microgravity, such a space flight and/or bed rest, induces bone resorption and bone loss. An increased excretion of urinary Ca, an increased bone resorption and a decreased bone mineral density (BMD) have been observed in bed rest experiment of healthy subjects. Bone resorption markers show the specific circadian rhythms in human. Cross-linked carboxyl-terminal telopeptide of type I collagen (ICTP) and the urinary excretion of deoxypyridinoline (Dpy) are the highest in the early morning and the lowest late at night. Bed rest immobilization might influence these rhythms, due to no mechanical loading with loss of daily life activity. Bone resorption markers in healthy subjects had been compared between before and during bed rest to determine disruption of diurnal rhythms of bone resorption.

Variable-speech-rate audiometry for hearing aid evaluation.

A new hearing aid evaluation method using variable-speech-rate audiometry (VSRA) was developed. VSRA was newly created based on the Japanese speech audiometry authorized by the Japan Audiological Society. The ordinary speech audiometry can not reveal a temporal factor in word discrimination ability of the hearing impaired. Since, with VSRA, we can compare several performance-intensity curves obtained from different speech-rate speech audiometries, the impact on the auditory system of each patient by the fast or slow speech rate could be easily determined. Taking the temporal factor of the auditory systems into consideration by using VSRA, hearing aid evaluation was performed for a master hearing aid with three types of signal processing and fitting for 36 hearing impaired subjects. Then hearing aid evaluation was performed using VSRA for a newly developed portable multi-function digital hearing aid with two types of signal processing and analog hearing aids which had been used by hearing-impaired patients. As a result, VSRA was useful for hearing aid evaluation, in particular, for cases when ordinary normal speech rate audiometry does not provide a significant difference in word discrimination scores. In addition, using VSRA revealed that amplitude compression is more effective for improvement of word discrimination than linear amplification.

[Fundamental and clinical consideration of repeated intracordal injection therapy of atelocollagen--a histological study of canine vocal cords and a review of clinical data of 96 patients].

Hoarseness or aspiration due to recurrent laryngeal nerve paralysis confront many patients with problems in their daily living. At our outpatient clinic, patients with unilateral laryngeal paralysis due to recurrent laryngeal nerve paralysis are treated by injecting atelocollagen (3% non crosslinked atelocollagen, Koken Co., Ltd.) into the vocal cords under endoscopic control, using a soft fiberscope. A single injection may not be effective, but a relatively satisfactory therapeutic result can be obtained after several repeated injections. In the present study, the tissue levels of atelocollagen in the vocal cords of 8 adult dogs were determined after intracordal injection to evaluate the efficacy of repeated injection therapy of atelocollagen. In addition, clinical data of 96 patients who were treated by this therapy at our outpatient clinic (57 males and 39 females with a mean age of 54.5 years) were assessed. Histological investigation indicated that relatively good amounts of atelocollagen were retained and accumulated in the vocal cord tissue after repeated injection, and that the tissue around the injection site showed no appreciable foreign body reaction. The clinical data showed that about half of the 96 patients had received repeated intracordal injection therapy of atelocollagen. Maximum phonation time (MPT) was determined in 75 of 96 patients. The preoperative mean MPT was 3.27 sec, whereas MPT after a single injection and after all injections increased to 6.67 and 7.16 sec, respectively. The difference was statistically significant (p < 0.001), as assessed by the paired t-test. The results of this study indicate that besides being beneficial in accidents that result in recurrent laryngeal nerve paralysis, injection therapy with atelocollagen is of clinical value because the patient can receive repeated injections under endoscopic control at the outpatient clinic at reasonable intervals of time, and thus makes the therapy more convenient.

[A case of traumatic dissecting aneurysm of the C1-2 portion of the internal carotid artery].

The authors report a case of traumatic dissecting aneurysm of the C1-2 portion of the internal carotid artery (ICA) in a 54-year-old woman. She suffered from traumatic SAH due to closed head trauma as a result of a motor vehicle accident. Twenty-five days after this accident, traumatic dissecting aneurysm of the C1-2 portion of the ICA that was caused by closed head trauma was ruptured and increased in size, as revealed by serial angiographic studies. In intraoperative finding, dissection involved the entire circumference of the C1-2 portion of the ICA. Clipping procedures were impossible, so internal carotid ligation and STA-MCA anastomosis was performed. To our knowledge, this traumatic dissecting aneurysm of the C1-2 portion of the ICA was the first case that presented with SAH. We discussed the mechanism of dissection of the ICA and operative strategy suitable for this aneurysm.

Intratumoral administration of tumor necrosis factor-alpha for malignant gliomas--two case reports.

Two patients with histologically verified glioblastoma multiforme and anaplastic astrocytoma were treated with four courses of intratumoral administration of human natural tumor necrosis factor-alpha (TNF) (specific activity 2.0 x 10(6) Japan reference unit [JRU]/mg protein) at intervals of 3-5 months. Each consisted of eight to 10 serial injections, ranging from 5 x 10(3) to 10(4) JRU/injection, at intervals of 3-5 days. There was no simultaneous administration of steroids. Serial neurological examinations and neuroimaging studies with computed tomography and magnetic resonance imaging demonstrated partial responses ranging from 28 and 36 months in duration. No significant TNF-related brain edema, intracerebral bleeding, or neurotoxicity occurred. Local immunotherapy with TNF may be used safely to contribute to therapeutic efficacy.

[Damus-Kaye-Stansel procedure following Van Praagh (PA-descending Ao shunt) for complete transposition of the great arteries, hypoplastic aortic arch and coarctation].

A newborn baby with d-TGA, straddling AV valve, large VSD, hypoplastic ascending aorta and coarctation of the aorta had congestive heart failure and oliguria on the 3rd day after birth. Van Praagh's operation (main pulmonary artery to descending aorta bypassing with synthetic graft) was performed on 4 days of age through a lateral thoracotomy. At the age of 2 months, the patient underwent left pulmonary artery banding because of respiratory insufficiency requiring prolonged ventilatory support. The patient subsequently underwent Damus-Kaye-Stansel procedure without enlargement of the arch, and the pulmonary patch angioplasty with right modified BT shunt on the age of 5 months. This baby was discharged on 4 months after the operation. Two-dimensional echocardiography has shown aortic and pulmonary competence postoperatively.

[Decreased theophylline clearance in children with acute asthma with concurrent febrile respiratory infection].

Elevated serum levels of theophylline, a first-line anti-asthma drug, are sometimes associated with febrile respiratory infection (FRI). Although it seems to be wellknown, conflicting reports still appear in journals. We studied retrospectively theophylline clearance (th-cl) in 74 episodes of acute asthma (44 with and 30 without FRI, in patients aged 12-mos.-13-yrs.) and tried to reveal the effect of fever or pathogens on th-cl. Th-cl was estimated by a single sample technique during intravenous drip infusion of aminophylline. The results were as follows: 1) Forty four episodes with FRI included 20 viral, 10 bacterial and 2 Mycoplasma pneumoniae infections (Table 2). 2) Th-cl in 44 episodes with FRI was significantly lower than in 30 without FRI (p < 0.001). 3) Decreased tendency of th-cl in episodes with viral infection was observed, but statistically it was not significant (p < 0.1). However, the differences of th-cl among 9 pair-cases with and without viral infection were quite clear (p < 0.01). 4) Th-cl in episodes with 10 bacterial and 2 Mycoplasma infections was reduced significantly (p < 0.01). 5) There was no difference in th-cl between episodes with viral, and bacterial or Mycoplasma infections (p < 0.1). As a result, it is suggested that fever, regardless of its origin, may decrease th-cl in acute asthma with FRI.

Quartz crystal microbalance for the detection of microgram quantities of human serum albumin: relationship between the frequency change and the mass of protein adsorbed.

We have developed a piezoelectric immunosensor for the detection of microalbumin. Human serum albumin (HSA) in the range 0.1-100 micrograms mL-1 could be detected using a flow cell; the immunosensor is sensitive enough to monitor levels of albuminuria. The immunosensor did not respond to bovine serum albumin, only to HSA, implying that the specificity for HSA was high. We investigated the relationship between the frequency change (delta F) and adsorption per unit area of piezoelectrically active quartz crystal (delta M). delta M was estimated with radioisotope-labeled anti-HSA or HSA. When anti-HSA was adsorbed onto the surface of the crystal or HSA was bound to anti-HSA supported by the crystal, values of magnitude of delta F/delta M were larger than the value predicted from theory (Sauerbrey's equation). Furthermore, magnitude of delta F/delta M for HSA was larger than that for anti-HSA.

Effects of nitro-L-arginine on endothelium-dependent relaxation of canine cerebral arteries.

1. The effect of NG-nitro-L-arginine (NO2Arg) on the relaxation of canine basilar artery was investigated and compared with those of middle cerebral and femoral arteries. 2. NO2Arg (10(-7)-3 x 10(-5) mol/L) inhibited the substance-P (Sub-P; 10(-12)-10(-8) mol/L) induced relaxation in the basilar artery precontracted with prostaglandin F2 alpha (PGF2 alpha; 10(-5) mol/L) or KCl (10(-2) mol/L) in a concentration-dependent manner and a ratio of the maximum inhibition by NO2Arg (3 x 10(-5) mol/L) was more than 90%. 3. The relaxation induced by A23187 (10(-9)-3 x 10(-6) was also abolished by NO2Arg (3 x 10(-5) mol/L), but that by glyceryl trinitrate (GTN; 10(-9)-3 x 10(-5) mol/L) was not, in the basilar artery precontracted with PGF2 alpha (10(-5) mol/L). NG-nitro-D-arginine (NO2ArgD; 3 x 10(-5) mol/L) did not affect the relaxation induced by Sub-P (10(-12)-10(-8) mol/L). 4. L-arginine (L-Arg; 3 x 10(-5)-10(-4) mol/L) did not inhibit Sub-P (10(-12)-10(-8) mol/L) induced relaxation in the basilar artery. Pretreatment of L-Arg (10(-4) mol/L) reversed the relaxation inhibited by NO2Arg (3 x 10(-6) mol/L) in the arteries. 5. NO2Arg (3 x 10(-5) mol/L) inhibited the Sub-P (10(-12)-10(-8) mol/L) induced relaxation in the canine middle cerebral artery as much as in the basilar artery.(ABSTRACT TRUNCATED AT 250 WORDS)

[Transsubmucosal maxilloethmoid sinus surgery through inferior turbinate (swinging of nasolacrimal duct and inferior turbinate)].

We recently devised a new technique for endonasal surgery, which simplifies maxillary antrotomy and ethmoidectomy, and dilates the middle nasal meatus widely enough to allow easier postoperative observation. Surgery is initiated by an incision similar to the one used for submucosal resection of the inferior turbinate. The bony inferior turbinate is resected, and the nasolacrimal duct is freed from the bone. Subsequently, the maxillary and ethmoidal sinuses are excenterated, with swinging of the membranaceous inferior turbinate and the nasolacrimal duct. Postoperative observation confirmed satisfactory results.

[Study on human diseased thyroid cells in collagen gel culture--morphology and their malignancy of cultured colonies].

Thyroid cells from 14 normal subjects, two patients with Grave's disease, four patients with follicular adenoma and eight patients with papillary carcinoma were cultured in collagen gel. The colonies of these cells were stereoscopically observed and their morphological characteristics were studied with regard to relation with pathological findings of mother tumor, extra-capsular invasion and metastatic potential. For normal thyroid, Grave's disease and follicular adenoma (except for one case), their own characteristic branching type colonies were found. For papillary carcinoma, both branching type and spheroid type of colonies were observed. The ratio of branching type/spheroid type varied individually in the patients with papillary carcinoma. However, the spheroid type was found to trend to be predominant in patients with extra-capsular invasion and/or lymph node metastasis. This means that the observation of spheroid type colonies in collagen gel is suggestive of risk of extra-capsular invasion or lymph node metastasis. From the obtained results, it seemed possible to diagnose poorly-differentiated cells in vitro by morphologically observing colonies of human thyroid papillary carcinoma cells developing in collagen gel.
