[Effect of a histamine aerosol in 20 subjects with histamine hyperreactivity. Application to a study of the protective properties of pipoxizine].

The authors made a study of the antihistaminic properties of Pipoxizine in 20 subjects with proven histaminic hyperreactivity. The design of the trial consisted in comparing the changes of VC, FEC1, expiratory airway resistance and V50 produced by a histamine aerosol given before and after administration of Pipoxizine. Pipoxizine was given by mouth to 10 patients and intravenously to the other 10 of the group. The statistical analysis of the results demonstrated an antagonist effect of Pipoxizine on the histamine induced bronchoconstriction. The data of this trial are confirmative of the results of other experimenters. It seems therefore reasonable to take into consideration the use of Pipoxizine in the preventive treatment of the paroxystic attacks of the asthmatic disease.

[Statistical study of the correlations between spirometric data and arterial blood gas tensions. II. During a 40-watt exercise (author's transl)].

PaO2 and PaCO2, after 5 min of a 40-Watt exercise, and spirometric data have been statistically evaluated in 152 patients with chronic obstructive bronchitis. In these patients, the hypoventilation syndrome increases during exercise. The correlation between age and PaO2 is identical during rest and exercise, but the correlation between PaO2 or PaCO2 and FEV1 (% predicted) is closer during exercise than during rest. The value 50 of the FEV1 (% predicted) divides the patients into two groups: the patients who stay normocapnic and those who become hypercapnic or increase their hypercapnemia. These data show the interest of the FEV1 (% predicted) values and allow to explain the evolution of the arterial blood gas tensions during exercise.

Changes of P50 in hypoxaemia, hypercapnia and polycythaemia: multivariate analysis.

Multivariate analysis of P50 changes in hypoxia, hypercapnia and polycythaemia was performed in an heterogeneous group of forty three patients: hypoxic subjects with or without hypercapnia, with or without polycythaemia and polycythaemic subjects without hypoxia. A statistical analysis was undertaken using comparison of the means, study of the correlations, principal component analysis, multiple regression and correspondence analysis. In the patients studied, P50 changes were not wholly explained by those of 2-3 DPG and pH; PaCO2, per se, did not play an important part. Haemoglobin concentration and P50 value would represent an adaptative mechanism to hypoxia: when hypoxia is moderate (80 greater than PaO2 greater than or equal to 65 torr) and isolated, oxygen haemoglobin affinity decreases (P50 increases); when hypoxia is severe (PaO2 less than 65 torr) and combined with hypercapnia and disturbed acid-base equilibrium, P50 comes back to normal range but haemoglobin increases, restoring thus, the normal blood oxygen content.

Sarcoidosis. A clinical study with special reference to the choice of biopsy procedure.

In a 10-year review of the records of 146 young male patients suspected of sarcoidosis on the basis of radiographic intrathoracic changes, the chest X-ray findings have been analysed in relation to the results of different biopsies. Most of the patients were military men, and almost all were apparently affected by the disease. Sixty-six per cent had bilateral hilar lymphadenopathy (BHL) only, 12% had isolated pulmonary lesions and 22% had both BHL and pulmonary lesions. In all patients at least one biopsy was performed, in most cases a scalene fat pad biopsy and/or a liver biopsy. Epitheloid cell granulomas were demonstrated in 86 biopsies from 64 of the 146 patients. Granulomas were obtained more frequently by scalene fat pad biopsies than by liver biopsies in the same group of patients. During the first 4--5 years of the 10-year period the scalene fat pad biopsies were done in local anesthesia in different general surgical departments without special experience of this biopsy procedure. In the last 5--6 years the scalene fat pad biopsies were done in general anaesthesia in a head and neck surgical department specially trained in this biopsy technique. Granulomas were obtained more frequently in the latter than in the former group. The highest frequency of granulomas was found among patients with both BHL and pulmonary lesions. Of the 71 patients in whom liver biopsy was performed, 43% had granulomas in the liver. A few patients had signs--although only slight--of liver affection. The results of the different biopsies are discussed in relation to the clinical picture. It is concluded that scalene fat pad biopsy in some cases can still be of some diagnostic aid in patients with radiographic intrathoracic changes suspected of sarcoidosis, although this type of biopsy has to a great extent been replaced by biopsy through mediastinoscopy. Furthermore it is concluded that comparison of different biopsies in one and the same patient could elucidate a possible relationship between the radiographic intrathoracic manifestations and the occurrence of granulomas in different intra- and extra-thoracic tissues. In this study there were only few and insignificant complications of the biopsies.

[Statistical study on correlations between spirometric data and arterial blood gas tensions. I. Under testing conditions].

Spirometric data, resting Pao2 and Paco2 have been statistically evaluated in 152 patients with chronic obstructive bronchitis. Spirographic studies show a restrictive and obstructive syndrome, associated with hypoxaemia and hypercapnia. Partial correlations show that Pao2 is positively correlated with age and negatively with VC (predicted percentage) and with FEV1 (predicted percentage); Paco2 is only correlated with FEV1 (predicted percentage). FEV1 (predicted percentage) divides the patients into two groups, the hypercapnic and normocapnic. These data show the interest of the FEV1 (predicted percentage) values.

Silica exposure and chronic airflow limitation in pottery workers.

We assessed the relationship between exposure to silica dust and chronic airflow limitation in an epidemiological survey conducted among pottery workers and controls who were of the same socioeconomic status (average age: 35 y; 78% males). Data were collected by questionnaire for respiratory symptoms, allergy, respiratory history, smoking habits, and occupation. Lung function was measured with a computer-equipped Gauthier spirometer. We excluded subjects with silicosis or doubtful chest x-ray, and two exposure levels were defined. No differences were observed between exposed subjects and controls with respect to respiratory conditions. Mean pulmonary function values for men and women were significantly lower, after adjustment for age, height, and smoking habits, in even indirectly exposed pottery workers, compared with controls. These results suggest that exposure to silica dust is a risk factor for chronic airflow limitation and is independent of radiographic changes.

Short-term effects of low-level winter pollution on respiratory health of asthmatic adults.

We studied the short-term effects of Paris winter air pollution (i.e., sulfur dioxide, Black Smoke, suspended particulates with an aerodynamic diameter close to 10 microm, and nitrogen dioxide) in 40 nonsmoking mild to moderate asthmatics (52% male; mean age = 46 y; 90% treated with inhaled steroids). During a 6-mo period, subjects recorded asthma symptoms and three daily peak expiratory flow measurements. Statistical analysis (i.e., generalized estimating equation models that accounted for autocorrelation of responses, weather data, and time trends) revealed consistent and significant associations between the pollutants and asthma attacks and symptoms in the entire study group, especially in the subgroup of individuals who took inhaled beta2 agonists as needed. Pollutants correlated negatively with morning peak expiratory flow in the subgroup that took inhaled beta2 agonists as needed, and they correlated positively with daily variability in asthmatics who received regularly scheduled inhaled beta2 agonists. The effects lingered several days after exposure occurred. Low-level pollution has consistent measurable effects on nonsmoking adults who have well-treated mild or moderate asthma.

Analysis of the methacholine dose-response curve: usefulness of a simplified log-logistic model in epidemiological studies.

The object of the study was to find a model to summarize all the information from dose-response curves, by determining the coefficients to be used to compare groups of subjects. Three coefficients were calculated from the following model: F(d)/F(o) = ONE - k(d-delta)alpha+, where F(d)/F(o) was the ratio between FEV1 at dose (d) of methacholine and prechallenge FEV1, 'k' the slope of the relative variation of FEV1 with the dose, 'delta' the threshold dose and 'alpha' a shape factor. The model was applied to the study of hyperresponsiveness in a population of 317 men. The results illustrated the interest of this model which was applicable to 91% of the population and permitted fine discrimination of the groups studied.

[Can an improvement in pulmonary function be demonstrated in children under 10 years sent to winter sport classes?].

The purpose of this research was to appreciate the benefit of a Winter sport class for young children. The study was carried out, in the Northern suburb of Paris, among girls and boys, aged 8.4 on average. Among them, 91 went to the mountains and 65 (controls) stayed in town. A spirometry and a flow-volume curve were measured: before starting, on coming back home and 2 years later. The parents filled in questionnaires about children's respiratory symptoms and history. Morphometric data were recorded. Reference values of this population were obtained by taking height into account. No difference in respiratory symptoms and history was observed between the groups at the beginning of the survey. However, the children who went to the mountains had lower age, weight and height than the others. Nevertheless, 2 years later, these children had a more important increase of F.E.F.1 and F.E.F.25-75. They had also a more important increase of their flows.
