The natural history of colonic diverticulitis rarely includes acute solitary localization in the cecum. Complications are even more rarely seen. We report retrospectively nine cases seen in our unit. There were 5 males and 4 females, mean age 54 years. The patients presented with a more or less painful abdomen with localization in the right iliac region and signs of peritoneal irritation. Physical examination revealed a painful mass in this zone. Fever and hyperleukocytosis were not found in all patients. Barium enema demonstrated signs of pseudocompression of the cecum in four cases suggesting a tumoral formation in two and diverticulitis in the two others. A right hemicolectomy was performed in five patients as peroperative pathological diagnosis could not be ascertained on the macroscopic specimen. Diverticulectomy in the four other patients was unsuccessful in direct relation with the disease course and late diagnosis.
Among 1,855 thoracic neoplasms seen from 1971 to 1990, there were 923 with bronchogenic carcinoma (CaBr), 50%. The relation male:female was 1.95:1. Sixty three period thirty one percent were male. The histologic type were epidermoid 32.2%, adenocarcinoma 28.4% and small cells 13.2% in men; in women adenocarcinoma 38.9%, epidermoid 28.9% and mixed 7.7% with a significative difference for both sexes for these neoplasms. Other types were less frequent. There is significative difference between smokers and non smokers of both sexes p < 0.001. Epidermoid, adenocarcinoma, small cells, large cells and mixed were the most frequent in male smokers, in women these varieties were more frequent in nonsmokers. Comparison with the reference group with no CaBr suggests that epidermoid, adenocarcinoma and small cells carcinomas have a great possibility to be found in male smokers. Ninety two period two percent of cases were stage III (Tumor Node MetastasIs) with no chance for radical treatment. Only 94 were subject to surgery with 36 total resections. CaBr is an important problem in the General Hospital of Mexico. Antismoking programmes must be stressed in relation to the frequency of CaBr in smokers.
Between October 1992 and July 1994, 55 patients with gastrooesophagal reflux disease underwent laparoscopic 270 degrees posterior fundoplication. The technique used was exactly the same as for the conventional approach. There were no deaths but there were 2 peroperative complications requiring conversions to laparotomy: one oesophagal perforation and one pleural perforation. The mean operative time was 150 min, ranging from 90 to 240 min. The mean hospital stay was 2.5 days, ranging from 1 to 14 days. One patient had to be operated on the 15th postoperative day because of a dysphagia secondary to an oesophagal motility disorder. One other patient underwent a laparoscopic treatment of a bowel obstruction on 30th postoperative day. None patient had dysphagia for more than 1 month after laparoscopy. The mean postoperative follow-up was 10 months with a good clinical result in 90% of the patients at 6 months. Four of our 15 first patients complained of recurrence of reflux symptoms. These preliminary results indicate that laparoscopic Toupet procedure can be performed safely and with similar results as for the open approach if the surgeon is well trained.

BACKGROUND: The aim of the study was to describe the etiology of enteropathogenic agents over a four-year period (1992-1995) in a University Hospital in Barcelona. METHODS: We studied 12,793 stool samples, 4519 were obtained from patients under 15 years and 8274 were obtained from patients over 14 years. The specimens were examined for bacteriological, parasitological and virological enteropathogens. RESULTS: In 3380 specimens of 12,793 stool samples studied were identified an enteropathogen (26.4%). Polymicrobial associations were observed in the 6.8% of the cases. Pathogens were identified in 45% of children samples and 16.3% of adults samples. The etiological enteritis agents more frequently detected in the paediatric patients were Campylobacter (13.5%), rotavirus (11.3%) and Salmonella (10.2%); and Salmonella (4.9%), Campylobacter (3.1%) and Giardia intestinalis (2.1%) in adults. Cryptosporidium (13.5%) was the most frequent cause of gastrointestinal tract infections in HIV-infected subjects. In the children with stools positives, the presence of red and white blood cells were more frequent than the adults with stools positives (73% versus 26.6%). CONCLUSIONS: The enteropathogenic agents such as Campylobacter, Salmonella, and Giardia were the most frequent cause of gastroenteritis in our environment. In the children, rotavirus infections predominated during the cold months. The most frequent cause of gastroenteritis in HIV-infected patients was Cryptosporidium followed by Campylobacter.
