[The P300 component of evoked potentials in the evaluation of schizophrenia: new evidence and future visions].

OBJECTIVE: To analyze the character as pathophysiological marker of the P300 component in the evaluation of schizophrenic illness. We therefore review clinical and neuropsychological evidence of studies in which this was used as a tool for evaluation to reach a diagnosis. DEVELOPMENT: The P300 component is useful for studying not only cognitive processes but also the physiopathology of complex disorders such as schizophrenia. This component reflects the state of functions of attention control and memory, both necessary for final evaluation of a stimulus. These cognitive functions become altered in schizophrenic illness, as is shown by abnormality in the generation of the P300 component. We review the most relevant results, which permit characterization of the many features of schizophrenia from genetic, symptom, morphology and neuropsychological studies in which the P300 component is particularly relevant. Similarly, we analyze current controversies due to the great heterogeneity of methodology and the nature of the illness itself. CONCLUSIONS: Current understanding allows us to establish that the P300 is a biological marker of relevant aspects of schizophrenia such as the genetic risk of suffering a psychotic episode, determine the type and gravity of the symptoms of the disease, as well as the degree of structural deterioration of the patient. The P300 component is sensitive to the effect of different neuroleptic drugs, on both the symptoms and neuropsychological functions of the patient. These findings let us suggest the P300 component as a valid marker for the evaluation of schizophrenia and the neuropsychological processes.

[Acquirement of the Spanish vocabulary among rural and urban students of the 9th region].

In order to test the assumption that rural children from the Araucanía region at southern Chile start school with significant handicaps in the acquirement of the spanish vocabulary when compared with their urban peers, the Peabody Picture Vocabulary Test (PPVT, spanish version) was used to compare a first group including all those children attending third grade at a public school from a low socio economic level neighborhood of the city of Temuco, Chile (n = 44, age average 9.01 years, s = 0.51) against a second group including all those children attending the same grade at three public schools from the rural areas surrounding the same city (n = 37, age average 9.07 years, s = 0.52). Previously, 19 children aged 3 to 5 years were tested both with the PPVT and with the chilean instrument for the measurement of development at the age group (TEPSI). Both scores show a positive correlation (r = 0.64, p = 0.05). Average scores for the PPVT among rural children were lower than among urban children (64.07, s = 11.36 against 103.05, s = 10.99, p = less than 0.01). In a second stage, 39 children attending 7th grade at the urban school (age average 13.61 years, s = 0.64) and 27 children of the same grade from the rural schools (age average 13.69 years, s = 0.69) were tested with the same instrument.(ABSTRACT TRUNCATED AT 250 WORDS)

[Design of a program of quality control of nursing in an intensive care unit].

Although in the last few years care quality programs have started to be used in services, these are still few in nursing equipment. Our aim is to show the process we have followed to design a nursing care quality program in ICU. We developed the stages of process, where a list of general activities performed in the service was created to establish a priority later, as it was necessary, due to the high number of aspects or activities to monitor, to choose those we would use to start the quality control program. The ultimate order was settled with the following activities: 1. Surveillance of the infection. 2. Accidental withdrawal of tubes and catheters. 3. Pressure ulcers.

[Atherosclerosis as a physiological manifestation of aging for measuring the rate of aging].

This work studies the follow-up of vitality using parameters related to the arterial wall. This study is based on the application of several mathematical models and different calculation procedures. Humoral parameters together with measurements of the daily insulin secretion (own methodology based on the C-peptide elimination) were applied for the calculations using the exclusively healthy population. In the case of the non-selected population the percentage of disease prevalences due to vascular affections were applied for calculations.

[The dynamics of aging of an unselected population of limited representation].

Aging rates of a sample which should represent the total population living in a district of Madrid were studied using the vectorial calculation as a mathematical model. The number of individuals who finally participated was 867, homogeneously distributed by sex and age decades in the age range of 20-100 years. A total of 15 variables was applied, but they were deduced from health-related data obtained from a special elaborated questionnaire. The resulting aging factor--a dimension to evaluate the aging acceleration--agrees with the corresponding results of biological aging studies previously published. Moreover, we contend that the obtained information from the simply sent questionnaire is accurate and of biological value, and would enable studies on the aging rates of populations.

Production of soluble CD23 from peripheral blood lymphocytes of asthmatic patients.

The low-affinity receptor IgE (Fc epsilon RII) as shown to be identical to CD23 several years ago. The presence of the CD23 was demonstrated on a variety of human cells, such as T cells, monocytes, eosinophils, platelets, lymphocytes B, etc., and it can be cleaved in soluble fragments. We studied the in vitro production of soluble CD23 (sCD23) in PBMC of patients with bronchial asthma and in healthy donor cells. If we stimulate production with a polyclonal activator [Phytohemagglutinin (PHA) at a concentration of 10 micrograms/ml], it can be seen that the greatest amounts of sCD23 are produced on the fourth day and that production is greater in asthmatic patients than in control group (p < 0.01). When stimulated additionally by an antigen, the production of sCD23: Is greater when stimulated with PHA. Is released in quantities greater than in healthy group cells, when the PBMC are stimulated with the antigen to which the patients are sensitive (olive pollen). The quantity released depends on the concentration of the antigen added to the culture. Is not released in greater quantities by healthy groups cells in the presence of the antigen than in healthy group cells without the antigen. Is not released in greater quantities by cells of asthmatics in the presence of an antigen to which they are not sensitive (D. pteronyssinus). This leads us to conclude that the release of sCD23 in these patients could play a role in the physiopathology of extrinsic bronchial asthma.

Relationship of blood EG2+ eosinophils in patients with bronchial asthma.

The presence of eosinophils in peripheral blood has been previously associated with different degrees of activity in the evolution of disease in asthmatics. The eosinophil cationic protein is a mediator released by the eosinophils when they are activated due to various stimuli. The monoclonal antibody EG2 binds to human ECP, the epitopes which EG2 recognizes is only present in the secreted form of ECP, hence it only stains eosinophils which are undergoing activation and/or secretion. Flow cytometry was used to measure the level of eosinophils stained by the monoclonal antibody EG2 (eosinophil EG2+) in the peripheral blood of subjects with unstable extrinsic bronchial asthma, stable extrinsic bronchial asthma and healthy control group. A statistically significant higher level of eosinophil EG2+ was found in the group with unstable asthma than in either the group with stable asthma or the group of healthy subjects (p < 0.0002). No difference was found between the patients with stable asthma and the control group. The level of eosinophil EG2+ in peripheral blood, measured by flow cytometry, could be used as an indicator of inflammatory activity in patients suffering from bronchial asthma.
