We have investigated brainstem and cortical auditory responses (BAERs and CAERs) in 16 cases of Friedreich's ataxia (FA) and have compared the findings with those obtained in 2 cases of familial spastic paraplegia (FSP), in 5 cases of Charcot-Marie-Tooth disease (CMTD), and in 6 cases of atypical FA of uncertain classification. BAERs could not be elicited in 11 FA patients and constantly disappeared at a higher intensity threshold than in normal subjects in the remaining 5 patients. BAERs were normal or only slightly abnormal in FSP and CMTD patients. CAERs were normal in all 29 patients. BAERs tended to disappear with the progression of FA and BAER thresholds were correlated with the Inherited Ataxias Clinical Rating Scale score, which is an index of the severity of illness. BAERs contributed to the diagnosis, or exclusion of FA in patients with an atypical picture. It is suggested that in FA myelinated fibers in the spiral ganglion are partially affected, resulting in the decrease of wave amplitude such as occurs for peripheral sensory potentials.

The effectiveness of Computerized Renal Angioscintigraphy with 99m-Tc-DTPA and Ultrasound in the differential diagnosis of complications after kidney transplant was evaluated. 13 patients were studied; 6 with a normal graft, 2 with acute tubular necrosis and 5 with an acute rejection episode. Technectium scan and ultrasound diagnosis were in good agreement with clinical assessment resulting in 1.00 and 0.92 sensitivity respectively. The most valuable features of radionuclide and ultrasound techniques are: noninvasiveness, simplicity, rapidity and reproducibility. The selection of a course of treatment could be usefully helped or supported by data obtained from scintigraphy and ultrasound.
A case of renal artery stenosis due to compression by a neurofibrous cord (histologically a pheochromocytoma) and resolved by the débridment of the vessel and angioplasty with a patch in the saphenous vein, is reported. After the compression has been relieved, the vessel appeared normal at inspection and palpation (pulsation); the manometric control, recorded distally and proximally to the area of compression by the pheochromocytoma showed a significant gradient of pressure that led to the exploration of the vessel, partially obstructed by a recent thrombus. These lesions were not detectable solely on inspection and palpation. It is stressed that a restored normal aspect after removal of the extrinsic pressure on the renal artery may not represent an equivalent optimal restoration of function. Only manometric control or rate of flow measurements are able to assess the efficacy of surgical reconstruction.

A case is presented of a chronic extrapleural hematoma of the right lung apex after attempted cannulation of the internal jugular vein. Although the percutaneous cannulation of internal jugular vein enjoys wide popularity, this procedure is not without potential for serious and late complications.

30 cases of pancreaticoduodenectomy with pylorus preservation are reported. The operative mortality has been 6,6%. Technical details of the operation are discussed and the importance of maintaining an optimal vascularization of the duodenal wall is stressed. Long term results (maximum follow-up 2 years) seem to be excellent with a clear improvement of the digestive function as compared to the traditional pancreatectomy with gastric resection. Jejunal ulcerative complications have not been observed. It is believed that pylorus preservation should become the operation of choice in pancreatectomy performed for benign diseases.
Transrectal ultrasonography is a new diagnostic method recently introduced in the study of normal and pathologic pelvis. The Authors describe their preliminary experience with the use of a rectal linear probe in the evaluation of the carcinoma of the cervix. This ultrasonographic method has resulted very useful in providing an imaging of cervical cancer in evaluating its effective diffusion and in precising demeasurements of the neoplastic mass. The results are particularly encouraging and justify continued experimentation.

A case of recurrent renovascular hypertension is described in a patient with a solitary kidney successfully treated as an emergency for acute renal failure and unresponsive hypertension, by renal autotransplantation in the right iliac fossa. The results suggests that renal autotransplantation is a safe and reliable procedure in correcting particularly difficult renal vascular diseases where, due to the presence of a solitary kidney, all possible causes of technical failure of revascularization should be avoided.
