[Binucleated lymphocyte lymphocytosis].

We reported here a case of persistent polyclonal B-cell lymphocytosis (PPBL) in a 31-year-old female patient. Peripheral blood smears showed atypical binucleated lymphocytes which were polyclonal B-cells with kappa and lambda expression, and without clonal rearrangement of immunoglobulin heavy chain. Cytogenetic analysis found a trisomy 8, with premature chromosome condensation. Clinically, PPBL is associated with moderated splenomegaly, adenopathy, and smoking. It remains to be established whether PPBL is a real pathology or a simple cytological abnormality.

[Tiapride in dermatology].

The action of tiapride was studied in various dermatological conditions with predominant psychosomatic features. A total of 36 cases were evaluated and results were satisfactory in 23 patients and poor in the other 13 cases. The indications which responded best were the lichens, eczemas, psoriatic affections and neurodermatoses. Tolerance was generally satisfactory.

[Peritonitis by ruptured pyonephrosis (author's transl)].

The authors have observed one case of peritonitis by ruptured lithiasis pyonephrosis. The elective treatment of this unusual (13th case in the literature) complication of the urinary lithiasis is the nephrectomy.

[Vitamin D and pregnancy: value of a single dose in the third trimester. Comparative study of 100 cases].

A comparative study (100 treated subjects, 100 controls) was carried out at the Centre Hospitalier at Meaux during the winter of 1984-85 in which pregnant women were given single doses of vitamin D to be taken at the seventh month of pregnancy. The results obtained for neonatal blood calcium levels clearly demonstrated the value of vitamin D supplements in pregnant women. This study was original in that a single dose of vitamin D was provided; this led to almost perfect therapeutic observance (99%).

[Anatomical and clinical correlations in a case of alexia without agraphia (author's transl)].

The authors report the anatomical and clinical findings observed in a case of alexia iwthout agraphia due to left posterior cerebral artery infarction, in which there were lesions of the occipital and thalamic regions, the splenium of the corpus callosum, and the hippocampus. They discuss, more particularly, the memory difficulties, and the objects naming trouble.

[Renal arteriovenous fistula following percutaneous renal biopsy. Report of surgical treatment with preservation of the kidney (author's transl)].

Surgical correction without parenchymal injury of a high flow renal arteriovenous fistula diagnosed 5 years after a percutaneous renal biopsy is reported. An arteriovenous fistula is elicited in 16% of all percutaneous renal biopsies. Spontaneous occlusion occurs in 50 to 70% of these cases. Persistent gross hematuria in the early stage or heart failure later may require correction of the fistula.

Analysis of commercial pollen extracts by enzyme determination. II. Determination of the contamination of pollen extracts with plant matter.

The contamination of pollen extracts by plant matter was studied by means of enzymes. Beta-glucosidase was detected only in flowers and leaves by using the Api-Zym System. 29 pollen extracts were prepared after microscopic control. Among these 23 did not contain any plant matter and no beta-glucosidase activity could be detected. In contrast, 6 extracts were contaminated with plant matter and in every case beta-glucosidase activity was found 34 commercial pollen extracts completed the study. beta-glucosidase activity was present in 8 of them. It may be postulated that glucosidase activity detected using 2- naphthyl - beta D-glucopyranoside as a substrate, can be correlated with the presence of plant matter from the raw material from which the extract was prepared. This contamination may be important to determine since some additional sensitization may occur with such extracts during immunotherapy.

[Regression of retroperitoneal fibrosis treated by tamoxifen].

By analogy with desmoid tumours, tamoxifen has been used in the treatment of idiopathic retroperitoneal fibrosis. The authors report the 6th case of retroperitoneal fibrosis treated by tamoxifen alone, which had not recurred 11 months after stopping treatment.

Primary biliary cirrhosis and systemic lupus erythematosus. A new case report.

Systemic lupus erythematosus and primary biliary cirrhosis are two autoimmune disorders that rarely occur in the same patient. Ten well-documented cases have been reported to date. We add the case of a woman who was diagnosed with systemic lupus erythematosus at 54 years of age then with primary biliary cirrhosis 18 years later. Interestingly, primary biliary cirrhosis antedated the systemic lupus erythematosus in all the other reported cases. The possibility that a patient may develop more than one autoimmune disorder should be borne in mind.

[Basic cognitive-perceptive module in schizophrenics].

A specific deficit in gaze discrimination has been hypothesized for schizophrenic patients (Rosse et al., 1994). Gaze discrimination is a basic ability for animals as well as for human beings. It plays an important role in mutual control of social interactions. According to Baron-Cohen (1995), sensitivity to eye gaze relies on a specific cognitive module, the Eye Direction Detector (EDD). The author distinguishes three basic functions of the EDD; first, the EDD is involved in eyes detection; second, the EDD is used in order to establish direction of gaze, and specially to compute whether the eyes one is looking at are directed to the subject or somewhere else; third, the EDD is implied in interpretation of gaze as seeing. Rosse et al. (1994) tested subjective impressions concerning gaze discrimination in a group of schizophrenic patients. Schizophrenics reported the subjective impression of being looked at by the portraits significantly more often than controls. The authors concluded that a specific impairment in gaze detection is present in the patients, and that it may be responsible for the paranoid symptoms often reported in schizophrenia. However, it seems difficult to assert that a response bias in schizophrenics toward perceiving faces as looking at them results from the deficit of an elementary perceptual module responsible for the detection of eye-direction. Rather we suspect such a bias to be the consequence of an impairment of the more complex level of mindreading, responsible for the interpretation of gaze as seeing in terms of mental states. The aim of the present experiment was to test in a more specific way the elementary gaze discrimination system. A series of portraits of models looking at five different directions (-30 degrees, -15 degrees, 0 degree, 15 degrees, 30 degrees), have been presented to 22 schizophrenic patients and 36 normal control subjects. In each trial one portrait was presented. Subjects were asked to determine the direction of its gaze by pressing the "z-key" (left side of the keyboard) if the portrait was looking to the left, and the "/-key" (right side of the keyboard) if the portrait was looking to the right. For each trial, we recorded both the side of the response (left key or right key) and the corresponding reaction time (RT). For the purpose of the analysis, the mean numbers of left responses were computed for each subject. The mean numbers of left responses recorded for each direction of gaze did not significantly differ between patients and controls. That is schizophrenic patients are not impaired in the gaze discrimination task used in the present study. In Rosse's experiment, subjects were required to decide whether the portrait on the screen was looking at them or not. On the contrary, in our task, subjects were simply required to state whether gaze was directed to the right or to the left. No explicit judgment was required as to whom or what gaze was directed. Therefore, we can assume that the present paradigm investigated the functioning of a more basic process than that tested by Rosse et al. Our data are consistent with those reporting that basic cognitive processes are unimpaired in schizophrenia, whereas explicit processes are extensively affected.
