Biological tolerance to polydioxanone absorbable clips: a comparison with metallic ligating clips.

An experiment was conducted in 96 rats to determine the tissue tolerance to a hemostatic and absorbable clip made of polydioxanone, as compared to standard metallic clips. Clinical tolerance was similar and no bleeding or death occurred because of the material. Pathological studies showed a mild histiocytic reaction with absorbable clips and a stronger inflammatory reaction with metallic clips. This experiment indicates that absorbable clips are efficient and well tolerated and we strongly recommend the use of this material.

[What is the use of the radioimmunoassay of prostatic acid phosphatase?].

Radioimmunoassay (RIA) of prostatic acid phosphatases (PAP) using the Gammadab (R) PAP pack supplied by Clinical Assays in an effective method for determining extension of prostatic cancer. In patients with lymph node or metastatic invasion serum assay values are almost always elevated (sensitivity = 94%), especially with well differentiated cancer. Elevated levels of this variable are practically pathognomonic of external extension (specificity = 94%). This is of importance in therapy since the degree of invasion determines the choice between palliative and curative treatment. Strategy leading to this therapeutic choice can be greatly simplified and thus modified by RIA of PAP.

[Distribution of lactate dehydrogenase isoenzymes in human myocardium obtained from endomyocardial biopsies].

The distribution of lactate dehydrogenase and its isoenzymes in myocardial tissue obtained by biopsies from within the right ventricle can be studied by electrophoresis in a similar manner to that for making serum determinations. Results are comparable to radioimmunologic dosing (Peters) and microelectrophoresis (Schultheiss) and complete the findings obtained from the histological study of biopsied fragments.

Blood lines should be considered in studies of biocompatibility during acetate haemodialysis.

In this one-year prospective study, the biocompatibility of blood lines used for acetate haemodialysis treatment was evaluated in 12 patients. These blood lines, polyvinyl chloride (PVC) and polyurethane extruded PVC (PE) respectively were compared in a schedule of four alternating three-month periods of treatment: PVC/PE/PVC/PE. White blood cell count, complement, IgE and thromboxane values were recorded monthly. A reduction in white blood cell and polymorphonuclear counts after 30 minutes of dialysis was significantly less during period 2 than period 1. Pre-dialysis eosinophil counts varied in a seasonally dependent pattern. We conclude that in spite of their small area, blood lines have some effect on biocompatibility and that the season of the year has to be considered in biocompatibility studies involving eosinophils.

[Urinary prostaglandins in the hepatorenal syndrome of cirrhotic patients: role of thromboxane A2 and an imbalance of precursor polyunsaturated fatty acids].

The hepatorenal syndrome is considered to be a functional renal failure due to active renal vasoconstriction. The purpose of this work was to study the urinary elimination of prostaglandins and the plasmatic polyunsaturated fatty acid precursors of prostaglandins. The urinary elimination of PGE2 was not significantly different in the groups of patients studied: controls, group I (193 +/- 42 ng/24 h), cirrhotic patients without ascites, group II (274 +/- 43 ng/24 h), cirrhotic patients with ascites, group III (269 +/- 41 ng/24 h). The urinary elimination of PGE1 and PGF2 alpha varied in the same way as PGE2. In cirrhotics with hepatorenal syndrome (group IV) the urinary elimination of vasodilating prostaglandins was greatly decreased (p less than 0.001); PGE2 (53 +/- 16 ng/24 h), PGE1 (65 +/- 11 ng/24 h). The urinary elimination of PGF2 alpha was also decreased (293 +/- 75 ng/24 h). On the other hand, the urinary elimination of thromboxane, a vasoconstrictor, increased progressively from group I (287 +/- 75 ng/24 h) to group IV (980 +/- 266 ng/24 h) (p less than 0.05). Plasmatic arachidonic acid was significantly decreased in group IV (5.0 +/- 0.6 p. 100) compared to group I (10.0 +/- 0.6 p. 100) (p less than 0.001), to group II (10.3 +/- 0.5 p. 100) (p less than 0.001), and to group III (8.5 +/- 0.7 p. 100) (p less than 0.05). In conclusion, in hepatorenal patients, urinary excretion of a vasoconstricting prostaglandin (thromboxane) is increased while urinary excretion of vasodilating prostaglandins is decreased. This decrease could be secondary to a lack of plasmatic arachidonic acid, precursor of prostaglandins.

Influence of new hydroxylated triphenylethylene (TPE) derivatives on estradiol binding to uterine cytosol.

Twelve homologous triphenyl acrylonitrile derivatives with a p-OH or p-CH3 group on one or more of the phenyl rings were synthesized in order to assess the relative influence of each position on binding to the estrogen receptor (ER) and on inhibition of prostaglandin synthetase (PGS). Their relative binding affinities (RBAs) for [3H]estradiol (E2)-labeled ER were compared at 0 and 25 degrees C in mouse and rat uterus cytosol with those of tamoxifen derivatives, cyclofenil and diethylstilbestrol. RBAs in both species were closely correlated (r = 0.92) although the RBAs were about twice as high in the mouse as in the rat. The unsubstituted skeleton had an RBA of much less than 0.1 (estradiol = 100). An OH-group in R1 or R2 (Fig. 1) engendered very low affinity whereas an OH-group in R gave rise to a compound with an RBA equivalent to that of E2, emphasizing the importance of this position in the interaction with ER. Compounds with an additional OH-group in R1 or R2 were significantly better competitors than E2. No further increase in RBA was noted with the trihydroxy derivative. The effect of the introduction of a hydrophobic CH3-group decreased affinity as expected in R, but also in position R1 unless a second OH-group was present in R2. None of the 12 test-compounds competed significantly for binding to the "anti-estrogen binding site" in rat kidney supernatant. Although polar groups were not necessary for inhibition of PGS, inhibition was enhanced by the presence of a hydroxy group in R or R1 (but not R2). Even greater inhibition was obtained by the further introduction of a CH3-group in R1 or R respectively. The conformations of these derivatives are compared to those of known estrogen ligands and anti-inflammatory agents in order to obtain further information on these protein recognition sites.

The effect of various acrylonitriles and related compounds on prostaglandin biosynthesis.

We have studied the effect of nearly 90 arylacrylonitrile derivatives, and several related compounds, on the biosynthesis of prostaglandins from bovine seminal vesicle microsomes. This effect was compared to that of triarylacrylonitrile derivatives known for their inhibiting properties. Several arylacrylonitrile derivatives have also proved good inhibitors, especially certain N-trisubstituted compounds: trans (dimethylamino-4 phenyl)-3 (methoxy-4' phenyl)-2 acrylonitrile was the best inhibitor of the group, with an IC50 of 0.07 microM. The absence of carboxy groups in these derivatives precludes the application of any of the site-models proposed to date. We suggest that the electronic charges carried especially by the three aromatic rings play an important role in the inhibiting mechanism.

Paraneoplastic hypercalcemia associated with bladder carcinoma: report of 2 cases.

Hypercalcemia associated with bladder carcinoma is rare. We report on 2 patients with hypercalcemia and bladder tumor without bone metastases. In both patients serum calcium returned to normal after removal of the tumor. Serum immunoreactive parathyroid hormone levels were normal in both patients. In 1 case a high gradient of immunoreactive parathyroid hormone across the tumor was demonstrated but the secretion of nephrogenous cyclic adenosine monophosphate was normal. Urinary excretion of prostaglandins also was normal in this case. These data show that hypercalcemia was caused by the tumoral production of some humoral factor different from parathyroid hormone.

The control of hand movements in a case of hemianaesthesia following a parietal lesion.

A 46-year-old patient with a lesion limited to the left retrorolandic area without involvement of the prerolandic motor strip was examined. Anaesthesia to tactile, warm, cold, and painful stimuli was complete for the right hand and wrist. Position sense was abolished for the right wrist and finger joints. Performance of the right hand in motor tasks was severely impaired. Simple visual and auditory reaction times were lengthened. Sustaining a constant level of force was impossible. In the absence of visual feedback, only simple, monoarticular movements could be correctly executed; more complex movements requiring coordination between several joints, such as prehension, were poorly, or not, performed. The role of somatosensory cortex in conveying kinaesthetic input to the motor areas and the importance of vision in substituting for kinaesthetic loss, are discussed.

[Experimental study of an absorbable hemostatic clip].

A new absorbable hemostatic clip was investigated by a study in the rat. The reliability and innocuity of the clip were similar to that of the metallic clips from the clinical point of view. Its absorbability and radiotransparency qualities make it the method of choice for ensuring hemostasis and lymphostasis when long-term follow-up investigations involve modern radiological techniques such as computed tomography.

Localization without content. A tactile analogue of 'blind sight'.

We examined the ability of a patient, who had a cerebral lesion involving the left posterior hemisphere, to identify and to localize stimuli applied to her "deafferented" right upper limb. We observed a functional dissociation between localization and identification in both performance and subjective report. This finding may be a tactual analogue of "blind sight."

Conservation of RNA secondary structures in two intron families including mitochondrial-, chloroplast- and nuclear-encoded members.

Two families of fungal mitochondrial introns that include all known sequences have been recognized. These families are now extended to incorporate a plant mitochondrial intron and several introns in chloroplast- and nuclear-encoded rRNA and tRNA precursors. Members of the same family share distinctive sequence stretches and a number of potential RNA secondary structures that would bring these stretches and the intron-exon junctions into relatively close proximity. Using several of these introns which have been extensively studied by either biochemical or genetic means, an attempt is made to integrate the available data into a common picture.

Deep dysphasia: an analog of deep dyslexia in the auditory modality.

A right-handed patient, with two left hemisphere lesions, a small one in the prefrontal lobe and a larger one in the temporal, presents an unusual syndrome: a massive deficit for oral language (expression and comprehension) contrasting with a fairly good preservation of written language (expression and comprehension). The processing of isolated words and sentences has been extensively tested with repetition and dictation tasks. The patient performs rather well with nouns, verbs, and adjectives, poorly with adverbs and function words, and completely fails with nonsense words. A remarkable feature of his repetition is the frequency of semantic paraphasias. Thus, this patient exhibits a behavior rather similar to deep dyslexia, hence the possible label "deep dysphasia." The paper presents a "preunderstanding" hypothesis to account for such behaviors.

Spontaneous diabetes mellitus syndrome in the rat. IV. Immunogenetic interactions of MHC and non-MHC components of the syndrome.

We have examined the frequency of three phenotypic characteristics of the syndrome of spontaneous diabetes (overt IDDM, lymphocytic infiltration of the pancreas, and depression of T lymphocytes) in the offspring of crosses between IDDM BB rats and rats of strains with the same and different RT1 genotypes. On the basis of these observations we propose that there are at least three components of the diabetic syndrome in the rat: (1) a requirement for the RT1u haplotype from the BB strain or a gene in close linkage with the gene coding for this haplotype, (2) a susceptibility for development of insular, periductular, or intraacinar lymphocytic infiltration in the pancreas, and (3) a susceptibility to depression of T lymphocytes. Interactions between these components as well as with other genetic and environmental factors contribute to the full expression of the syndrome.
