[Monitoring the immune status of breast cancer patients under adjuvant chemotherapy and polychemotherapy].

The effects of various combinations of chemotherapy on the immunological status of breast cancer patients were investigated. The proportion of T cells and B cells in the peripheral blood of the patients were followed and the reactivity of lymphocytes to the mitogen phytohemagglutinin (PHA) was determined. During observation periods of 6 months it was found that patients under adjuvant chemotherapy developed only a transitory depression of B cells and T cells as well as of PHA-induced lymphocyte stimulation. However, patients under polychemotherapy showed a marked depression of these immunological markers and functions during the whole observation period. The possibility of an improved monitoring of chemotherapy in cancer patients by observation of these immunological parameters is discussed.

[Electrophoresis mobility inhibition test (emt) in immunological early diagnosis of gynaecological malignomas (author's transl)].

The electrophoresis mobility inhibition test (EMT) was employed in the examination of a total of 74 woman patients with various gynaecological malignomas, 50 women with non-malignant diseases and 41 healthy female controls. In the group of patients with malignoma, 78.4% of the test results agreed with the histological diagnosis, 21.6% of the results were false negative. In the group of patients with benign diseases, 78.0% of the test results were correct, whereas 22.0% were false-positive. One false-positive test result was seen among the healthy controls. Extensive tumour growth, chronic infections and virus diseases seem to falsify the test result. Significance, perspectives and limitations of this test system within the framework of tumour diagnosis, are discussed.

[The influence of pregnancy specific serum factors on the mitogen induced culture of lymphocytes (author's transl)].

The addition of 10% serum of pregnant patients to a culture of lymphocytes from healthy non-pregnant control persons inhibits the stimulation of these lymphocytes by the mitogens phytohemagglutinin (PHA), pokeweed mitogen (PWM) and concanvalin A (Con A). The inhibitory effect increases with increasing duration of pregnancy. Sera from pregnant patients which already inhibit during the first trimester of pregnancy show an increase of this effect with increasing length of gestation. The influence of several hormones and proteins was investigated in order to detect a factor in the pregnant serum responsible for the inhibition of the lymphocyte culture. Compounds which are known to increase during gestation were added as pure compounds in various concentrations to the lymphocyte culture. An inhibitory effect on the lymphocyte culture in vitro was shown dependent on the amount, by human placental lactogen, alpha-fetoprotein, human chorionic gonadotropin, cortisol, progesterone and SPI glycoprotein and alpha-2-macroglobulin. The estrogens estriol and estradiol in physiologic dosages did not show an inhibitory effect and showed a stimulatory effect in higher dosages.

The lecithin/spingomyelin quotient as determined using a modified method and human placental lactogen in the amniotic fluid.

Thirty amniotic fluid samples in late gestational age were analysed for HPL-values and the L/S-ratio. The pregnant women used in this study were healthy except three patients who developed a rh-incompatibility. The amniotic fluid was only obtained by abdominal amniocentesis. We found a decreasing tendency of the HPL-values in late pregnancy. The L/S-ratio increased. A significant correlation of both values could not be observed. In all three cases of rh-incompatibility the decrease of HPL and the increase of the L/S-ratio occurred obviously earlier and steeper. Further studies will prove the significance of these results.

[Alpha-1 fetoprotein in maternal serum (author's transl)].

Serum alpha-1 fetoprotein (AFP) had been determined by radioimmunoassay in 578 patients. During normal pregnancy the AFP concentration increases continuously till 32nd week of pregnancy while thereafter a decrease has been observed. Values of AFP which were found beyond standard deviation indicate fetal distress. Further studies will contribute to the significance of AFP for monitoring normal pregnancies, determination of gestational age and diagnosis of fetal distress in cases of high risk pregnancies.

[Family planning and pregnancy with thalassemia major].

We present the case of a young Italian woman with thalassemia major and describe a series of complications such as infertility, fetal growth restriction, difficult intubation, and cardiac decompensation that occurred in the course of family planning, pregnancy, and the puerperal period. By review of the literature and based on own experiences, we show how the preconceptional consultation and the care during a pregnancy, especially the individualized planning of delivery, can be optimized. The intensive and interdisciplinary care by obstetricians and internists is very important.
