[Is systematic syphilis serology still useful in a department of internal medicine? A one-year prospective study using a diagnostic and therapeutic decision algorithm].

During one year syphilis serology was systematically studied, using the TPHA and VDRL tests, in each of the 1,279 patients hospitalized in an Internal Medicine department. In all cases diagnosis and treatment were analyzed by means of a decision algorithm. Only 37 patients were found to have one or both serological tests positive. No evolutive syphilis was observed, and none of these positive tests was contributive to the diagnosis of another disease. Only 14 patients received a specific antibiotic treatment on the grounds that their positive test confirmed a late asymptomatic syphilis. We conclude that systematic syphilis serological tests are not useful in an Internal Medicine department, except in some patients epidemiologically at high risk of syphilis.

[Successful treatment of ventricular tachycardia by transvenous electrical ablation of the right Tawara branch].

Recurrent episodes of ventricular tachycardia not responding to medical treatment occurred in a 56 year old man. Electrophysiological investigation showed ventricular tachycardia due to bundle branch reentry. Using a method similar to catheter ablation of the atrioventricular junction an ablation of the right bundle branch was performed by an electrical shock of 250 J. While before the ablation ventricular tachycardia occurred several times a day, and its induction by programmed ventricular stimulation was facilitated by the administration of antiarrhythmic drugs, no initiation of ventricular tachycardia was possible after ablation of the right bundle branch. Over a follow-up of 4 weeks the patient has not suffered from tachycardia and the artificial right bundle branch block persists.

Queuosine modification of the wobble base in tRNAHis influences 'in vivo' decoding properties.

The 'in vivo' decoding properties of four tRNAHis isoacceptors, two from Drosophila melanogaster and two from brewer's yeast, were studied after their microinjection, along with turnip yellow mosaic virus (TYMV) coat protein mRNA, into Xenopus laevis oocytes. The two Drosophila isoacceptors are identical besides containing either a guanosine (G) or the hypermodified nucleoside queuosine (Q) in the wobble position. The brewer's yeast isoacceptors differ by four bases in the anticodon stem, and by one base in the amino acceptor stem. Our results show that, under competing 'in vivo' conditions, the Drosophila tRNAHis with the anticodon GUG clearly prefers the histidine codon CAC to the codon CAU, whereas little preference is observed for the tRNAHis with the anticodon QUG for the codon CAU, and no preference for either codon by the two yeast isoacceptors. Hence, it can be concluded that the presence of the Q-base clearly affects the choice of the codon. This is the first demonstration of an 'in vivo' codon preference by tRNA isoacceptors differing in the modification of the wobble base during the elongation step of protein synthesis. These results imply that one function of the Q-base is at the translational level.

[The effect of low-dose heparin therapy on patients in an internal medicine intensive care unit].

It is reported on 191 patients of an intensive care unit, in whom for the purpose of the prophylaxis of thromboses and thromboembolic complications a low-dose heparinisation was carried out. For comparison served the documents of 137 patients who were admitted to the same department for homogeneous reasons but had not been treated prophylactically with anticoagulants. In no case more massive haemorrhages developed which might have compelled us to interrupt the therapy. In spite of the low-dose heparinisation 4 pulmonary embolisms as well as thromboses of the abdominal aorta, the radial artery and the coronary arteries were the result; they were confirmed autoptically . Thus the low dose heparinisation does not represent a sufficient protection against the thromboembolic complications for patients with internal diseases who are confined to bed. The demand is made to establish the individual need of heparin and to adjust the heparin dose according to this.

Sensitization potential of benzotriazole.

Predictive guinea pig sensitization studies have been performed with commercial and purified 1H-benzotriazole. In only 3 animals, induced and challenged according to the maximization protocol with more than 100 times the normal use concentration, were slight erythemas elicited with the commercial grade of benzotriazole. The optimization test with commercial and purified materials, as well as the maximization test with the purified material, was negative. We conclude that benzotriazole has a very low sensitization potential.

[Relation between atmospheric events and internal medicine emergency situations].

Over a period of 2,541 days (7 years) patients with the diagnosis groups acute oesophageal and gastrointestinal haemorrhage, respectively (n = 395), cerebrovascular insufficiency (n = 689) and acute myocardial infarction (n = 701) were registered. From the individual meteorological data registered in the same period the temperature the evaporation warmth of the water, the specific warmth capacity of the air, the air pressure and a constant calculation factor were used for the calculation of the equivalent temperature. Thus by its means a measure for the warmth content of the air was at our disposal which is determined by a series of factors. Between the emergency situations mentioned and the equivalent temperature established a correlation test for significance was performed. In these cases was the result that there are significant relations between the cerebrovascular insufficiency. Though already a larger number of meteorological parameters entered into our investigations, the result supposes that further important meteorological data should not be neglected, such as phases of weather and front transits. This shall be taken into consideration in future investigations.

Atmospheric Pressure

[Detoxication by hemoperfusion].

In 13 patients (2 males, 11 females, 15-70 years of age) on account of severe intoxications with hypnotics sedatives, psychopharmaca, in most cases mixed intoxications, with propranolol and halogenized hydrocarbons a 4--8-hour haemoperfusion treatment with amberlite XAD-4-Resin was performed. 11 patients survived, 1 patient died in irreversible cardiogenic shock of a propranolol intoxication, another patient of the sequels of a crotylbarbital and methaqualone intoxication. The curves of the course of the blood concentration of the individual substances showed a good elimination for phenobarbital and crotylbarbital, methaqualone, meprobamat and didropyridine as well as trichlorethylene, a less good elimination for nitrazepam, propranolol and tetrachlorethylene, Altogether the effectiveness of the detoxication clearly higher in the time unit, compared with the dialysis.

[The transesophageal approach to the heart in electrotherapy of trachycardiac arrhythmias. I. Transesophageal electrostimulation procedure].

Transoesophageal electrostimulation techniques were used in 124 cases of different tachycardiac disturbances of rhythm. By means of highly frequent transoesophageal atrial stimulation we succeeded in transformating into sinus rhythm in 46% of the patients with atrial flutter, in 29% of the cases with atrial tachycardias and in 75% of the patients with av-junctional tachycardias. At least atrial fibrillation (with decrease of the ventricular frequency) could be induced in 48% in pre-existing auricular flutter, in 38% in auricular tachycardias and in 15% in av-junctional atrial (partial success of therapy). Sinus tachycardias, atrial fibrillation and ventricular tachycardias could practically not be influenced by highly frequent transoesophageal atrial stimulation. Moreover, for the first time the techniques of the doubled or coupled atrial stimulation, in 2 cases also ventricular stimulations with higher frequency were tested on transoesophageal way and were introduced into the treatment of tachycardiac disturbances of rhythm. The therapeutic results of transoesophageal electrostimulation techniques seem to be comparable entirely with those of intracardiac stimulations, in which cases the non-invasive and uncomplicated techniques are accessible also to institutions without possibilities of heart catheterization.

[Infusion therapy and parenteral feeding in acute pancreatitis].

The acute pancreatitis can be a life-threatening disease and can demand the performance of intensive-therapeutic measures. The central position there in occupy the infusion therapy and the parenteral nutrition. On account of the increased catabolism already in the initial phase of the acute pancreatitis the supply of suitable carriers of calories in adequate quantity is necessary. The indication of glucose and the differential indication of fructose and Alvesin "Neu" are discussed. In the treatment of shock human albumin, plasma and plasma substitutes as well as blood take the precedence. Apart from the diagnostic and therapeutic value of the peritoneal lavage in the acute pancreatitis is possesses the additional effect of the dialysis in acute renal failure.
