Polyoma large T can activate middle T expression by a hit-and-run mechanism.

We studied the activation of polyoma middle T expression in revertant cells carrying transcriptionally inactive copies of the middle T (pmt) oncogene. Introduction of polyoma large T with neo into a rat cell line containing multiple copies of pmt stably integrated into the genome corrected the transformation defect in some of the transfected cells by activating the resident pmt gene. However, once the cells were transformed, continuous expression of the large T protein was not required for the maintenance of pmt expression and hence the maintenance of the transformed state. Transformants arising spontaneously as well as those induced by large T exhibited frequent rearrangements of the pmt inserts. Our results suggest that large T activated pmt expression by a hit-and-run mechanism involving recombination of sequences in the viral insert.

High-frequency changes in transcriptional activity in polyomavirus-transformed cell lines.

We applied the Luria and Delbruck fluctuation test to analyze high-frequency changes in the phenotype of rat cells transformed by a plasmid carrying the polyomavirus middle T (pmt) gene. All of the transformed cell lines analyzed were capable of switching to the normal state with rates ranging from 10(-3) to 10(-2) per cell per generation. Analysis of both middle T antigen and middle T transcripts indicated that the reversion occurred by a mechanism involving a transcriptional block of the pmt locus. Cell lines containing two separate loci reverted with a lower rate, suggesting that phenotypic switching in these cells involved two independent events affecting each locus. The flat revertants mutated to the transformed state with rates in the range of 10(-5) to 5 X 10(-5) per cell per generation. To determine whether changes in pmt expression would affect neighboring sequences, we transfected a hybrid plasmid carrying pmt linked to the neo marker and selected either for morphological transformants or for G418-resistant cells. Although their coordinate regulation was not absolute, both genes were usually subject to the same changes, reflected by loss and reacquisition of transcriptional activity.

Short-term prophylaxis with cefotaxime in prostatic surgery.

During a 20 month period 167 patients undergoing transurethral resection of prostate were divided into two groups: a control group without prophylactic antibiotics and a treated group who received cefotaxime 3 g intravenously perioperatively. In the first seven post-operative days 50 patients in the control group had a urinary tract infection (greater than 10(5) cfu/ml) compared with 14 patients in the treated group (P less than 0.0001). Two blood cultures were positive, both in the controls. A full course of antibiotics was administered to 55 in the control group and 14 in the treated group. On the thirtieth post-operative day 24 patients had urinary infection in the control group compared with 5 in the treated group (P less than 0.0001). We conclude that this short-term prophylaxis with cefotaxime significantly reduced the rate of urinary infection after transurethral prostatectomy.

[Arterial hypertension induced by a nasal decongestant. Value of the use of an automatic and programmable recording technic].

The case of a twenty-three-year-old woman with spasmodic coryza in whom the overuse of a nasal decongestant resulted in arterial hypertension is reported. The relationship between the treatment and the occurrence of hypertension is firmly established. The original feature of this case is the use of the technique for monitoring the blood pressure and heart rate, before and after the treatment had been discontinued, which ascertained the hypertension, making it appear as an experimental model.

Endoscopic internal urethrotomy for treatment of urethral strictures: midterm survey.

Success in the management of urethral strictures can be claimed only after many years, patients sometimes faring well for ten years or more before suffering a recurrence. We used endoscopic urethrotomy as primary treatment for a variety of urethral strictures in 123 patients. Follow-up is over two years for 63 patients and more than five years for 18. Over-all success rate at five years was 76 per cent. Failures are twice as frequent in strictures, regardless of their origin, involving the anterior urethra than in the bulbar or posterior urethra. Two-thirds occurred in the first year of follow-up, but, as in other techniques, recurrence can occur after four years.

[Urinary complications and sequelae of carcinoma of the cervix treatment (author's transl)].

The authors describe the sequelae and urinary complications of:--radiotherapy only in 524 patients, stage I to III treated between 1960 and 1974;--721 operations performed between 1968 and 1974, according to the protocol of treatment as used in the Fondation Curie. This reveals:--for irradiated patients, no iatrogenic damage to the upper urinary tract, whilst bladder complications were often present. No major complication was found in this group;--for operated patients, the surgical act was without harmful consequence in the 672 cases when the operation was foreseen in the initial protocol (0,6% of chief complications). On the other hand, surgery performed for persistent tumour or recurrence after exclusive radiotherapy showed 8 per cent of chief complications. In this group they analyse the factors that might influence ureterohydronephroses, especially the extent of node dissection and associated external irradiation.

[Urinary complications of the resectable cancers of the cervix uteri (author's transl)].

The authors studied the urinary complications following 721 operations performed between 1960 and 1974 for cervix uterin carcinoma. The surgical method was the one used at the Fondation Curie according to the protocol of treatment. They have noted that: --first, the surgical act as primary treatment (672 cases) was innocuous, as this series showed only 0,6% of the severe complications; in contrast, when the surgery is performed for a recurrence after a total dose irradiation, the severe complications raised to 8%; --secondly, the uretero-hydronephroses following this type of surgery are relatively frequent on the systematic urograms, but most of them are quite asymptomatic and without later sequelae only 0,3% of them required a surgical treatment. The authors analyse the factors influencing the ureteral stenoses, especially the extent of lymphadenectomy and the associated external irradiation. They studied too the ureteral stenoses due to a local recurrence.
