Response of insulin, glucagon and growth hormone to arginine infusion in patients with chronic renal failure.

To determine whether glucose intolerance in patients with chronic renal failure could improve by hemodialysis, the effects of arginine infusion on the concentration of blood sugar, insulin, glucagon, growth hormone were examined in healthy volunteers, undialyzed and dialyzed patients with chronic renal failure. Plasma concentrations of sugar and hormones in undialyzed and dialyzed patients responded similarly to arginine infusion. While blood samples were collected at 30, 45, 60, 90, 120 and 180 min after beginning infusion of arginine, the concentrations of sugar and hormones in both patients had no statistically significant differences. However, plasma concentrations of growth hormone in both patients 180 min after beginning of arginine infusion gave statistically significant differences. In the present study, the results suggest that hemodialysis might not improve the glucose tolerance in the patients with chronic renal failure.

Water soluble vitamins in patients with chronic renal failure and effect of B6 administration of immunological activity.

Blood concentrations of water soluble vitamins were studied in 29 undialyzed and 35 dialyzed patients with CRF, and 36 healthy volunteers. Effects of B6 administration on immunological parameters were studied in dialyzed patients. In dialyzed patients, whole blood B1 decreased, while plasma B2, B6 and serum B12 and folic acid increased. In undialyzed patients with uremia, plasma B2, serum B12 and folic acid increased, while plasma C decreased in patients with moderate CRF. Oral administration of B6 for 4 wks was associated with improved tuberculin skin tests and PHA mitogen responses in dialyzed patients. Supplementation of B1 is required for patients with CRF while B6 and C may be considered.

A case of renal transplant recipient complicated with cryptococcosis and amphotericin B induced acute tubular necrosis.

An adult renal transplant recipient was complicated with cryptococcal lung granuloma and meningitis. Treatment with the antifungal agents, 5-fluorocytocin and clotrimazole had to be discontinued due to side effects. Whereas, the intrathecal administration of amphotericin B proved effective for meningitis but intravenously it induced acute tubular necrosis to the transplanted kidney. In order to cure the persistant fungal lung granulomas in renal transplant patients early surgical excision seems to be essential.

The inhibitory effect of reserpine on the active sodium transport across the frog bladder.

1. The effects of reserpine and harman derivatives on the sodium transport across the frog bladder were examined using a short-circuit current method. The effects of harman derivatives on the Na,K-ATPase activity of the frog kidney were also investigated. 2. Reserpine and harman derivatives inhibited active sodium transport of the frog bladder and their inhibitory effect decreased as reserpine greater than harmine greater than harmaline = harman greater than harmalol. 3. Harman derivatives inhibited Na,K-ATPase activity of the frog kidney. 4. These results suggest that reserpine and harman derivatives inhibit active sodium transport by suppressing the Na,K-ATPase activity of the frog bladder.
