[Radioimmunologic determination and analysis for saturation of the thyroid hormones: comparison and use in the practical clinic].

Reliability of a recent method of assay by saturation analysis for assessment of thyroid status, which is to be recommended for its practicability and technical improvement, has been verified. Its diagnostic efficiency in more than 3,000 assays has been evaluated. On the whole T3 Uptake (T3U) and serum Thyroxine (T4D) determination showed a quite good accuracy, that decidely improved when both the tests were carried out and the Free Thyroxine Index (Free Thyopac Index) was drown. Moreover the present availability of the radioimmunological determinations of triiodothyronine allowed to assay simultaneously both thyroid hormones in normal euthyroid subjects, in pregnancy, in hyperthyroid, hypothyroid and goitrous patients. Results obtained have been analysed and discussed and clinical conditions pointend out where determination of both hormones may be useful.

Changes in thyroid status in newborn infants utero-exposed to ritodrine.

A clinical study to assess whether utero exposure to ritodrine influences thyroid status was performed in 21 healthy term newborn infants, 10 exposed in utero to ritodrine (treated group) and 11 non-exposed in utero to drugs (control group). The treated group had a T3/T4 ratio significantly higher than the control group (mean +/- SD: 124.10 +/- 23.70 vs. 96.09 +/- 18.11, p less than 0.005) and T3 slightly increased (mean +/- SD nmol/l 2.48 +/- 0.69 vs. 1.95 +/- 0.56). The mean serum values of the other parameters studied, TSH, T4, fT4, fT3, were not significantly different in either groups. Since beta-mimetics induces deiodinating activity of the liver and propranolol reduces extrathyroidal conversion of T4 to T3, it was suggested that ritodrine enhances deiondinating activity of fetal and neonatal liver.

Amoxycillin and clavulanic acid in the treatment of urinary tract infections in children.

The efficacy of amoxycillin-clavulanic acid combination in the treatment of urinary tract infections resistant, in vitro, to amoxycillin was studied in 42 children. Of the 24 children with urinary tract infection for the first time, combination therapy, dosing twice daily for 5 days (40 mg/kg.day), cleared the infection in 23 (96%) cases. Relapse occurred in four (17%) cases within 30 days. Of the 18 children who presented with recurrent urinary tract infections therapy, as above, cleared the infection in 16 (89%) cases. In these cases, long-term therapy was performed at a dosage of 20 mg/kg once daily. Tolerance was good; gastro-intestinal disorders in five (12%) cases which regressed by dosing at 8 h rather than 12 h intervals. In conclusion, amoxycillin-clavulanic acid can be considered a first choice treatment of urinary tract infections in children.

[Diabetes and atherosclerotic coronary damage in uremia].

BACKGROUND: Cardiovascular risk factors are common findings in uraemics, but the impact of each single factor on the development of atherosclerosis is still a matter of debate. PATIENTS AND METHODS: In order to evaluate the relationship between diabetes and ischaemic heart disease (IHD) in uraemia, we carried out a retrospective study comparing the results of 33 coronary angiographies performed in non-diabetic patients with those of 13 diabetics (2 had type 1 diabetes, 8 were treated with insulin, 2 with sulfonylureas and 3 received no therapy). Coronary angiography was performed in 29 patients awaiting kidney transplantation and in 17 subjects with IHD. RESULTS: Age, sex, length of time on renal replacement therapy, smoking history, clinical diagnosis of cerebrovascular and peripheral vascular disease, systolic blood pressure (BP), cholesterol, triglycerides, calcium, phosphate, albumin and degree of anaemia were comparable in the two groups. On the contrary, frequency of IHD (77 vs. 30%, p<0.01) and atrial fibrillation (23 vs. 3%, p<0.05) were higher, while diastolic BP (79 +/- 7 vs. 85 +/- 8 mmHg, p<0.05) and calcium phosphate product (47 +/- 10 vs. 57 +/- 15 mg2/dL2, p<0.05) were lower in diabetics than in non-diabetics. Stenotic lesions of the three major coronary arteries were more prevalent in diabetics than in non-diabetics (left anterior descending artery (LAD) 100 vs. 48%, p<0.01; right coronary artery (RCA) 77 vs. 39%, p<0.05; left circumflex artery (LCA) 69 vs. 24%, p<0.01) and in the same way diabetics showed higher narrowing percentage (LAD 74 +/- 30 vs. 30 +/- 36%, p<0.01; RCA 71 +/- 41 vs. 26 +/- 38, p<0.01; LCA 41 +/- 38 vs. 15 +/- 29, p<0.05). CONCLUSIONS: Our study demonstrates that although the uraemic milieu is a risk factor for IHD, diabetes increases the degree of atherosclerotic vascular damage independently of the other cardiovascular risk factors.

[The normal aspect of the suprapatellar bursa in echography of the knee].

Sonography is suitable to recognize fluids in the suprapatellar bursa, which can represent an indirect sign of intra-articular knee effusion. In order to obtain the physiologic parameters of suprapatellar bursa, 50 healthy volunteers, 38 sedentary and 12 sportsmen, were evaluated by means of US. The bursa was recognizable in 8/50 cases (16%) with quadriceps in prolapse (mean thickness: 1.8 mm, mean length: 20.5 mm), and in 23/50 cases (46%) with quadriceps in contraction (mean thickness: 2.6 mm, mean length: 22.6 mm). Suprapatellar bursal thickness ranged 1-4 mm, and its length ranged 12-44 mm. The suprapatellar bursa appeared as a well-defined hypoechoic band, cranial to patella, anterior to the distal femoral shaft, and posterior to quadriceps tendon. Quadriceps contraction increases both thickness and length of bursal swelling (1 and 2 mm, respectively, on the average). When sportsmen were compared with sedentary subjects, suprapatellar bursa diameters were observed not to change, but the bursal itself was evident in 3/12 (25%) and in 8/12 (66%) of cases, according to whether the quadriceps was in prolapse or in contraction. Bursal contents were homogeneously anechoic and bursal outline smooth and regular in all cases (23/23). Bursal morphology was symmetric, in 23/23 cases, in length, regularity of outline, and echogenicity of contents. Thickness alone resulted asymmetrical in 2/23 subjects (8%), even though variations were less than or equal to 2 mm. In conclusion, we can consider as pathologic features: thickness greater than 3-4 mm, asymmetry greater than 2 mm, irregular outline, and inhomogeneous contents. These parameters must be examined in pathologic subjects and further research is mandatory.

Percutaneous endoureterotomy of uretero-ileal stenosis in the Camey-Le Duc substitution bladder. Our experience in two cases.

Two patients with a Camey-Le Duc substitution bladder who presented monolateral uretero-ileal stenosis were treated by percutaneous endoureteral incision using electrocautery and moulding stents. The results were encouraging in one patient after 12 months follow-up. In the other case, however, endourological manipulation had solely symptomatic purpose since the stenosis was caused by local recurrence of primitive neoplasm.

Experience with our first 400 treatments with ESWL and URS.

From September 1988 to April 1989, 400 patients with stones in the calix (40%), in the renal pelvis (45%), in the ureter (15%) and with staghorn calculi (5%), underwent shock wave treatment. In the majority of patients therapy was carried out with general anesthesia. Disintegration was achieved in 95% of stones in the kidney and 44% of stones localized in the ureter following in situ ESWL. The ureteronephroscopy and stone push-up with replacement into the renal pelvis was performed in 56% of ureteral calculi. The average number of impulses to achieve disintegration was 1,850. In 20 patients 4,000 shock waves were necessary. We studied these patients with echotomography, CT scan and magnetic resonance imaging 48 hours after the treatment. The aim of this trial was to assess the efficacy and cost benefit ratio of the Tripter X1 lithotripter.

Combined therapy of staghorn calculi with ureteronephroscopy (URS) and extracorporeal shock wave lithotripsy (ESWL).

We report our experience with 25 cases of staghorn calculi treated by combined ureteronephroscopy and extracorporeal shock wave lithotripsy. The equipment used and the ultrasonic disintegration techniques are described. We believe that the procedure, followed by ESWL, may constitute, in selected cases, an alternative to conventional combination therapy.

Quality of life in patients with advanced bladder cancer treated by tumorectomy and systemic adjuvant polychemotherapy (M-VAC). Our experience in 14 cases.

Fourteen patients with advanced bladder cancer T3 to T4, N1 to N2 positive, were treated with reductive surgery (tumorectomy) and systemic adjuvant polychemotherapy (M-VAC: methotrexate 30 mg/m2, vinblastine 3 mg/m2, adriamycin 30 mg/m2 and cis-platin 70 mg/m2) in the period from December 1985 to December 1987. The aim of this combination was to preserve detrusor function in order to give tha patients a better "quality of life". After a follow-up varying from 12-24 months, complete clinical remission for 18 months was observed in 2 patients, the remainder showed an improvement of the performance status compared with those treated by conventional therapy (radical cystectomy and/or radiotherapy) but no changes in overall survival was achieved.
