Airway response to inhaled histamine in asymptomatic long-term marijuana smokers.

Bronchial challenge with histamine was used to assess bronchial reactivity in asymptomatic individuals who were long-term social smokers of marijuana. Their reactivity was compared to that of nonsmokers and asthmatics. Spirometry results were normal in the marijuana users. There was no significant difference in bronchial reactivity between marijuana smokers and nonsmoking controls, whereas the asthmatics demonstrated the expected hyperreactivity. These observations suggest that customary social use of marijuana may not produce abnormalities in airway function detectable by spirometry or bronchoprovocation.

The role of IgE in childhood asthma.

The influence of age and sex has been in 208 asthmatic children on the basis of the following parameters: total serum IgE concentration (RIST), serum concentration of specific IgE antibodies (RAST), and positive prick tests against Dermatophagoides Pleronissynus, grasses, and Alternaria Tenuis. The results indicate that all these parameters are significantly influenced by age. Arithmetic values for RIST show a significant difference between the sexes which is obscured by logarithmic calculation. The data strongly suggest that IgE antibodies cannot be considered the main mechanism by which asthma is produced in small children.

Simultaneous Hodgkin's disease in three siblings with identical HLA-genotype.

We report 3 cases of Hodgkin's disease (HD) within a family of seven children. All three children had mixed cellularity type in different stages of the disease (IV-B, III-A, and II-A). The diagnosis was made within a one-month interval. HLA typing shows an identical genotype in the 3 HD patients, which was different from the rest of the family. One of the patients developed an idiopathic thrombocytopenic purpura (ITP) with temporary response to prednisone and MOPP-C chemotherapy that subsided after splenectomy. The HLA identity, in addition to the simultaneous onset of the HD, suggests a combination of both genetic and environmental factors in the pathogenesis of this disease.

Cardiac arrest in the Emergency Medical Service System: guidelines for resuscitation.

A two-year study of 198 consecutive patients treated for cardiac arrest in the emergency department at Stanford University Medical Center was undertaken. The relatively poor overall survival rate of 3% and the complexity of deciding how to treat cardiac arrest victims suggest the need for guidelines to assist the emergency physician when resuscitating cardiac arrest patients. From the above study and a survey of the literature, the authors formulated the following guidelines of when resuscitation should be discontinued or not attempted: Cases of apnea and pulselessness known to have exceeded 10 minutes, no response after more than 30 minutes of advanced cardiac life support (ACLS), no ventricular EKG activity after more than 10 minutes of ACLS, and preexisting terminal illness.

Polysplenia with left ventricular hypoplasia and partial anomalous pulmonary venous connection.

A case is reported of left ventricular hypoplasia associated with anisosplenia. Even though congenital heart malformations are frequently found in cases of splenic anomalies, the association of polysplenia with hypoplastic left heart syndrome is extremely rare. The existence in addition of partial anomalous pulmonary venous connection of the right pulmonary veins to the right atrium, gave us the possibility of study the different changes in pulmonary arteries wall thickness in both lungs.

Continuous sutures in replacement of mitral and tricuspid valves with prostheses.

Four years' experience with 363 patients, in whom 360 mitral and 65 tricuspid prosthetic valves have been implanted with single continuous sutures, is presented. The occurrence of prosthetic valve detachment is particularly discussed. There were 4 cases of detachment of the prosthetic mitral valve and no cases of tricupsid dehiscence. All 4 patients with detached valves required reoperation. Calcifications that penetrate into the annulus and myxomatous degeneration of the valve are considered more important factors in the production of prosthetic cases so long as a uniform distribution of stitches is maintained (not too close to each other) and excessive tension in the suture line is avoided.

Ethanol and cytokine secretion.

Cytokines are regulatory polypeptides secreted during the generation of an immune or inflammatory response by lymphocytes, cells of the monocyte/macrophage series, and a variety of other cell types. Alterations in the production, site of action, or metabolism of cytokines by exogenous factors, such as ethanol (EtOH), may have deleterious effects on the immune system as a whole. EtOH has been implicated in the onset of a variety of immune defects in vivo including effects on the production of cytokines critically involved in inflammatory responses (tumor necrosis factor, interleukin 1 and interleukin 6). In this review, we examine current knowledge regarding the effects of EtOH on the release of cytokines in humans and in animal models, in vitro and in vivo, which may help to elucidate the adverse actions of EtOH on mammalian immune systems.

Synthesis and expression of the gene coding for noxiustoxin, a K+ channel-blocking peptide from the venom of the scorpion Centruroides noxius.

A set of six synthetic overlapping oligonucleotides coding for noxiustoxin were coupled into a continuous DNA fragment by means of recursive polymerase chain reaction. The polymerase chain reaction product was digested with SalI and HindIII, ligated into the E, coli vector pCSP 105 and expressed as a fusion protein. The fusion protein was purified and digested with trypsin and the hydrolysis products were separated by high-performance liquid chromatography. Approximately 1.3 mg of recombinant noxiustoxin per liter of culture was obtained. Amino acid analysis and N-terminal amino acid sequence of the recombinant noxiustoxin confirmed the nucleotide sequence of the cloned DNA. Binding experiments using rat brain synaptosomal membranes revealed that recombinant noxiustoxin displaced bound radioactive native NTX with a similar efficiency to cold native noxiustoxin.

Cocaine metabolite (benzoylecgonine) in hair and urine of drug users.

Two methods of drug detection, urinalysis and hair analysis, were compared with respect to the efficiency of identification of drug use in a population of men living on the Arizona-Mexico border. The standard curve of cannabinoids in urine was linear to 20 ng/mL. The GC/MS levels for all cannabinoids combined in urine were very similar to that obtained by radioimmunoassay (RIA), 91% concordance. Similar results were obtained from samples analyzed dually for the cocaine metabolite benzoylecgonine (BE) after spiking. As determined by RIA of urine, 74% of the subjects were positive for cannabinoids. The majority were in the range of 100-1000 ng/mg creatinine. The pattern of excretion of THC metabolites with respect to the verbally reported time of first use was fairly normal, with the peak rate of elimination 13-24 hours following the last reported use. Washed hair samples were extracted by overnight acid hydrolysis. Urine samples and neutralized hair extracts were analyzed for cocaine and BE by RIA. Of the hair samples, 55% contained cocaine/BE, as compared with only 4.3% of the urine samples. Most hair samples contained cocaine/BE in the range of 25-100 ng/sample (100 mg hair). All hair samples testing negative for cocaine/BE by RIA also tested negative by GC/MS, and four samples containing the highest amounts of cocaine and BE by RIA were similarly found to contain the highest amounts by GC/MS. Hair analysis, therefore, gives a wider window of detection of drug use than does urinalysis and shows merit in the confirmation of cocaine use in small clinical research studies.
