Determination of 6-N-trimethyllysine in urine by high-performance liquid chromatography.

A method for determination of 6-N-trimethyllysine in urine is described. Trimethyllysine and the chemically analogous 6-N-triethyllysine internal standard were isolated from aqueous samples by microcolumn ion-exclusion chromatography. The specimens were derivatized by reaction with 1-fluoro-2,4-dinitrobenzene and reaction byproducts extracted by organic solvents. The trimethyllysine and internal standard derivatives were separated easily from other sample constituents by reversed-phase paired-ion high-performance liquid chromatography with spectrophotometric detection at 405 nm. Standard curves were linear over a sample concentration range of 10-150 nmol/ml; the detection limit corresponded with 0.1 nmol trimethyllysine injected into the chromatograph. The procedure was used for determination of trimethyllysine in human urine.

Giant hyperplastic polyp of the colon simulating adenocarcinoma.

Hyperplastic polyps of the colon have been previously reported as frequent incidental asymptomatic lesions limited to 1 cm. in size. To our knowledge, this is the first recorded case of a hyperplastic polyp of such magnitude as to cause obstructive symptoms and clinically simulate adenocarcinoma of the colon.

Adenocarcinoma arising in a sigmoid diverticulum: report of a case.

A case of adenocarcinoma arising in a sigmoid diverticulum is presented. A patient who had colonic diverticulosis was found to have one diverticular lumen distended by tumor mass. Microscopically, this was seen to be adenocarcinoma arising from the diverticular mucosa. Apparently, this is the first reported case of adenocarcinoma arising in an intact diverticulum of the colon.

Psychometric equivalence of an improved Basic Language Morningness (BALM) scale using industrial population within comparisons.

An existing 13-item composite circadian rhythm 'morningness' scale was simplified into a '(BA)sic (L)anguage (M)orningness' (BALM) scale at a seventh grade (12-13 years) reading level. It was given to 150 each of continuously rotating shifworkers and fixed-schedule dayworkers in an industrial manufacturing plant. Multiple factor analyses confirmed three prior identified factors, morningness/effort, morning alert, and evening. Morningness scores for dayworkers were higher than for shiftworkers (p < 0.001). The BALM scale revision is suitable for most worker levels. A suggested psychometrically 'purer' 9-item morningness scale omits the inconsistent evening factor scale items.

Anterior cruciate ligament reconstruction as an outpatient procedure.

With the demands of insurance companies and the need for cost containment, more and more surgeons are performing the anterior cruciate ligament reconstruction (ACL) procedure on an outpatient basis. The shorter hospital stay has increased the demands of nurses to provide appropriate nursing care, while also meeting the educational needs of patients receiving an ACL reconstruction as an outpatient. This article reviews the preoperative, intraoperative, and the postoperative phases of the care for patients undergoing anterior cruciate ligament reconstruction on an outpatient basis. It also emphasizes the role of the nurse in assessing, implementing, and coordinating care for patients having an ACL reconstruction.

Adrenal medullary fibrosis in IDDM of long duration.

We conducted a retrospective pathology study to determine whether subjects with long-standing insulin-dependent diabetes mellitus (IDDM) have abnormalities of the adrenal medulla compared with subjects with non-insulin-dependent diabetes mellitus (NIDDM) and nondiabetic individuals. Slides were scored from 0 (no fibrosis) to 3+ (complete fibrosis). Nineteen IDDM subjects aged 30-60 yr (mean +/- SE 44.9 +/- 2.5 yr) at autopsy and with duration of diabetes 13-45 yr (26.8 +/- 1.8 yr) were studied. Twelve NIDDM subjects aged 61-84 yr (73.3 +/- 2.5 yr) with duration of diabetes 17-33 yr (22.8 +/- 1.9 yr) were studied. Twenty-two nondiabetic subjects aged 32-77 yr (53.7 +/- 2.9 yr) were studied. Four of 19 (27%) IDDM subjects had moderate to severe fibrosis compared to 1 of 12 (8.3%) NIDDM subjects and 1 of 22 (4.5%) control subjects. Thirteen of 19 (68%) IDDM subjects had a score of 1, 2, or 3 compared to 3 of 22 (13.6%) control subjects (P less than .0005). There was an association between duration of IDDM and fibrosis score (r = .46, P less than .05) and between age and fibrosis score among IDDM subjects (r = .57, P = .01). No association between age or duration of diabetes and fibrosis score was observed for NIDDM or control subjects. Adrenal medullary fibrosis may be an anatomical correlate of the diminished epinephrine secretion that occurs in response to insulin-induced hypoglycemia in some IDDM subjects.

Management of Colles' fractures.

The Colles' fracture is probably the most common fracture seen in the Emergency Department. Although there are several different methods of reduction and fixation, the goal of any treatment plan is to return the patient to normal function. Depending on the complexity, the Colles' fracture can be successfully treated open or closed. This article reviews the mechanism of injury, assessment, conservative or operative treatment, and rehabilitation of these fractures.
